Role Description
This is a full-time on-site role for a Case Manager at Lakeside Assisted Living in Staten Island, NY. The Case Manager will be responsible for coordinating care plans, conducting assessments, collaborating with healthcare providers, advocating for patients, and ensuring quality care delivery.
Qualifications
Case Management, Care Coordination, and Assessments skillsExcellent communication and interpersonal skillsAbility to advocate for patients and work in a team environmentKnowledge of healthcare regulations and patient rightsBachelor's degree in Social Work, Psychology, or related field preferredExperience in a healthcare or long-term care setting is a plusFull Time and Part time positions available.
