The Care Team Specialist is also responsible for checking-out patients, becoming familiar with all self-pay and third party insurances, creating appointments for patient, providing a patient plan connecting them to needed services and ensuring all care team information was input correctly. Job DescriptionJob DescriptionPay Range: $21.00 to $29.00 per hour, Depends on Experience (DOE)The Phlebotomist is responsible for performing La Maestra Community Health Center’s routine laboratory tests in order to provide data to patients and providers for use in diagnosis and treatment of disease. Responsibilities: Interpret physicians’ orders and secure a requisition before performing testObtain the correct amount and type of specimenDraw blood from patient’s finger, heel, or vein, observing principals of asepsis to obtain blood samples.Conduct quantitative and qualitative chemical analyses of body fluids such as blood, and urine, under supervision of lab supervisorComplete appropriate routing, handling, and processing of various medical specimenPerform tests in an accurate and timely mannerInform physician of abnormal lab resultsDocument all requests and results of in-house lab work accuratelyRecognize abnormalities and notifies physician immediatelyPerforms daily quality control and cleaning procedures per guidelinesInput patients’ tests performed and results in i2i (i.e. A1C test). Job DescriptionJob DescriptionSummary: Under the direction of the Patient Services Representative Supervisor (PSRS) the Patient Service Representative (PSR) is responsible for the patient check–in process at La Maestra. The PSR is also responsible to assist the patient or his/ her representative to review and complete all necessary forms and disclosure of information via paper or electronically through the EMR or any other software being used. The position is considered a crucial link between the patient and the care delivered by clinical and service staff of the organization. The PSR must work collaboratively with their Care Team and all other clinical services staff in support of direct patient services, exhibiting flexibility and a “can do” attitude. Patient services are the key priority in this position requiring the PSR to communicate with our patient population and orient them to our organization, all with the goal of fostering an environment which promotes patient comfort and trust. The position must exemplify the mission of the organization, always exercising, empathy and respect in patient/staff interactions.Requirements:Education: High school diploma or equivalent required.Certification: CPR certified required.Experience: Prior administrative or clerical experience in clinical or community-based setting highly preferred.
