This is a great opportunity for a Peer Support Navigator H.O.M.E. to join our team. This position is a full-time position that will require some weekend and holiday hours. The Peer Support Navigator (Housing Opportunities and Meaningful Employment) serves as a bridge to ongoing services by coordinating and monitoring RBHA/Community-based resources and facilitates the delivery of these necessary services and support. The goal is to provide individuals who are voluntarily and able to give informed consent, assistance in accessing services. Meets with service recipients in the community as referred. Assists in mapping out the most appropriate next steps for the individual related to housing needs. We are looking for an individual to join our team as an Employment Specialist for our Housing Opportunities and Meaningful Employment program.
The primary duties of the Employment Specialist are to assist with promoting an “employment culture” within the organization towards meeting the employment and other service goals as dictated by Arizona Department of Behavioral Health Services. Our SMI Case Management Navigator will provide outreach and engagement and case management services. Complete assessments, service plans and implement services to consumers in accordance with their individualized service plan (ISP). Communicate and document consumer’s progress toward their recovery.
Essential Responsibilities:
Provide outreach to members not currently engaged in services, educate, communicate, and coordinate care with consumer, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented. Provide transportation to consumer as appropriate and determined by the clinical team. Perform basic case management functions associated with caseload including completing the assessment and service planning processes. We are currently looking for an On Call Clinical Coordinator (Supervisor) for our Crisis Department at our Brand New Glendale location. As a Clinical Coordinator of Crisis, you will ensure the quality of services to clients, day-to-day coordinate operations of the program, and recommend and implement changes that would improve the delivery of services. Provide direct service counseling, ongoing counseling, prevention services, and education services to adults, families, and youth. This position directly supervises clinical staff. We are looking for Peer Support Specialists to work with the Phoenix Fire BHU team. As Peer Support, for the Phoenix Fire Behavioral Health Unit, you will work as a collaborative part of the City of Phoenix Community Action Program (CAP) and the Behavioral Health Unit (BHU) to ensure community safety. In this role, you will be the Integral part of intervention, outreach, and stabilization. You will alternate levels of support with Phoenix Fire Department CAP services.
Essential Responsibilities:
Respond to dispatches to community members identified by the City of Phoenix 911 system and/or First Responders.Provide Peer Level support utilizing relatability, crisis intervention, resource connection, transportation, and coordination of care.Collaborate with Phoenix CAP staff and Management to assisting an efficient and effective program.Follow all Phoenix Fire CAP protocols and guidelines as assignedAssistance with other programmatic needs as assigned.What you’ll provide:
Excellent oral and written communication skills.Ability to remain tactful, calm, and persuasive in controversial and/or confrontational situations.Willing to report to shift at any of the identified Phoenix Fire CAP locations.Valid Arizona Driver’s License, with no major infractions in the past three years is required. If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.Education and Experience:
Openly identify as a Peer (Someone with lived experience utilizing Behavioral Health Services navigating the Behavioral Health system for a family member)1 year of Peer Support experience preferredPeer Support Certification preferredMust be 21 years of age or older at time of employment.What you’ll experience:
The typical work environment will be community based and not EMPACT facility basedEmployees must be able to lift up to 25 pounds at times.Occasional travel to training and out-of-state locations may occur as needed.

A qualified candidate will have personal experience in receiving behavioral health services, have successfully navigated through the behavioral health system, and is ready to help others on their personal journey.
This is a great opportunity for a Peer Support Specialist to join our agency in our City of Maricopa location. (The city of Maricopa, in Pinal county, is 25 miles southwest of Chandler, AZ).
The Peer Support position is a full time position which will require some evening hours. Experience working with severely mentally ill adults and the adult system of care is required. The Peer Support position serves as a bridge to ongoing services by coordinating and monitoring RBHA/Community based resources and facilitates delivery of these necessary services and support. Acts as a consumer advocate, assists consumers in the ISP process, assists in crisis intervention and outreach/engagement. The goal is to provide individuals who are voluntary and able to give informed consent, assistance in accessing services.
What you’ll provide:
Minimum of High School Diploma or equivalent.Bachelor’s degree in the related behavioral healthcare field or equivalent work experience preferred.Experience working with adult system of care is required.Peer Support Certification is required within first 90 days of employment.Must be a recipient or have received services through the community behavioral health system.Must have the ability and willingness to share their experience of receiving behavioral health services.Must have a valid AZ driver's license with no major infractions in the past three years.Must have regular access to reliable transportation.Must have a Fingerprint clearance card or be eligible to obtain one.Bi-lingual (Spanish) Diff per hour available.Must be at least 21 years of age at the time of employment.Additional Requirements and Responsibilities:
