You will advocate for consumers, connect them with resources, and ensure their treatment plans are tailored to meet their unique needs.What You’ll Do:Engage & Support – Provide care in accordance with professional standards, empowering consumers to participate in their recovery.Assess Needs – Conduct thorough evaluations and arrange specialized services when requiredDevelop & Implement Plans – Collaborate with consumers to create and monitor individualized treatment plans.Advocate for Services – Assist consumers and families in accessing benefits such as medical assistance, transportation, and education.Teach Daily Living Skills – Support budgeting, money management, and medication management.Ensure Compliance – Maintain timely and accurate documentation (including TCM-required reports)Collaborate & Report – Serve as a mandated reporter, attend meetings and training sessions, and contribute to a positive, team-oriented culture.What We’re Looking For:Education: Bachelor’s degree in social work, Psychology, Sociology, or a related fieldValid Wisconsin driver’s license, reliable transportation, and auto insurance (must meet La Causa, Inc. driving standards) –Flexibility to work evenings/weekends as program needs requireExperience: At least 1 year of experience working with adults with significant mental health needsStrong communication skills (oral and written) – Ability to collaborate effectively with adults, families, and diverse cultural/economic groupsExcellent critical thinking, organizational, and problem-solving skillsMust successfully complete background checks (including vaccination requirements)Proficiency with Microsoft Office SuiteDay-to-Day Setting Work will take place in both field and office environments, with regular local travel and occasional statewide travel required.
