Graduate of an accredited Physical Therapy programCurrent Pennsylvania Physical Therapy LicenseMinimum two years of physical therapy experience in Home Health or similar settingsExperience treating geriatric patients in a home health or similar care settingExperience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care settingExperience providing care in hospice-adjacent contextsValid Drivers License and reliable vehicleProof of Auto InsuranceStrong communication skills for effective collaboration with patients, families, and the care teamExcellent organizational abilities and experience managing a diverse caseloadEligible to work in the United StatesExperience with Matrix Care is a plus.ExperiencePhysical Therapy: 2 years (Required)
