    As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. This role is ideal for someone who is energized by one-on-one patient support and collaborative care, and who wants to contribute to better outcomes for individuals and families across the communities we serve.

    ResponsibilitiesProvide personalized rehabilitation services to patients in their homes.Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.Collaborate closely with a multidisciplinary team to support patient care and recovery.Guide patients through their recovery journey with compassion and professionalism.Help restore patients' independence and well-being through high-quality therapeutic care.

    Requirements

    Graduate of an accredited Physical Therapy programCurrent Pennsylvania Physical Therapy LicenseMinimum two years of physical therapy experience in Home Health or similar settingsExperience treating geriatric patients in a home health or similar care settingExperience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care settingExperience providing care in hospice-adjacent contextsValid Drivers License and reliable vehicleProof of Auto InsuranceStrong communication skills for effective collaboration with patients, families, and the care teamExcellent organizational abilities and experience managing a diverse caseloadEligible to work in the United StatesExperience with Matrix Care is a plus.Experience
