The Intake Coordinator assists individuals while admitting to Key Works' Long-term Substance Use Disorder Treatment program. This position is responsible for providing exceptional customer service while admitting individuals into residential care, orienting, and assisting patients in complying with program and treatment requirements consistent with all clinical policies and procedures. The Intake Coordinator completes all admission paperwork documentation in the electronic medical record and completes all aspects of checking the patient into the program. When time permits, the Intake Coordinator helps patients prepare for their discharge from Key Works.
Key Responsibilities:
Admit patients and provide support to their families during the intake process. These tasks include but are not limited to:
Completing patient intake paperwork.Greet new patients and new patient families.Help orient and familiarize patients with Key Works treatment protocols.Connect the new patient to their identified peer support. Work with appropriate staff to determine room assignments for new patients, review program guidelines, give facility tour and review medical emergency sheets within 12-hours of check in. Log all patient belongings and obtain patient’s signature. Place patient belongings in the appropriate storage space and determine what a patient can keep with them while in treatment.Record patients’ external appointments (medical, legal, etc.) in the electronic medical record so it is updated on the daily run/transportation list Collaborate with the transportation department to make sure new admissions have rides to and from appointments Collaborate with the Medication Room Coordinator when patients bring medications in at the time of admission Collaborate with Medication Room Coordinator and case management to ensure patient has continuity of care with medication providers Update the communication log with information regarding the new admission Transport patients if necessary – only use Key Works vehicles Model appropriate behavior, communication, and dress code according to Key Works policies Complete all incident reports as required Key Works procedures and policies Apply knowledge of organization’s policies and procedures to uphold these policies with patients and residential patient family members. Key Works is seeking a compassionate and skilled Dual-Credentialed Therapist to join our dynamic team. The ideal candidate will hold both licensure in mental health (LICSW, LMFT, LMHC) and shall meet SUDP/T requirements. This role involves providing clinical treatment services, conducting assessments, and developing individualized treatment plans.
Duties and Responsibilities:
Diagnose mental health conditions in accordance with the most recently published Diagnostic and Statistical Manual (e.g. DSM-5). Delivering all program services related directly to patient treatment and care as assigned by and under the supervision of the Behavioral Health Clinical Director. Oversee the daily operations of the kitchen and staff. The Kitchen Manager will be responsible for overseeing food preparation, the cooking process, ordering supplies, scheduling shifts, and monitoring inventory levels. Organize and provide a clean, orderly, kitchen and food storage space. Ensure all food products are in compliance with health and safety regulations. Education/Experience:

Minimum of 3 years' experience in a commercial kitchen work environment to include procurement/buyer experience in the Restaurant chains or a similar industry with food and beverage procurement experience.Proven culinary and management experience.Experience with volume menu development and preparation, portion control and food preservation.Experience in using MS Office including Word, Excel, Outlook, Teams.Knowledge, Skills, and abilities:

The Transportation Coordinator is responsible for safely and efficiently transporting patients in facility van or other vehicles according to schedule. Capable of quickly organizing each day’s transportation needs, working effectively with supervisor, residential and clinical staff and back-up resources to assure that each day’s transportation needs are completed and on time. Partners in-person with external medical resource staff to provide transportation assistance to patients for the purpose of receiving medical care at local medical and other patient focused support facilities.
Shift: Monday-Friday 9:00 a.m.-5:30 p.m.
Duties and Responsibilities:
Drives facility van and other vehicles with the following responsibilities: Assists in planning and implementing facility van or other vehicle schedules Promptly reports any concerns regarding safe and proper functioning of van or other vehicles to supervisor Follow all city and state driving and transportation rules and regulations Engage and communicate with patients as appropriate to address any behavior or communication issues during transportation and communicate patient complaints or concerns to management as they arise Maintain effective level of communication with supervisor and management team to report any service impacting issues Keep records of who attended offsite activity (medical, legal, etc.) and ensure that all are returned to site Assure interior of van or other vehicles is neat and clean Serve as role model for patients by displaying responsible, cooperative, positive attitude, and exhibit good patient boundaries Represents Key Recovery in a professional, courteous, friendly manner Communicates respectfully with patients, facility staff and people of all cultures and backgrounds Coordinates pick-ups and drop-offs for timely, safe, and efficient transportation.
