    POSITION OBJECTIVE/SUMMARY:

    MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION:

    A Bachelor’s or Master’s Degree in Social Work, Psychology or Human Services.A minimum of two years experience working in Human Services as Case Manager, Social Services Case Worker, Educational Psychologist or related field in a psychiatric environment working with clients who are persistently and severely mentally ill, preferred.Physically well, emotionally stable, and good communications skills, both verbal and written.Ability to work with a multidisciplinary treatment team essential.

    GENERAL STATEMENT OF FUNCTIONS:

    Under the direction of the Chief Medical Officer, provides continuing, comprehensive health wellness, preventive and medical care to adults and children. Coordinates and manages the care of these vulnerable patients and that they receive optimal care, including acute illness management, chronic disease management, and preventive care.     REQUIRED SKILLS AND QUALIFICATIONS:

    Possession of a Doctor of Medicine (M.D.) Degree and a valid license to practice medicine issued by the State of California Medical Board with no pending or previous disciplinary action from any state licensing entity. Must have board certification or eligibility in a primary care specialty (Internal Medicine or Family Medicine); current DEA license and BCLS certification. Clinical experience in a Federally Qualified Health Center (FQHC) or with medically underserved populations with progressively responsible experience in management, coordination, planning and evaluation preferred. Experience in medical administration, utilization review and quality management. Experience in implementing meaningful use indicators: familiarity with requirements and processes to become PCMH Certified.     The licensed clinical social worker (LCSW) will work to reduce the distress and improve the mental health of underserved populations presenting with complex medical and psycho-social problems. Works alongside other professionals in multidisciplinary teams in order to deal with complex patient problems.

    DESCRIPTION:

    The LCSW provides social work services including assessment, short term and crisis counseling, individualized care planning, and case coordination to patients and their families. Serves as liaison between Kedren Community Clinic and the community to integrate services, coordinate care for clients, and enhance relationships through public relations activities.Assesses, develops and coordinates a clinical treatment program for patients with acute or chronic mild to medium mental health disorders.Consults with staff regarding diagnosis, strengths and deficits of member, as needed or appropriate.

    DUTIES:

    Work as a part of a multidisciplinary team alongside medical providers, nurses, social workers, front office staff, and case managers.Provide individual and group psychotherapies and/or psychosocial rehabilitation treatment to clients with anxiety, depression, chronic pain, hypertension, medication non-adherence, addiction, social and interpersonal problems, personality disorders and challenging behaviors. Develop individualized treatment plans as part of ongoing assessment of patients and in collaboration with the case manager. Refer patients to institutions, support services, psychiatry, and other specialists as needed. Provide consultation services to multidisciplinary team members by evaluating the biopsychosocial implications of the patient’s illness when the team is assessing and planning medical treatment. Provide mental health services at satellite sites as needed. Provide functional assessments in reports to the Department of Public Social Services, Department of Child and Family Services, Social Security Administration, and the Judiciary. May provide some case management or referral to assist homeless patients with accessing vital resources such as housing, substance abuse treatment, specialty mental health care, and public benefits. Document all assessments, diagnoses, care plans, interventions, and referrals in our electronic medical records system.
