    JOB SUMMARY:

    This position is responsible for leveraging clinical nutrition expertise and patient education to support patients receiving complex infusion therapies while advancing the organization’s parenteral nutrition (PN) growth strategy. As part of the sales team, the specialist combines clinical insight with relationship-building skills to support high-quality patient outcomes while contributing to market development, provider engagement, and sustainable business growth.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    1. Call on hospitals, physicians, clinics, skilled rehabs and any ancillary referral sources to educate and assist in gaining parenteral nutrition referrals.

    2. Assist in patient education and coordination of referral management to ensure a successful on-boarding process to KabaFusion including assisting in obtaining all necessary documentation from the referral source to ensure the patient is a safe home infusion candidate and meets payer qualification to secure reimbursement.

    3. Manage and ensure intake processes for parenteral nutrition patients are compliant with ACHC and government regulations. Comply with all applicable laws and regulations.

    4. Lead and facilitate nutrition support team meetings and patient care conferences and contribute appropriate clinical information. Coordinate clinical plans of care with the nutrition support teams.

    5. Identify potential growth opportunities for parenteral nutrition. Develop and maintain customer relationships. Serve as a content expert for sales support activities which may include providing continuing education programs.

    6. Coordinate and conduct in-services and meetings with physicians, hospital discharge planners and other key decision makers

    7. Collaborate with operational or other sales counterparts on referral management, marketing reporting and various reimbursement and collections reports.

    8. Participate in national and local nutrition organizations.

    9. Maintain knowledge of current trends and research/developments in the field by reading journals, books, and other literature and attending related conferences.

    10. Provide patients with timely and accurate information and take appropriate actions regarding our services to ensure complete satisfaction.
    11. Other related duties as assigned by manager or designee.

    JOB REQUIREMENTS AND QUALIFICATIONS

    Education:

    • Bachelor’s degree in nutrition and dietetics; equivalent professional experience in a relevant industry may also be considered.

    Certifications or Licenses:

    • Registered Dietitian with current license in state of operation. Additional licensure may be required in multi-state service areas.

    • Current registration by the Commission of Dietetic Registration of the American Dietetic Association.

    Experience

    • Minimum of two (2) years’ experience with parenteral nutrition and/or home infusion required.

    • Intermediate experience with Microsoft Office including Word, Excel, PowerPoint, Outlook, Teams, and/or electronic health care record software system preferred.

    JOB SUMMARY:

    Provides safe and appropriate pharmacy services to patients in the alternate care setting in accordance with policies and procedures, professional standards, and applicable regulatory and accrediting entities. Coordinates patient services. Assesses appropriateness of therapy, dispenses medications, supplies and medical equipment, documents patient progress toward goals, intervenes as clinically appropriate and provides education regarding appropriate medication administration and use. Additional dimensions may include working with operating budget, revenue responsibility, purchasing responsibility, etc.

    Ensures company’s goals and objectives are aligned with the organization’s broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager or Director of Pharmacy or Operations based on branch leadership.


    MAJOR DUTIES AND RESPONSIBILITIES:

    1. Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.

    2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.

    3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.

    4. Recommends and writes patient-appropriate orders.

    5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.

    6. Assesses laboratory results and performs pharmacokinetic dosing.

    7. Makes appropriate dosing recommendations and documents.

    8. Initiates, creates and updates patient medication profiles.

    9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.

    10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.

    11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.

    12. Accurately submits electronic claims for medications, as applicable.

    13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.

    14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.

    15. Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.

    16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP <797> standards and company policies and procedures by all pharmacy staff.

    17. Ensures required documentation is completed on QA/QC records and compounding documents.

    18. Completes company-required orientation and annual competencies and in services.

    19. Provides concise, applicable and timely responses to requests for drug information.

    20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates

    21. Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient's need.

    22. Plans work to ensure cost-effective delivery methods are utilized.

    23. Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.

    24. Delegates non-clinical tasks to assigned support personnel.

    25. Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.

    26. Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.

    27. Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.

    28. May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.

    29. Performs other related duties as directed by supervisor.

    SKILLS AND ABILITIES:

    • Excellent verbal and good written communication skills.

    • Advanced level of multi-tasking and problem-solving skills.

    • Moderate computer skills including MS Office, Windows Outlook, and Internet.

    • Moderate level of interpersonal skills to interact effectively and professionally with both internal and external customers.

    • Moderate mathematical skills to perform pharmaceutical calculations.

    • General knowledge of modern office procedures, filing, telephone techniques and office equipment.


    EDUCATION AND/OR EXPERIENCE:

    1. Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master’s of Science degree in Pharmacy desirable) from an accredited college of pharmacy.

    2. Currently licensed as a registered pharmacist in state of employment.

    3. A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy

    4. Experience and strong knowledge base in intravenous admixture services and intravenous therapy.

    5. Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.

    6. Must be able to work effectively with others to provide safe and effective drug therapy.

    7. Must demonstrate ability to problem solve and supervise technical and other support staff.

    8. Must be flexible with work schedule.

    9. Good organizational and time-management skills.

    10. Participation and membership in professional societies and organizations is desirable.

    11. Excellent verbal and written communication skills.

    JOB SUMMARY:

    Assists the pharmacist(s) in storing, preparing, dispensing, reconciling, and charging of all medication to patients serviced by the pharmacy. Assists the pharmacist(s) in maintaining an adequate drug inventory and quality control procedures. The Pharmacy Technician shall work under the direct supervision of a licensed registered pharmacist and shall only perform activities that are in compliance with the State Board of Pharmacy practices. Ensures company's goals and objectives are aligned with the organization's broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager.


    MAJOR DUTIES AND RESPONSIBILITIES:

    1. Assists pharmacist in organizing the pharmacy workload.

    2. Performs prescription/order data entry, generates prescription labels, compounding documents and delivery slips under the direct supervision of a pharmacist.

    3. Compounds and otherwise prepares pharmaceuticals, supplies and equipment under the direct supervision of a pharmacist as allowed by state and federal law and in accordance with USP 797, company policies and accrediting entity standards.

    4. Cleans, tests, maintain and track pumps and other infusion equipment as requested.

    5. Performs and records various quality control activities including air sampling, bacterial counts, and Steri-Testing. Reports all “positive” results to the pharmacy supervisor immediately.

    6. Ensures the clean room suites and related equipment are cleaned and maintained per organizational policy and procedure, USP 797, USP 800 and state and federal regulations.

    7. Completes KabaFusion orientation and annual competencies and mandatory inservices.

    8. Updates patient's demographic information in the electronic information management system as needed and ongoing.

    9. Answers/triages telephone calls and provides non-clinical assistance to callers as appropriate or directs callers to appropriate department.

    10. Proactively manages patients' medication/supply/equipment inventory needs by communicating with patient or responsible party prior to filling anticipated needs. Maintains refill/follow-up schedule/calendar as appropriate.

    11. Assures authorization to receive payment for services rendered is in place prior to shipping medications, supplies and equipment to patients.

    12. Track and retrieve patient laboratory results for pharmacists and fax results to prescribers and other healthcare providers involved in a patient's care.

    13. Documents all communication with physicians. Referral sources, nurses, patients and caregivers in the electronic information management system as required.

    14. Assists pharmacy staff in scheduling, coordinating and filling patient shipments and deliveries.

    15. Sorts and distributes faxes. Faxes correspondence to prescribers. Home health agencies and others as applicable.

    16. Assists pharmacy manager and staff in handling a wide range of administrative functions and projects.

    17. Assists in maintaining pharmacy files and records in an accurate and efficient manner.

    18. Assists with managing and ordering patient start of care forms and other pharmacy forms.

    19. Generates and distributes quality assurance and management reports as assigned.

    20. Communicates any and all nurse. Patient and caregiver clinical concerns and questions to the pharmacist, pharmacy manager or other appropriate clinical staff.

    21. Prepares and places orders for needed drugs, supplies and equipment, and stock/maintain such materials upon receipt.

    22. Actively participates in the KabaFusion Performance Improvement (Pl) program

    23. Provides support to other local office departments as needed.

    24. Ensures required documentation is completed on QA/QC records and compounding documents.

    25. Completes company required orientation and annual competencies and in services.

    26. Plans work to ensure cost-effective delivery methods are utilized.

    JOB SUMMARY:

    Responsible for coordinating the daily warehouse functions of receiving medications, supplies and medical equipment in a cost-effective and timely manner. Follow State BOP regulations and company policies for handling, storage and disposal of medications and waste. Ensures medications are packaged to meet appropriate shipping and temperature storage conditions. Works under the direct supervision of the Center Manager/Director of Operations and performs clerical, equipment and delivery activities as required. Assist the pharmacy department and local office staff as needed.


    MAJOR DUTIES AND RESPONSIBILITIES:

    1. Supervises all incoming and outgoing deliveries for accuracy.

    2. Coordinates the daily receiving of medications. supplies and equipment from distributors and suppliers.

    3. Checks all medication and supply packages for damage, tampering, and expiration dates.

    4. Ensures the proper handling, rotation and storage of all medications and supplies in warehouse.

    5. Assigns and/or adjusts product locations and documenting them in CPR+.

    6. Stock replenishment of quantities and lots through company purchase orders (PO). Process P.O. with matching packing slips per local office procedure.

    7. Handles sharps, bio-hazardous, cytotoxic, hazardous materials and pharmaceutical waste in accordance with federal and state regulations and company policies and procedures.

    8. Monitors min/max shelf quantities and documents on ordering log.

    9. Restock picking locations daily from over stock areas.

    10. Assembles shipping supplies.

    11. Performs medical equipment cleaning and testing as required.

    12. Data entry management of new, current or returned serialized equipment.

    13. Contacts patients as needed to coordinate pick up of equipment.

    14. Ensures completion of daily route sheets.

    15. Enters shipping data into common carriers' computer systems and tracks shipments as needed.

    16. Performs monthly generator and fire extinguisher checks.

    17. Assists manager with performing quarterly building checks not limited to but including first aid kit,

    eye wash stations, lighting, disaster kit etc.

    18. Perform monthly and quarterly cycle counts with pharmacy department.

    19. Follows all safety regulations and legal documentation pertaining to warehousing.

    20. Works with Patient Service Representatives to schedule pickup of infusion pumps from active and inactive patients.

    21. Monitors pumps while they are rented out to patients for annual maintenance.

    22. Inputs newly rented infusion pumps into CPR+ and sends returned pumps back to our rental companies.

    23. Assists management with ordering functions using CPR+.

    24. Performs other related duties as directed by management.

    25. Other duties as assigned.

    SKILLS AND ABILITIES:

    1. Good written and oral communication skills (multi-lingual desired).

    About the role:

    Our Nursing Coordinators have direct interaction with our patients and are the first voice they hear from our nursing department. In this role you will coordinate and schedule patient visits, manage patient charts, gather important documentation, and interact with providers as needed.

    Essential Duties & Responsibilities:

    Obtains incoming outsourced agency nursing notes and invoices to match up dates and times, enters Patient Care Management System and forwards to QA Nurse for review. Enters all scheduling data into the computer system in a timely manner and ensures that data is accurate and complete. Assists with sending out physician plan of treatment and free form orders for signature and tracks for return, works with sales representatives to obtain timely signature as needed.

    As a Nursing Coordinator, you bring:

    JOBSUMMARY:

    This position is responsible driving Chronic and IVIG patient referrals within a defined territory. This role requires close collaboration with cross-functional teams including pharmacy, intake, nursing, and operations. In addition, this position is accountable for achieving net revenue and profit goals, expanding market share, and supporting sales and marketing initiatives. Key customers include clinics, physicians, and payors.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    1. Cultivate and manage relationships with key referral sources, including clinics, physicians, and payors, to generate Chronic and IVIG patient referrals.

    2. Collaborate with pharmacy, intake, nursing, and operations teams to ensure seamless service delivery and customer satisfaction.

    3. Achieve net revenue and profit (EBITDA) goals by expanding market share and securing new business opportunities.

    4. Participate in sales meetings, training sessions, and in-service presentations to support business development efforts.

    5. Analyze territory performance and develop strategic plans aligned with company objectives.

    6. Address and resolve customer service issues promptly, ensuring high levels of client satisfaction.

    7. Support managed care initiatives and assist with pricing submissions in coordination with sales leadership.

    8. Maintain accurate and timely documentation, including call reports, expense reports, and other required submissions.

    9. Develop and apply knowledge of relevant disease states, drug therapies, and reimbursement policies.

    10. Assist in training staff on customer service best practices and contribute to continuous improvement initiatives.

    11. Other related duties as assigned by supervisor or designee

    JOB REQUIREMENTS AND QUALIFICATIONS

    Education:

    • Bachelor’s degree preferred, ideally in business, healthcare, or a related field. Equivalent work experience will be considered.

    Experience:

    • Proven experience in business development and customer service is required.

    • Minimum of two (2) years in infusion services or a related healthcare field preferred.

    • Familiarity with reimbursement policies preferred.

    Skills and Competencies:

    • Strong ability to sell healthcare services.

    • Knowledge of reimbursement processes preferred.

    • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, SharePoint).

    • Excellent planning, organizational, and time management skills.

    JOB SUMMARY:

    Provides safe and appropriate pharmacy services to patients in the alternate care setting in accordance with policies and procedures, professional standards, and applicable regulatory and accrediting entities. Coordinates patient services. Assesses appropriateness of therapy, dispenses medications, supplies and medical equipment, documents patient progress toward goals, intervenes as clinically appropriate and provides education regarding appropriate medication administration and use. Additional dimensions may include working with operating budget, revenue responsibility, purchasing responsibility, etc.

    Ensures company’s goals and objectives are aligned with the organization’s broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager or Director of Pharmacy or Operations based on branch leadership.


    MAJOR DUTIES AND RESPONSIBILITIES:

    1. Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.

    2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.

    3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.

    4. Recommends and writes patient-appropriate orders.

    5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.

    6. Assesses laboratory results and performs pharmacokinetic dosing.

    7. Makes appropriate dosing recommendations and documents.

    8. Initiates, creates and updates patient medication profiles.

    9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.

    10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.

    11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.

    12. Accurately submits electronic claims for medications, as applicable.

    13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.

    14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.

    15. Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.

    16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP <797> standards and company policies and procedures by all pharmacy staff.

    17. Ensures required documentation is completed on QA/QC records and compounding documents.

    18. Completes company-required orientation and annual competencies and in services.

    19. Provides concise, applicable and timely responses to requests for drug information.

    20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates

    21. Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient's need.

    22. Plans work to ensure cost-effective delivery methods are utilized.

    23. Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.

    24. Delegates non-clinical tasks to assigned support personnel.

    25. Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.

    26. Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.

    27. Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.

    28. May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.

    29. Performs other related duties as directed by supervisor.

    SKILLS AND ABILITIES:

    • Excellent verbal and good written communication skills.

    • Advanced level of multi-tasking and problem-solving skills.

    • Moderate computer skills including MS Office, Windows Outlook, and Internet.

    • Moderate level of interpersonal skills to interact effectively and professionally with both internal and external customers.

    EDUCATION AND/OR EXPERIENCE:

    1. Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master’s of Science degree in Pharmacy desirable) from an accredited college of pharmacy.

    2. Currently licensed as a registered pharmacist in state of employment.

    3. A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy

    4. Experience and strong knowledge base in intravenous admixture services and intravenous therapy.

    5. Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.

    6. Must be able to work effectively with others to provide safe and effective drug therapy.

    7. Must demonstrate ability to problem solve and supervise technical and other support staff.

    8. Must be flexible with work schedule.

    9. Good organizational and time-management skills.

    10. Participation and membership in professional societies and organizations is desirable.

    11. Excellent verbal and written communication skills.

    12. Dependable in attendance and job performance.

    JOB SUMMARY:

    Assists the pharmacist(s) in storing, preparing, dispensing, reconciling, and charging of all medication to patients serviced by the pharmacy. Assists the pharmacist(s) in maintaining an adequate drug inventory and quality control procedures. The Pharmacy Technician shall work under the direct supervision of a licensed registered pharmacist and shall only perform activities that are in compliance with the State Board of Pharmacy practices. Ensures company's goals and objectives are aligned with the organization's broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager.


    MAJOR DUTIES AND RESPONSIBILITIES:

    1. Assists pharmacist in organizing the pharmacy workload.

    2. Performs prescription/order data entry, generates prescription labels, compounding documents and delivery slips under the direct supervision of a pharmacist.

    3. Compounds and otherwise prepares pharmaceuticals, supplies and equipment under the direct supervision of a pharmacist as allowed by state and federal law and in accordance with USP 797, company policies and accrediting entity standards.

    4. Cleans, tests, maintain and track pumps and other infusion equipment as requested.

    5. Performs and records various quality control activities including air sampling, bacterial counts, and Steri-Testing. Reports all “positive” results to the pharmacy supervisor immediately.

    6. Ensures the clean room suites and related equipment are cleaned and maintained per organizational policy and procedure, USP 797, USP 800 and state and federal regulations.

    7. Completes KabaFusion orientation and annual competencies and mandatory inservices.

    8. Updates patient's demographic information in the electronic information management system as needed and ongoing.

    9. Answers/triages telephone calls and provides non-clinical assistance to callers as appropriate or directs callers to appropriate department.

    10. Proactively manages patients' medication/supply/equipment inventory needs by communicating with patient or responsible party prior to filling anticipated needs. Maintains refill/follow-up schedule/calendar as appropriate.

    11. Assures authorization to receive payment for services rendered is in place prior to shipping medications, supplies and equipment to patients.

    12. Track and retrieve patient laboratory results for pharmacists and fax results to prescribers and other healthcare providers involved in a patient's care.

    13. Documents all communication with physicians. Referral sources, nurses, patients and caregivers in the electronic information management system as required.

    14. Assists pharmacy staff in scheduling, coordinating and filling patient shipments and deliveries.

    15. Sorts and distributes faxes. Faxes correspondence to prescribers. Home health agencies and others as applicable.

    16. Assists pharmacy manager and staff in handling a wide range of administrative functions and projects.

    17. Assists in maintaining pharmacy files and records in an accurate and efficient manner.

    18. Assists with managing and ordering patient start of care forms and other pharmacy forms.

    19. Generates and distributes quality assurance and management reports as assigned.

    20. Communicates any and all nurse. Patient and caregiver clinical concerns and questions to the pharmacist, pharmacy manager or other appropriate clinical staff.

    21. Prepares and places orders for needed drugs, supplies and equipment, and stock/maintain such materials upon receipt.

    22. Actively participates in the KabaFusion Performance Improvement (Pl) program

    23. Provides support to other local office departments as needed.

    24. Ensures required documentation is completed on QA/QC records and compounding documents.

    25. Completes company required orientation and annual competencies and in services.

    26. Plans work to ensure cost-effective delivery methods are utilized.

    27. Performs other related duties as directed by supervisor.

    EDUCATION AND/OR EXPERIENCE:

    • Education or experience equivalent to a high school diploma is required.

    • Current and violation free Pharmacy Technician Licensure or Certification as required by State law in the state requesting employment.
