The RNP/PA will be offering routine medical care in an outpatient setting for the residents of the Recuperative Care Center. Position Purpose:
Under the direction of the Chief Medical Officer, and in compliance with the agency’s standardized procedures, the Nurse Practitioner (NP) is responsible for providing high-quality, patient-centered care to patients of JWCH, Wesley Health Centers. The NP will deliver primary and preventive healthcare services, including patient assessments, diagnosis, treatment, and management of acute and chronic conditions. The role involves working collaboratively with a multidisciplinary team to ensure comprehensive care that addresses the medical, social, and emotional needs of patients. The NP will also engage in health promotion, patient education, and disease prevention activities to improve overall community health outcomes.
Key Responsibilities:
Provide direct patient care, including performing physical examinations, obtaining and reviewing medical histories, identifying diagnoses, and developing and implementing appropriate treatment plans.Conduct initial intake assessments and ongoing evaluations; monitor, reassess, and manage acute and chronic medical conditions, including addressing newly identified health concerns.Diagnose and treat acute illnesses, infections, and injuries, and continue treatment plans initiated by hospitals, emergency departments, or primary care providers.Order, perform, and interpret diagnostic tests (e.g., laboratory tests, EKGs, imaging), and ensure timely follow-up and evaluation of abnormal results.Educate and counsel patients on disease management, treatment plans, medications, potential side effects, and interactions, using appropriate tools and methods tailored to individual needs.Ensure informed consent is obtained for assessments and treatments, and provide clear, comprehensive education related to care and follow-up instructions.Coordinate care with primary care providers, specialists, and interdisciplinary teams to support optimal patient outcomes; participate in care conferences and discharge planning.Facilitate referrals and collaborate with internal and external partners, including Housing for Health teams, and review referral outcomes as appropriate.Provide specialized, patient-centered care to designated populations, including individuals experiencing homelessness; identify barriers to care and housing, and support goal setting and care planning.Utilize motivational interviewing, de-escalation techniques, and supportive communication to enhance patient engagement and adherence to care plans.Provide training and education to clinical support staff, including community health workers and nursing attendants, as needed.Identify operational or clinical workflow challenges, training gaps, or collaboration issues, and communicate them to leadership.Participate in quality assurance activities, including periodic review of referrals, abnormal findings, and follow-up processes.Maintain compliance with all applicable laws, regulations, and professional standards, including the California Nurse Practice Act.Maintain current clinical knowledge through continuing education, training sessions, and professional development activities.Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.Participate in required safety and compliance trainings and apply learned practices in daily work activities.Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.Participate in JWCH’s customer service training called AIDET on an annual basis.Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions.Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes.Performs other duties as assigned.Qualifications & Requirements:

Position Purpose:
Under the direction of the Program Manager, the Care Team Specialist will be participating in a multi-disciplinary team, contributing to client engagement, retention, safety, and support. The position will interact with staff and clients as a care team member by building rapport and fostering a sense of community.
The Pathway Home program is an Interim Housing program that assists homeless, individuals to recover and improve their quality of life. These individuals may also suffer from mental health and substance abuse issues, sometimes causing behaviors such as yelling, cussing, and outbursts of anger. The role of each CTS staff at DTLA is to assist with safety of each residence. Each CTS during shift is expected to use our electronic tagging system to check all perimeters of the building including the top floors.
Program Population:
The Pathway Home Housing Project is a HFH funded program, operated by JWCH. The Team consist of a cohesive team of MSW, case managers, site monitors and other professionals who collaborate to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. The clients/residents may come with behavioral and/or substance abuse issues. The program utilizes the Harm Reduction Model and we “do whatever it takes” to encourage success of the client.
Principal Responsibilities:
Performs job functions as prescribed by established policies and procedures within defined scope of education, training, and responsibilities.Establish a positive rapport with clients and express a concern for their well-being.Demonstrate understanding and practice harm reduction methodsDemonstrate understanding of trauma-informed careDemonstrate non-violent restraint methods, if needed and only as an absolute last resortMonitor the floors by walking corridors as assigned, and or being rotated.Maintain a front desk sign in/out sheet for clients, as well as visitors. The Non-Emergent Transportation Driver will provide safe, reliable transportation services to individuals at risk of or currently experiencing homelessness in the Downtown Skid Row area. The driver will operate on a fixed route, transporting passengers to Wesley Health Centers and partner agencies such as LA Christian Health Centers, County Department of Health Services sites, and LAC+USC Hospital outpatient clinics and emergency department. Passengers will be ambulatory individuals seeking access to healthcare and social services.
Principal Responsibilities:
Safely transport individuals at risk of or experiencing homelessness to and from Wesley Health Centers and partner agency sites.Ensure compliance with all traffic laws, vehicle regulations, and safety protocols, prioritizing the well-being of passengers.Conduct daily vehicle inspections, including checking fluid levels, tire pressure, lights, and overall vehicle condition, and maintain a daily checklist.Refuel, clean, and maintain the vehicle as needed, including arranging weekly vehicle washes.Collect demographic information from passengers to accurately complete the daily ridership log.Compile and submit transportation data for monthly reporting.Assist with other office duties as assigned by management, contributing to the smooth operation of the transportation service.Provide assistance to passengers with limited mobility, ensuring safe entry and exit from the vehicle, and securing seat belts or mobility aids when necessary.Recognize signs of medical distress or behavioral crises and promptly report incidents or seek appropriate help, such as alerting medical or emergency services if needed.Collaborate with supervisors to plan efficient routes and adjust to road conditions, traffic, or special needs of passengers.Accurately document and report any incidents involving the vehicle, passengers, or any traffic violations in a timely manner.Other duties as assigned.Requirements:
