Practices medical care for each patient visit. Completes pertinent history and physical exam. Documents chief complaint, history of present illness, pertinent review of systems, and physical exam in electronic health record ( EHR ). Reviews and updates past medical history, family history, social history, medications, and allergies in EHR. Reviews vital signs. Reviews results and reports from prior orders. Places new orders. Performs and documents procedures. Administers medications and immunizations. Determines and documents assessment and plan. Prescribes medications. Provides patient education, guidance, and counseling. Arranges plan for follow-up. Signs EHR encounter within timelines in established policies and procedures. POSITION SUMMARY
The physician provides day-to-day management and medical care to the patients of the Center and those in hospital.
The physician is responsible to the Chief Medical Officer for exercising each of several functions described in this position description and is accountable, through the Chief Medical Officer, for the delivery and quality of the medical care services provided to patients at the Center and its satellites. The physician will serve as an important part of the management team.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Administrative Responsibilities:
Serve as staff physician for the Center and its satellites.Assists with the establishment of health care standards and protocols for each service described in the Center s health care plan and to ensure quality of service; to assist with development of the Center s health care plan with the assistance of staff to ensure that all goals and objectives are outlined.Be responsible for the day-to-day management of the patients seen in the manner that ensure effective and efficient performance.Operate within the parameters of a written position description that complies with the Center s policies, rules and regulations. Establishes appropriate systems to assure quality of patient care.See that the Quality Assurance Protocols, HIPAA, Corporate Compliance and CLIA standards are adhered to. Establishes appropriate systems to assure the quality of patient care.Maintains all Federal initiatives pertinent to patient care and participates in the process of budget development and in financial management decisions as they affect clinical activities.Participates regularly in administrative meetings as a member of the Center s management team.

JOB SUMMARYThe (Physician / NP / PA) is a key member of the Cooperative Health Center's multi disciplinary primary care and behavioral health care teams. Primary, preventive, and episodic primary and behavioral health care is compassionately delivered to promote health and wellness. The clinical program is divided into units that foster a team approach to care. The Nurse Practitioner will assume an active role at the site becoming involved with quality improvement activities, meetings, and other team building efforts.

Staff Physician
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
The Staff Physician works as part of the medical provider team providing medical services to the patients of the health center. In addition, Staff Physicians, unless specifically exempted by the Chief Medical Officer, shall be qualified to serve on the active Medical Staff of a local hospital, provide inpatient care services, assume "on call" after hours responsibilities and supervise health center PA's or NP's as appropriate.
SPECIFIC DUTIES AND RESPONSIBILITIES:
Provides primary medical care by caring for all patients in accordance with the physician's medical specialty.Develops a plan of care for each patient, including: complete medical history, physician examination, diagnosis, appropriate treatment and/or referral, including hospitalization where necessary.Stresses the importance of preventive health care measures.Uses all available resources in diagnosis and treatment, such as laboratory and radiological testing, in an appropriate and cost efficient manner.Confers with consulting physicians, nurse, patients, and patients families concerning treatment and care of patients.Refers those cases which require specialist services, but maintains responsibility, assuring that continuity of care is provided.Participates in the development and implementation of a regularly up-dated quality assurance plan for the consortium.Participates in the Health Promotion/Disease Prevention activities required by the U. S. Public Health Service.Attends all medical staff meetings and participates in the health center's Q/A program.Performs other necessary duties as required by the health center to meet the goals of providing primary health care.
REPORTING RELATIONSHIPS:
The Staff Physician reports to, is supervised by and evaluated by the Chief Medical Officer. The Staff Physician may be asked to serve as Physician in Charge at a particular clinic site and as such will supervise the medical team at that site. The Staff Physician may also be asked to supervise a PA or NP.
EVALUATION:
The evaluation of work performance will be ongoing, and will be carried out by the Chief Medical Officer. It will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will normally be performed annually, but may be initiated at other times by the Staff Physician or the Chief Medical Officer.
QUALIFICATIONS:
Education/Training: Medical Doctorate Degree from an accredited US institution and appropriate licensure required. Successful Completion of Family Medicine Residency Training Program.Certification: Board Certified/Board Eligible Experience: Preferred but not mandatory.Ability and willingness to serve on the active medical staff of a hospital used by HCPSC
License to practice medicine in the State of South Carolina.
The ability to relate with warmth and effectiveness to the patients and other health care providers of the health center.
Must be qualified in Basic Life Support techniques.
Ability and willingness to serve on the active medical staff of a hospital used by the health center if required.

SALARY:
The salary for this position is negotiated between the individual and the Chief Executive Officer using the approved salary scale of the organization as a basis.
