    MAINTENANCE POSITION
    Join our team at Jan Werner Adult Day Care!
    We are seeking a dependable, hardworking individual to help maintain our facility, fleet vehicles, and grounds. This position plays an important role in keeping our center safe, functional, and welcoming for staff and participants.

    Duties:
    • Service brakes and perform routine maintenance on fleet vehicles, including cars and lift-equipped vans
    • Provide light building maintenance, repairs, and upkeep
    • Maintain grounds, including yard work and snow removal
    • Perform all other duties as required to support the facility and program

    Minimum Qualifications:
    • Good moral and social characteristics
    • Good mental and physical health
    • Basic knowledge of vehicle maintenance and troubleshooting
    • Aptitude for learning new skills
    • Ability to work under supervision and follow instructions
    • Ability to work cooperatively with others
    • Pleasing manner and good personality characteristics

    Other Details:
    • Able to lift up to 100 lbs.
    • Full-time, part-time, or temporary positions may be available

    Job Summary:
    Jan Werner Adult Day Care is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated team. The Treatment/Medication Nurse will provide high-quality nursing care under the supervision of the Director of Clinical Services and collaborate with our team to ensure exceptional participant care. This role requires a professional who demonstrates leadership, communication skills, and a deep understanding of gerontological nursing.

    Responsibilities:

    Administer medications and nursing treatments while supervising non-professional staff to ensure quality participant care. Monitor and document the administration and effectiveness of self-administered medications. Maintain accurate medical records, including progress notes, daily charting, and physician orders for medications and treatments. Order and manage nursing and participant supplies. Stay up-to-date on state requirements and operating standards for healthcare facilities. Communicate participant health changes to team members and assist with coordinating 24-hour care delivery. Ensure compliance with medical record documentation standards.

    Minimum Qualifications:

    Graduate of an accredited school of Vocational Nursing. Active nursing license in the state of Texas. Minimum of one year of clinical experience with direct patient care. CPR Certification Demonstrated leadership, creativity, integrity, and strong communication skills. Understanding of the organization and functions of healthcare institutions in the community. Knowledgeable in gerontological care with the ability to relate to aging individuals. Flexibility, tact, sensitivity, creativity, and a sense of humor are essential.

    GENERAL DESCRIPTION
    The Participant Nurse Manager promotes and maintains the health of enrolled participants through nursing assessment, treatment, teaching and counseling within an Interdisciplinary Care team setting. Responsible for completing enrollment and periodic assessments of health needs to contribute to the individual s plan of care. Responsible for the development and implementation of nursing care plan and coordination of nursing with other services.

    RESPONSIBILITIES:

    Using all information sources available, performs and documents initial nursing assessment for prospective enrollees, develops and implements nursing care plan.
    Completes assessments for semi-annual care planning according to regulatory requirements and as condition change indicates. Provides skilled nursing care and prescribed treatments to participants, primarily in the day center, which include nursing treatments, medication administration, diagnostic and preventive measures and other services within the scope of a professional registered nurse. Provides both routine and episodic nursing care according to the care plan. Counsels and teaches participants and their families toward self-help and increased independence in medical support and preventive activities. Ensures adequate clinical supplies and equipment, including oxygen. Ensures the continuation and coordination of 24 hour care delivery and clinical care programs which includes, medical services, implementation of appropriate hygiene practices for participants and dietary requirements, keeping in mind cultural and personal preferences when possible. Provides on-call coverage to troubleshoot, advise, teach, coordinate and deliver care. Attends daily staff meetings and weekly clinic meetings and communicates participant changes to other staff members and participates in the development and revision of participant care plans in cooperation with the interdisciplinary team. Participates in family meetings as required. Coordinates with ancillary providers including dentist, podiatrist, pharmacist, audiologist and ophthalmologist and required specialty services to arrange appointments and share pertinent information. Completes and ensures completion of documentation of clinical treatment and services in the medical record. Meet documentation requirements in the medical record and the timeframe for the completion of each assessment and plan of care.

    MINIMUM QUALIFICATIONS
    EDUCATION/EXPERIENCE

    A graduate of an accredited nursing program with a license to practice in the State of Texas as a Registered Nurse. Must have one year of experience in working with a frail elderly population

    KNOWLEDGE/SKILLS/ABILITIES

    Must have integrity, good judgment, initiative, ability to adapt to change, ability to problem solve and function independently. A mature individual with a strong knowledge base and keen understanding and appreciation of the elderly. Must have strong inter-personal skills and be able to communicate effectively with other staff, other disciplines and representatives from outside agencies. Must be flexible and able to work collaboratively with other departments. Possess leadership abilities and attitude with the ability to delegate responsibility as well as accept supervision in an appropriate manner.
