    POSITION SUMMARY

    The Volunteer Coordinator is responsible for supporting and coordinating signature Volunteer programs and activities to support the mission of Iris House in New York. The Volunteer Coordinator works with individual volunteers as well as corporate and community groups in coordination with the needs and operations of Iris House, under the supervision of the Director of Food and Nutrition Services.

     

    ESSENTIAL POSITION FUNCTIONS

     

    1.    Maintain volunteer work records

    3.    Organize volunteer recognition events and activities

    4.    Recruits and energizes volunteers

    5.    Management of volunteer relationships

    6.    Volunteer scheduling

    7.    Communicates with volunteers

    8.    Keeping volunteers informed

    9.    Maintains volunteer management software system

    10.  Organize volunteer training sessions

    11.  Volunteer applications

    12.  Volunteer appreciation

    13.  Coordinate activities to thank volunteers

    14.  Develop programs and projects for volunteers

    15.  Generate reports about volunteer work

    16.  Lead volunteers through orientation and training

    17.  Managing and organizing the volunteer database

    OTHER POSITION FUNCTIONS

    ·        Provide operational support for the Meal Program including serving meals to families, inventorying, and putting away donations

    ·        Identify programs which volunteers are needed to educate the community and clients (i.e. home garden techniques).

    ·        Coordinate and collaborate with internal teams to understand and determine current and changing needs of the organization; prepare projects for volunteer groups

    ·        Provide timely communication to the volunteer team and other internal stakeholders as needed related programs and key projects

    ·        Create monthly volunteer newsletter

    ·        Support recognition strategies for groups supporting the House; ranging from sending daily Thank You emails to supporting major events (e.g. Volunteer Recognition Awards dinner)

    ·        Assist with annual volunteer fundraising events and other organization-related events

    ·        Serve meals as needed

    ·        Welcome new and returning volunteer groups to the House; lead tours, encourage and thank them, keep them connected to the mission through knowledge sharing on impact

     

    1.    Shares on-call status with other staff members.      

     

    2.    Demonstrates teamwork and open communication.

     

    3.    Performs other related duties as assigned.

     

     

    POSITION REQUIREMENTS

     

    Education:                           Associate’s degree in Human Services or a related field or equivalent educational and work experience.

     

    Experience:                              Two (2) or more years of related experience. 

     

    Other Requirements:            Excellent verbal and written communication and organizational skills.

    Internal:                              Frequent contact with Iris House clients and department staff members; occasional contact with visitors.

    POSITION SUMMARY

     

    Under the direction of the Program Manager, the Case Manager is accountable for providing intensive case management and crisis intervention services for clients and their families and other individuals identified as their support system.  The incumbent advocates aggressively for clients to obtain a full-range of needed services and ensures that the services received are integrated with other services provided by Iris House.  She/He prepares comprehensive intakes and assessments/reassessments, develops individualized service plans and monitors clients’ progress towards meeting their mutually-agreed upon goals.  She/He makes appropriate referral internally or externally for other needed services. 

     

     

    ESSENTIAL POSITION FUNCTIONS

     

    1.     Maintains a full-time caseload expected to be no more than 20 clients inclusive of their children and self-identified family or support members.  

     

    2.     Prepares comprehensive intake, service plans and assessments/reassessments of clients, their families and/or support members to effectively evaluate their needs; takes appropriate action to initiate needed services and prepares mutually-agreed upon service plans for clients. Needs assessments include but are not limited to physical health (including HIV/STI), mental health, substance use, dental health, tobacco use, education, employment, domestic violence and trauma.  

     

    3.     Case Manager will work with client to produce a service plan every sixth month’s with well-developed goals and progress markers and will use motivational interviewing and other evidenced-base practices to engage the client. Will utilize recovery oriented and trauma informed case management intervention based on client sand community needs and will work to engage the client in the community.

     

    4.     Ensures all client documentation is completed on time and is accurate and meets the contract provider’s requirements.

     

    5.     Monitors clients to assess their progress towards meeting their service plan goals, to offer support and guidance when needed and to assist the client in meeting their desired goals.  Promotes clients’ compliance with their medical appointments, medications and monitor possible side effects of the medication.

     

    6.     Provides quality case management services in a manner that expedites meeting the contract provider’s requirements, the program’s outcomes and Iris House’s goals and objectives.  

     

    7.     Provides crisis intervention services by assessing the client’s needs and providing the most appropriate solutions based on assessment of their immediate needs.  Assists in managing the agencies’ response to medical and personal emergencies such as the need for hospitalization, threatened suicide or physical abuse.

     

     8.   Establishes relationships or linkages with other community-based providers to ensure services are accessible to clients when needed. 

     

    9.    Collaborates with other agencies and community resources to refer clients for treatment and/or appropriate services when those services are not available at Iris House.  Collaborates with other Iris House departments to ensure that services are fully integrated and that services are not duplicated.

     

    10.   Schedules weekly home visits to assess the needs of the client and her or his family members or domestic partner and children when appropriate.  

     

    11.   Attends and participates in weekly team meetings and/or case conferences; meets with the director for weekly supervision.

     

    12.   Participates in creating and maintaining a workplace that values diversity and is free of racism, sexism, heterosexism and other discriminatory practices.

     

    13.   Keeps abreast of the latest development in relevant case management issues and concerns by conducting research on, reading available written resource materials, attending appropriate meetings and workshops and engaging in other useful methods to maintain up-to-date awareness of practices that can improve client well-being and programmatic outcomes.
