Job Description This Community Health Center is an award winning nationally recognized leader in community health care, and the third largest community health center network in Illinois, seeking a Pediatrician to join their team. Their staff of over 1,000, including health care providers, social workers, educators, counselors and advocates, is all committed to serving more than 70,000 patients and clients annually across 19 health centers throughout Chicago and the counties of Cook, Iroquois, Kane, Kankakee, Stark, Vermilion and Will. Job Description This Community Health Center seeks a Family Medicine Physician to provide a continuum of care that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). This Community Health Center is at the forefront of expanding access to affordable, comprehensive healthcare. We understand that we must continually work to evolve our thinking around patient care. As a community health care leader, we know that regardless of all the resources and advancements in medicine today, patient engagement is the most critical element in improving health outcomes. To achieve this outcome, we innovate and develop better practices to improve our care delivery model. To continue successfully down this path, we search for candidates who share our values and goals. The Family Medicine Physician will: provide outstanding, patient-centered primary and preventive care to patients of all ages work with a team of physicians, nurse practitioners, midwives, and other providers, along with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care exhibit a positive attitude during daily activities and foster a positive work environment for the entire team may function as a preceptor in teaching programs including medical residents, medical students, and advance practice nurse students, as well as other allied health professions. 40 hours per week: 36 Clinical and 4 Administrative Locations Available: Multiple Loan repayment opportunities through National Health Service Corps!
