International Health and Medical Services is looking for an experienced Paramedic to provide emergency medical care and responding to medical crises involving patients, ensuring timely and effective treatment in high-pressure situations. This role includes collaborating with the healthcare team to implement treatment plans, monitor progress, and provide crisis intervention as needed, fostering mental health and well-being. Key ResponsibilitiesProvides direct psychiatric care to patients via onsite and telepsychiatry services for scheduled appointments, emergency, or urgent care visit. Consults with the Psychiatrist and/or Clinical Director for complex psychiatric patients and as required by client policy. Provides care including admission, discharge, and daily rounds for patients that require higher levels of psychiatric care or monitoring. Performs comprehensive psychiatric evaluations and initiates treatment plans for patients as required by client policy, or as required for patient psychiatric complaint. Works within and performs functions as outlined in the client Psychiatric Advanced Practice Provider Collaborative Practice and Prescriptive Authority Agreement and the client formulary. Under the direction of the National Medical Director and in collaboration with the Office of the Chief Medical Officer (OCMO) of U.S. Customs and Border Protection (CBP), Regional Physician Supervisors deliver healthcare services to persons in custody in accordance with the polices and protocols listed in the Statement of Work (SOW) and Department of Homeland Security (DHS) directives for the delivery of healthcare services to persons in custody. Regional Physician Supervisors share the administrative functions that directly impact medical services and collaborate with the management team in overall planning and execution of program directives. Regional Physician Supervisors work to assure delivery of quality healthcare services in accordance with the SOW for the medical services contract within medical units at U.S. Customs and Border Protection sites including, Office of Field Operations (OFO) ports of entry, U.S. Border Patrol Border (USBP) Stations. Performs the clinical responsibilities as agreed upon by the National Program Director. Contracted medical staff will be working alongside uniformed Customs and Border Protection (CBP) Agents/Officers in secured federal facilities along the southwest border of the United States. These secured federal facilities are staffed and monitored by CBP personnel and/or contracted security staff 24 hours a day, 7 days a week to assure a safe working environment to include the co-located medical unit. Contracted medical staff work together with CBP personnel to provide health care services for persons of all ages, to include infants, children, adults, and pregnant females. MISSION CBPs Office of the Chief Medical Officer (OCMO) employs a trauma informed care model to provide frontline medical care for persons in CBP custody in accordance with Enhanced Medical Support Efforts CBP Directive (2210-004). This model provides a team of medical professionals comprised of physicians, advanced practice providers (e.g., physicians assistants, nurse practitioners) and support staff (e.g., Licensed Practical/Vocational Nurses, Paramedics, EMTs, Certified Medical/Nursing Assistants). These contracted medical teams provide basic assessment and medical treatment for persons in CBP custody to include emergency care as needed. Duties and ResponsibilitiesServes as the sector/region medical authority for the medical services contract and medical units at for the U.S. Customs and Border Protection sites including, Office of Field Operations (OFO) ports of entry, and/or U.S. Border Patrol Border (USBP) Stations. Provides medical direction and necessary guidance/oversight as required by state licensure for appropriate advanced practice provider supervision, oversight, consultation, coordination, and direction for all medical services, to include mental health and pediatric care in the absence of a behavioral health advisor or pediatric advisor. Clinical supervisory authority over sector physicians, mid-level providers, all clinical staff, all medical unit functions, laboratory, and pharmacy. Provides on-call coverage for assigned medical units/sector 24 hours a day/7 day a week, however, call coverage may rotate based upon staffing. Responsible for developing and coordinating all aspects of healthcare delivery at his/her assigned facilities. May provide direct care to patients in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges. Maintains non-involvement in the security aspect of patient care and custody. Participate in disease surveillance activities and public health and epidemiology studies/efforts, as needed. Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions for all ages. Serves as the laboratory director in sites where CLIA waived testing is performed, providing oversight of appropriate diagnostic tests and services, to include monitoring and meeting CLIA requirements. Acts as a reviewer for all outside referrals/consults to specialty services, to include hospital or emergency care that exceed services offered at the medical unit. Audits referrals and post-referral follow-up, as needed, as part of the medical quality management (MQM) program. Provides oversight for any patients requiring enhanced medical monitoring. May be expected to respond to and or coordinate response to medical emergencies and medical sentinel events. Seeks guidance from the Medical Director, Pediatric Advisor, Behavioral Health Advisor, or other leadership as appropriate. Serves on the national and local MQM committees and oversees root cause analyses and quality improvement projects/initiatives in collaboration with the national and local MQM committee. Performs collaborative mid-term review of performance with assigned Physicians and APPs as part of an annual performance evaluation process, addressing any clinical practice concerns if identified. Portions of the APP peer and chart review process, FPPE, and OPPE may be delegated, if a physician is assigned to the local facility. Provides emergency care to employees or visitors for life, death, loss of sight/limb situations until Emergency Medical Services Arrives. Serves as a professional advisor to physician colleagues from medical schools, graduate training programs in medicine and osteopathy, boards of licensure, and professional organizations relating to the medical profession and other health professions. This list is non-exhaustive, and the role holder may be required to undertake additional duties that are not specifically listed above. Qualifications
Basic Requirements/Certifications: Must possess a current, active, full, and unrestricted license or registration as a Physician from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. American Heart Association (AHA) Basic Life Support (BLS). Correctional/detention clinical experience preferred. Minimum of five years clinical practice with direct patient care preferred. Have knowledge of a wide range of complex patient care concepts, principles, and practices to perform assessments of considerable diversity to include mental health, medical, surgical, ambulatory, and emergency care. Flexibility and ability to adapt to sudden changes in schedules and work requirements. Assignments are usually of a long-term, recurring, or broadly defined nature. Highly effective interpersonal and organizational skills; strong writing skills to include document formulation, editing, and review. U.S. Public Health Service Officers must comply with and obey all federal rules, regulations, policies, guidance, and established procedures that includes professional competence, readiness, and suitability. All Employees shall have at least one year of general experience that demonstrates the following: The ability to greet and deal tactfully with the public. Capability of understanding and applying written and verbal orders, rules, and regulations. All personnel shall be literate and be able to interpret printed rules and regulations, detailed written orders, training instructions and materials, and must be able to compose reports. Good judgment, courage, alertness, an even temperament, and render satisfactory performance through knowledge of his/her position responsibilities. Ability to maintain poise and self-control during situations that involve mental stress, such as fires, explosions, civil disturbances, and building evacuations. Education Required: Doctor of Medicine or Doctor of Osteopathy from a school in the United States or Canada approved by a recognized accrediting body in the year of the applicant's graduation or Doctor of Medicine or equivalent degree from a foreign medical school that is certified by the Educational Commission for Foreign Medical Graduates (ECFMG). Evidence of board eligibility/certification is required in an appropriate primary care specialty (Internal medicine or family medicine) by an American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board. Physical Requirements: This position is considered hazardous duty. Required to walk unaided at a normal pace for up to 5 minutes and maintain balance. Required to jog/fast walk up to ¼ mile. Requires physical exertion such as lifting objects greater than 30 pounds. Required to perform CPR/emergency care standing or kneeling. Must have the ability to assist sick, injured, or aging patients or staff exiting the building (may require lifting, dragging, wheeling, or carrying someone who weighs significantly more than self). Must be able to see, hear and smell with aids if necessary. Must be able to lift, push, or carry 30 pounds. Must perform the duties of my job in a stressful and often austere environment without physical limitations. Sitting and/or standing for extended periods of time. Average manual dexterity for computer operation. Phone or computer use for extended periods of time. Other Special Qualifications: Minimum of 21 years of age. Must maintain current/physical residency in the continental U.S. Pass a medical examination conducted by a licensed physician within 30 days prior to initial assignment. Be a U.S. citizen or a person lawfully admitted into the U.S. for permanent residence. Have resided in the U.S. for the last five years (unless abroad on official U.S. government duty). Successfully engage in and complete a thorough Background Investigation. Poses or have ability to obtain required security clearances. Proficiency in Spanish is preferred.

Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
