Schedule: Full-Time, Sun-Wed 4:30pm-8am (blend of working and on-call hours)
Location: Most visits are in Stillwater/Woodbury, MN and Hudson, WI, but can include visits to all of Washington County/St. Croix County (St. Croix, Afton, Lakeland, Forrest Lake, Mahtomedi, MN) and parts of Pierce County in WI (Osceola, Baldwin, River Falls, WI).
Pay rate: Up to $74.20/hour Depending on Experience! (Separate stipend for on-call hours)
*This is a temporary assignment expected to start around July 13th and last until September 25th.

Responsibilities Include:
Triage phone calls as needed.Complete necessary steps for medication refills as needed.Durable Medical Equipment support and troubleshooting.Symptom management for hospice patients.Making visits for deaths and/or helping patients with issues that are not able to be managed over the phone.All other duties as assigned.
Required Skills/Qualifications:
An active RN license in the states of MN and WI are required.At least one year of previous experience working as an RN in a hospice, long-term care or palliative care setting. Must have reliable transportation for patient visits.Must have a reliable phone/workspace at home to provide phone triage assistance remotely.

What we ask of you:
Assess physical, psychological, social and environmental needs of the patient and family.Integrate principles of development and disease process including co-morbidities in creating the plan of care.Maintain necessary records and use them appropriately for the care of patients.Provide patient/family education including patients self care and care provided by family.Serve as a case manager for assigned patients and coordinate with all providers involved in patient care including RN, SW, home health aide and other rehab disciplines.Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.Provide skilled clinical care including evaluation and treatment of patients, and the development/implementation of care plans. Required Skills/Qualifications:
Must have a current license with the Minnesota Board of Physical Therapy and either have or be open to obtaining a WI license.Must have a Bachelors, Masters or Doctorate degree in Physical Therapy.At least one year of previous experience as a PT in home care settings.Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.Proficiency with communication and data processing equipment, clinical apparatus related to home care, and electronic medical record system.Must have BLS certification through American Heart Association (AHA) or American Red Cross (ARC).Must have reliable transportation for client visits.

What we ask of you:
Basic and complex audiological evaluation including pure tone air conduction, bone conduction and speech testing, skills with both children and adults.Clinical Impedance audiometry, special diagnostic testing for site of lesion, malingering and other diagnostic problems.Neonatal testing, ABR testing and ENG testing, evaluation in specialty area of Audiology such as APD and TRT.Evaluate patient's need for amplification with appropriate audiological testing and appraisal of patient's hearing requirements. Must possess Clinical Doctorate in Audiology from an accredited University program.Must possess/maintain current Minnesota Audiology license or possess current Minnesota Temporary Audiology license.At least one year of experience working as an Audiologist.Must possess and maintain licensure or registration which allows for dispensing of Hearing Aids Dealers within the state of Minnesota.Demonstrated verbal and written communication skills, including appropriate use of English grammar and spellingDemonstrated ability to function with multiple priorities and interruptions.Strong leadership and problem solving skills, ability to interact with patients and staff in a courteous manner.

Position Summary:
