    Hospice Social Worker -  Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. You will have independence and flexibility in the field to visit patients one on one in their home environment. 

    Hours:   Full-time, Days, Monday - Friday, Tentative On-Call rotation dependent upon staffing levels

    Pay and Benefits:

    Territory:  Our office is in Roseville, MN and the Social Worker will have a general territory within our service area where they will provide in-home hospice care to the patients.  The territory for this position would primarily be in the Southern section of the Twin Cities Metro area. 

    A few must-haves for this position:

    Graduate of a Master’s program in Social Work accredited by the Council on Social Work Education.Licensed to practice as a Social Worker in the state of Minnesota.  License is active and in good standing.Minimum of one (1) year's experience in health care.Hospice experience preferred.Able to travel within geographic area serviced by office. Valid drivers license, reliable transportation, and proof of auto insurance are required.

    Some functions of what you would do as a Medical Social Worker

    Conducts an initial psycho-social assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient’s/family’s needs and the Interdisciplinary Group (IDG) plan of care.Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.Identifies special needs related to religious or cultural diversity and how these impact the implementation of the plan of care.Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources, as well as provides follow-up and acts as a liaison when indicated. Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.Assists the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end-of-life choices.

    As a Hospice Chaplain, you will be providing spiritual services and comfort to the patients and their families either directly or indirectly through effective coordination with the patients clergy or other community clergy or spiritual counselors.

    Hours:  Full-time, Days

    Territory: Our office is based in Roseville, MN and our service area covers the 13 County Twin Cities Metro area. The territory for this position will be primarily in the South Metro.  We are looking for someone who is willing to travel throughout Interim's service area based on the client’s needs.

    Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, terminal individuals and their families or caregivers.Hospice experience preferred.

    Some functions of what you would do as a Chaplain:

    Assesses and identifies the patient/family/caregiver spiritual status and needs.Provides direct spiritual services to patient/family/caregiver according to their spiritual identity and/or religious/spiritual tradition or acts as liaison to spiritual caregivers resourced in the community, as requested by patient/family/caregiver.Assures that all documentation meets requirements for accuracy, timeliness that is reflective of services provided and compliance requirements, billing requirements, patient/family/caregiver needs, and confidentiality.Participates in the development of the interdisciplinary group (IDG) plan of care to meet identified spiritual needs with appropriate interventions and goals.Conducts spiritual and/or religious services for patient/family/caregiver as well as hospice and community staff as requested.
