We are seeking a Full-Time Home Health RN Clinical Manager. Below is a brief list of what we are seeking. Salary is commensurate with experience ($95K - $100K).
Five (5) years of Home Health experience (Medicare knowledge a must)At least three (3) to five (5) years in a RN clinical management roleWorking knowledge of SHPMust be clinically sound and able provide training to new clinicians in the community.Understands VBP and PDGM impact on home health servicesExtensive OASIS knowledgeAn understanding of Intake and Scheduling processesAble to manage supplies during episodic careStrong knowledge of wound care a plusWilling to assist with Scheduling, Clinical Issues, and Patient VisitsWilling and able work ON-CALL at least once per monthSystem Knowledge – Must be system savvy (able to toggle between multiple screens and systems simultaneously)

As our RN Hospital Liaison, you will serve as the clinical bridge between the hospital and home, ensuring patients experience a safe and successful transition of care.
Your responsibilities include:
Collaborate with physicians, discharge planners, case managers, and hospital staff to coordinate home health services.Identify patients who would benefit from skilled home health care.Assess patients' clinical and home care needs prior to discharge.Meet with patients and families at the bedside to educate them about Interim HealthCare's services and answer questions.Coordinate complex patient transitions to promote continuity of care and reduce avoidable readmissions.Review referrals, obtain essential clinical information, and communicate with our Intake and Clinical Management teams.Assist with insurance eligibility, authorizations, and physician orders when appropriate.Build strong relationships with hospital staff while serving as a trusted clinical resource.Participate in quality improvement initiatives and collaborate with interdisciplinary teams to improve patient outcomes.What You'll Bring
