Interim Healthcare of Chicagoland is seeking a RN Clinical Liaison for the Chicagoland areas of Oaklawn, Hyde Park, and Streeterville, IL
Salary range: $90,000 - $110,000 (based on full-time) part-time work also available
Must-Haves
• Home Health experience (RN) - knowledge of home health duties (not hospice) and Intake.
• Licensed RN in the State of Illinois
• Minimum of two (2) years of strong clinical experience
• Clinical Astuteness - ability to observe unusual symptoms, recognize hidden clinical patterns, and apply critical reasoning to make rapid, accurate diagnoses.
• Customer Relations - strong ability to establish and maintain positive and professional relationships with facility staff, including case managers, care planners, and patient/family.
• Reliable transportation
• Proficiency in EMR, Excel and other medical-related software
Your role
• You will work onsite collaboratively with the patient, patient’s family, the acute care hospital or sub-acute facility, physician group or health plan staff to facilitate a safe and effective transition to home care.
• Make weekly facility rounds with staff.
• Review files and assess for home health services fit.
• Make connections with physicians and clinicians for patient services.
• Assess the patient's and family caregiver's homecare needs and coordinates home health care services as required prior to the patient's transition home.
• Provide patient, family member, caregiver with introduction and education to Interim services and answer any questions. This will be provided bedside prior to patient’s release from facility.
• Obtain necessary medical information regarding the patient's health status, current medications and goals of home care intervention from designated inpatient or care manager staff.
• Communicate directly with our Intake team regarding referrals, resolve payor challenges to ensure timely SOC scheduling.
