Nurse Practitioner / Physician Assistant — Wound Care SpecialistInnovative Wound Solutions | 29 Palms & Coachella Valley, CA

Innovative Wound Solutions is a specialized medical group dedicated to delivering expert, patient-centered wound care to the post-acute population — a community that is often underserved and overlooked. We go where the patients are, providing advanced wound care services directly within skilled nursing facilities and in patients' homes across the 29 Palms and Coachella Valley region.

We are a team of clinicians who take their work seriously and support each other genuinely. If you want to specialize in a high-impact, clinically rewarding field and build real expertise over time, this is the place to do it.

Wound care is one of the most clinically engaging specialties in post-acute medicine. Every patient presents differently, every wound tells a story, and the progress you drive is visible and measurable. As an NP or PA with Innovative Wound Solutions, you will manage your own panel of wound care patients across assigned skilled nursing facilities and in private home settings, working with autonomy while staying connected to a collaborative, physician-supported team.

No two days look the same. You will move between facility and home environments, bringing specialized wound care directly to patients who would otherwise go without it. This role is ideal for a provider who wants to develop or deepen a specialty focus, work a structured schedule, and make a tangible difference in patients who genuinely need an advocate.

Wound Assessment and Treatment — Perform comprehensive wound assessments including tissue viability, wound dimensions, infection status, and associated symptoms across both facility and home settings, and implement evidence-based treatment plans tailored to each patient.

Advanced Procedures — Perform wound debridement, apply advanced dressings, and carry out other wound care procedures within your scope of practice in skilled nursing facilities and in patients' homes.

Patient and Family Education — Guide patients and families through wound care techniques, prevention strategies, and self-care practices that support healing and reduce recurrence, with particular attention to home care instruction for patients seen in their own environment.

Collaborative Care — Partner with physicians, nursing staff, physical therapists, home health agencies, and the broader interdisciplinary team to coordinate care and optimize patient outcomes across all care settings.

Documentation and Quality — Maintain accurate, timely clinical records for both facility and home visits, and actively participate in quality improvement efforts that strengthen the care we deliver.

    Qualifications

Education — Master's degree or higher in Nursing (NP) or completion of an accredited PA program. Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or PA-C required.

Licensure — Active, unrestricted California NP or PA license and current DEA registration. APRN certification where applicable.

Experience — Minimum of 6 months of clinical experience as an NP or PA. Prior acute care, hospital, post-acute, home health, or wound care experience is strongly preferred though candidates with strong clinical foundations are encouraged to apply.

Wound Care Certification — WOCN certification or wound care specialization is a plus, not a requirement. We support and encourage certification for the right candidate.

Clinical Acumen — Ability to assess and manage complex wounds across a diverse patient population with strong critical thinking and sound clinical judgment in both structured and independent care environments.

Communication — Clear, compassionate communicator with patients, families, facility staff, home health teams, and care team members across multiple settings.

Adaptability — Comfortable working across varied environments, from skilled nursing facilities to private residences, with the organizational skills to manage a mobile patient panel effectively.

Commitment to the Population — A professional dedication to providing dignified, high-quality care to patients in post-acute, long-term care, and home settings.

Take your practice in a direction that matters. Apply today.

Nurse Practitioner / Physician Assistant — Primary Care
Innovative Medical Group | Temecula, CA (Riverside County)

Innovative Medical Group is a fast-growing, physician-led medical group built around a simple mission: bringing exceptional, patient-centered primary care to those who need it most. We specialize in caring for the post-acute patient population across skilled nursing and long-term care facilities throughout Riverside County, and we take real pride in the relationships we build with our patients, families, and facility partners.

    The Opportunity

We're looking for a Nurse Practitioner or Physician Assistant who thrives in an autonomous, fast-paced clinical environment and genuinely enjoys the complexity of the post-acute patient. This is a SNFist-model role, meaning you'll carry your own panel of patients across one or more skilled nursing facilities, managing everything from admission assessments and chronic disease management to acute illness and care transitions.

If you're the kind of clinician who wants ownership over your practice, variety in your patient encounters, and a team that actually has your back, this role was built for you.

Patient Care and Rounding — Manage acute and chronic conditions, write orders, and guide individualized care plans for a post-acute patient panel across assigned SNF locations.
Admissions and Transitions — Lead SNF admissions and coordinate hospital-to-SNF care transitions in partnership with discharge planners.
Chronic Disease Management — Manage complex, multi-morbidity patients including CHF, COPD, diabetes, renal disease, and dementia with evidence-based care plans.
Collaborative Practice — Partner with physicians, nursing staff, social workers, therapists, and consulting specialists to deliver coordinated, whole-patient care.
Documentation and Compliance — Maintain timely, accurate clinical documentation and participate in quality improvement initiatives and facility care conferences.

Prior acute care, hospital, or post-acute/SNF experience strongly preferred.
Clinical Acumen — Comfortable managing medically complex, elderly patients with multiple comorbidities and strong clinical decision-making skills.
Communication — Confident and compassionate communicator with patients, families, facility staff, and interdisciplinary team members.
Autonomy — Self-directed and organized, with the ability to manage a patient panel independently while knowing when to escalate.
Commitment to Patient-Centered Care — Demonstrated dedication to delivering dignified, high-quality care to the geriatric and post-acute population.

Ready to make your practice mean something? Apply today.
