The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.
Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.Document care provided in accordance with established policies, guidelines, and applicable regulations.Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner.Collaborate with multidisciplinary teams in care planning for participants.Monitor appetite and look for changes in functional ability and skin condition.Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary.Report any variance, accident, or unusual occurrence to the appropriate personnel.Assist with loading and unloading of participants from vehicles.Assist the transportation department with participants’ take-home supplies.Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.Attend in-service and continuing education programs as scheduled and necessary.Other duties as assigned.REQUIRED

The Social Worker Assistant (SWA) supports the administrative activities of the Social Work staff in accordance with InnovAge policies and all applicable regulations. The SWA will work under the supervision and direction of the Social Services Supervisor. The tasks assigned to the SWA may not include activities required by regulation or contract requiring or describing a Social Worker and requiring a Masters of Social Work or other requirements such as care planning, case management, assessments, psycho-social services. The tasks assigned to the SWA may include but are not limited to non-skilled, routine administrative activities necessary to ensure that participant needs are being addressed such as confirming the delivery of goods and services or start date of external services and supports ordered by the IDT as part of the Care Plan; assisting participants with Medicaid recertification; researching/identifying community-based agencies to address social determinants of health or other participant needs; other tasks as directed by the Social Services Supervisor.

Additionally, the Social Work Assistant may provide hands on assistance to participants applying for housing, homelessness prevention, food security or other similar programs identified by participants’ InnovAge Social Worker as a needed intervention. Administrative support for end-of-life coordination may be assigned as directed by the Social Work Supervisor. Assists with housing encounters documentation in EPIC
Community Resource Research: The SWA may be assigned to research, identify and catalogue community or faith-based organizations; city, county or state programs; other services to address the needs of individual participants or participant populations within the Center. This would include services provided, contact person, hours of operation, and contact information necessary for the assigned Social Worker to make referrals and/or for participants to self-refer.
Outreach: The SWA may be assigned tasks related to locating and validating participant location, telephone number, address, emergency contacts and other demographic information necessary for ongoing engagement with the participant. Additionally, at the direction of the Social Work Supervisor, the SWA may be responsible for updating key demographic information in InnovAge systems as defined by policy or regulatory requirements.

The Dietitian completes participant nutritional assessments and reassessments, screens and assesses participant’s nutritional status and uses pertinent data to plan and implement appropriate nutrition interventions, obtains participant food preferences and makes dietary modifications as needed, in addition to providing nutrition education to participants, caregivers and InnovAge PACE staff. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties.
Participant Care– 60%
• Develops and implements nutritional Plan of Care (POC) for participants and reassesses every six (6) months for each participant, or as necessary if there are changes in participant status.
• Assesses participants’ access to food and makes referrals to Inter-Disciplinary Team (IDT) for; meal preparation evaluation, Meals on Wheels, sack lunches and grocery shopping needs.
• Determine participants’ need for and orders liquid nutritional supplements accordingly.
• Designs and implements the nutritional components of participant care plans including active participation and development of therapeutic dining programs.
• Provides instruction to participants and caregivers regarding dietary regimens and nutritional requirements. Home visits may be necessary.
• Develops and implements the nutrition care plans for high risk participants including medical therapy recommendations.
• Communicates with In Home Service providers regarding participants’ stock of groceries, intake of food and fluids at home or as needed.
• Monitors food intake, therapeutic diets and special dietary modifications and communicates information and concerns with the IDT.
• Calculates data obtained from food intake records and calorie counts, and makes recommendations for changes to appropriate staff.
• Documents nutrition notes in the participant’s medical record.
• Communicates diet changes and nutrition concerns with dietary aides, interdisciplinary team, participants and/or caregivers.
• Provides basic nutrition and sanitation education for participants, families, primary caregivers and/or InnovAge PACE staff as needed.
• Assists with meal service on an as needed basis.
• Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.

Administration – 40%
• Conducts sanitation and safety checks for kitchen and kitchen equipment as needed.
• Monitors adequacy of lunch and snack menus for the ADHC and notes/makes changes as necessary.
• Updates the sack lunch list, supplement sheet, diet lists and assisted dining room guide as needed.
• Attends Care Planning conferences, Family and wound care meetings and reports on nutritional status of participants.
• Assists in the training of new Dietary Aides and Lead Dietary Aides.
• Participates in work area development, in-service training and department meetings.
• Works with the State Food Program as guided by state rules and regulations
• Participates in the development and implementation of InnovAge PACE Quality Assessment and Performance Improvement Plan for nutrition services.
REQUIRED
• Bachelor’s degree in food science, nutrition, and/or dietetics from an accredited college/university is required.
• Current medical terminology and the ability to complete written assessments and document participant care.
• A minimum of one (1) year experience working with the frail or elderly is required.

Responsibilities
The Claims Processor II is responsible for ensuring the accurate and timely processing of all UB, HCFA and Dental claims submitted by external providers for participant care per company and CMS guidelines. This position monitors and processes claim audits to maximize accuracy and minimize expense, using various software and customized applications. The position interacts with all external providers, vendors, and external agencies on issues related to claims submission, process and payment.
Train providers and address provider appeals per CMS and NCCI guidelines adjusting claims as appropriate.Monitor and clear pended claims as necessary via research and system updates/corrections.Downgrades DRG claims and reprocesses per the direction of InnovAge’s external audit vendor.Processes provider refunds, creating, coordinating and reconciling activity with AP.
