Role Overview:
The RN will provide specialized care to post-cardiothoracic surgery patients in a step-down setting. Responsibilities include comprehensive assessments, medication administration, monitoring and interpreting telemetry, and collaborating with the interdisciplinary team to ensure safe patient transitions. Flexibility to rotate to general floor care is required.

Regional Nurse (RN)

About the Role:
You'll conduct home visits to deliver services like wound care, medication administration, health assessments, and patient education. You may also work in a Diagnostic & Treatment Center or medical practice as needed. Collaboration, clinical judgment, and flexibility are essential in this role.
Key Responsibilities:
Perform home-based clinical services including wound care, medication administration, and comprehensive assessments
Follow individualized care plans and physician orders with clinical accuracy
Communicate patient status and changes promptly to the care team
Educate patients and caregivers on disease management, medication, wound care, and safety
Maintain accurate, timely documentation in EHR systems
Participate in interdisciplinary meetings and adhere to infection-control and regulatory protocols
Travel within designated service area for patient visits and coverage
Provide occasional coverage in clinical settings as assigned by license scope
Function as part of and contribute to the interdisciplinary team (IDT)
Qualifications:
Graduate of an NLN‑accredited nursing program (RN/BSN) or 5+ years of healthcare experience
3+ years of clinical nursing experience; 1–2 years in home care or similar preferred
1+ year working with frail/elderly populations (or training upon hire)
Active, unrestricted RN license in New York State
Reliable travel availability and flexibility for weekends, nights, and on-call needs
Proficiency with computers and EHR systems
Valid vehicle and NYS driver’s license preferred
Physical Requirements:
Able to manage up to 6 hours of standing, 6–8 hours of sedentary work, lift up to 50 lbs, perform bending/squatting, navigate stairs or varied terrain, complete fine motor tasks, visually assess and document care accurately, process auditory information clearly, and exercise sound cognitive decision-making.
