This role supports patients, providers, and interdisciplinary teams by promoting accuracy, collaboration, and continuity of care from admission through discharge. What you'll Do * Electronic Medical Record Management: Process and manage all Electronic Medical Record (EMR) workflows from admission through discharge, ensuring accurate, timely, and complete documentation that supports continuity and quality of care. * Clinical Leadership: Serve as a professional clinical leader in assessing, planning, guiding, and evaluating patient care, applying sound nursing judgment and evidence-based practice to support positive patient outcomes. * Interdisciplinary Collaboration: Interact effectively with management, physicians, and field staff using a teamwork-focused approach to coordinate care, resolve issues, and ensure alignment with organizational goals and patient care standards. What You'll Need * Registered Nurse (RN) degree from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) and/or the Commission on Collegiate Nursing Education (CCNE) preferred. * Currently licensed and registered as a Registered Nurse in the State of Florida. * Bachelor of Science in Nursing (BSN) preferred. * Current Basic Life Support (BLS) Certification required. * Hospice Certification preferred. * One year of recent experience in hospice, oncology, medical/surgical, home health, or public health nursing preferred. * Demonstrated expertise in support skills, palliative care principles, and patient/family education techniques. * Flexible, creative, assertive, articulate, nurturing, and self-nurturing professional demeanor. * Experience with Homecare Homebase software a plus. * Strong computer skills required, including proficiency with clinical documentation systems. * Ability to work effectively under stress and meet deadlines in a fast-paced environment.
