    Purpose: In the delivery of treatment, rehabilitation and support services, the Mental Health Professional provides clinical supervision in assessment and treatment planning, and provides support and back-up to the Team Leader. Provide service coordination (case management for an assigned group of participants including coordinating and monitoring the activities of the individual treatment team. Provide individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social skill development, and assertiveness training to increase participants’ social and interpersonal activities in community settings.

    Supervision Received: Receives direct supervision from the ACT Director.

    Supervision Exercised: N/A

    Qualifications: M.A. degree in Human Services/Social Sciences, Healthcare or related field and Post-secondary school training is required, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illness. Experience with individuals who have severe and persistent mental illness, providing guidance so they can live in community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services. Ability to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential. Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large. Must be available for local travel; valid driver’s license required.

    Scope: Frequent talking and listening. Requires the ability to lift, carry, hold, kneel, reach, stack, stoop and bend, which may include direct intervention with residents. Must be available or scheduled emergency on-call service and will be required to carry a mobile phone during regularly scheduled working hours and scheduled on-call service.     We are seeking a qualified Licensed Practical Nurse (LPN) candidate interested in joining our Residential Group Home Services team at our location Wilmington, Delaware 19803.

    Here is some information about the opportunity we're offering, and you may be searching for: 40 hours workweek, Monday-Friday, 3P-11P, residential group home setting, working independently and in a team environment.

    Hire requirements include new hire orientation only first four (4) workweek dayshift Mon-Thu schedule at our Philadelphia location and mileage reimbursement

    The Licensed Practical Nurse (LPN) as part of the RGH team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician’s orders. Some of the responsibilities include the following.

    Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.Provides ongoing assessment of participant’s mental health symptoms and responds to recovery plans.Educates and supervises participants in self-administration of medication.Ensures annual review of the participants’ physical condition and assist access to and encourage compliance with treatment for identified physical problems.

    Qualifications:

    Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact.Knowledge of mental health systems and supports.Knowledge of multiple support systems for substance abusers.Good communication skills, both oral and written.Ability to work independently and as part of a focused team.Possession of a valid driver's license with an acceptable driving record.Computer literacy, very strong organizational and analytical skills.

    Currently, we are seeking a On-Call Outreach Specialist to work in our Homeless Services Division. This position has responsibilities of providing outreach services to homeless individuals encountered on the street during hours mandated by the City of Philadelphia’s Coordinated Evening Outreach Initiative. They also may Engage homeless persons into accepting appropriate services, which may include the facilitation of voluntary/involuntary hospitalizations and/or referrals to shelter or other supports.

    Some other responsibilities include the following:

    Providing outreach and engagement services during overnight shifts. Engage homeless individuals through outreach activities by utilizing local resource networks.Coordinating referrals with other agencies/facilities to meet the assessed needs of the clients.Working cooperatively with local service providers and funding sources.Participating in the resolution of crisis situations. Ensure safety by facilitating voluntary and involuntary (when necessary) hospitalizations through the petitioning of 302 procedures. Responsible for completing all documents required to complete petitions.Representing the agency and advocate for client when subpoenaed for 302 hearings.Maintaining a close liaison with the local community and area service providers/agencies to facilitate program cooperation and provide for constructive input and/or alternative resources.Monitoring the trends within the homeless community and share knowledge with the Program Manager.Serving as an advocate and encourage clients to assume a proactive role in their rehabilitation and recovery process.

    Requirements:

    High School diploma or GED and a minimum of (3) years of relevant experience in substance abuse counseling; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.Certified Addiction Counselor (CAC), preferred or on-going coursework in Drug and Alcohol Counseling to qualify for certification.Strong interpersonal skills.Ability to work independently and as part of a focused team.Mental Health knowledge desired

    Currently, we are seeking a Full-Time Case Manager to work in our Homeless Services Division. The position provides community-based supports to individuals in compliance with the Agency service contract and policies and procedures. Approximately 75% of work is done in the community. The selected candidate does home based visitations and interacts with members of the multidisciplinary team to ensure that participants receive individualized services designed to meet their needs and to help them successfully maintain housing and accomplish their recovery goals.

    Providing engagement and case management support including home-based visitations and other services within the Philadelphia community.     Currently, we are seeking a full time Psychiatric Rehabilitation Worker to work in our Behavioral Health Services Division. This position has the responsibilities of providing Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned to enhance an individual’s knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing. They may also help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice and monitor and track progress of individual’s in assigned caseload; participate in the development of individualized rehabilitation plans. Some other responsibilities include the following:

    Provides and facilitates individual’s skill development and support which includes daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning including community integration and community connections.Maintain and monitor an assigned caseload of individual’s.Participate in the analysis and identification of service delivery gaps, changes and problems under the guidance of the supervisor.Consistently meet productivity expectations as set by administration, department and funding sources.Participate in the resolution of crisis situations; assist with identifying individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.Complete and assist with required documentation. Develop, review, revise forms, procedures and standards of care according to accepted professional practice.Establish and maintain liaisons with other service providers to promote program coordination and enhance individuals services.Participate in relevant training to enhance professional growth, and development with an emphasis on psychiatric rehabilitation services and evidence based practices.Serve as advocate and encourage individual’s to assume a proactive role in their rehabilitative process, including mastery of skills.

    Requirements:

    Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years work experience in Human Services, which must include 1 year of mental health direct service.Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.Strong computer skills are required, as well as Internet research abilities.Ability to work independently and as part of a team.Knowledge of mental health system and supports.Must possess a valid driver’s license with an acceptable driving record.

    Scope:

    Frequent talking and listening.Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.Must be available for local travel.May require crisis intervention for emergency situations.Participation in on call coverage as applicable.May be required to report to work during emergencies including inclement weather.
