    Responsibilities:

    Provide patient care according to client’s care plan and needs
    Perform wound care treatments and assist clients with medication management
    Teach the client and/or family members about care and support skills
    Communicate regularly with RN Case Managers, Supervisors, and the Care Team

    Qualifications:

    Licensed as a Practical Nurse / LPN in the state of Minnesota
    Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
    Demonstrated ability to work autonomously
    Prior Nursing experience in a TCU, Hospice, or Home Health setting
    Experience with Electronic Medical Records (EMR) software is preferred

    Military Occupational Classification (MOC) Codes: If you're a military member transitioning to civilian life, we value your service and skills. Applicable MOS codes for this role include:

    Certified Nursing Assistant / CNA Qualifications:

    18 years of age or older
    At least 1 year of caregiving experience in Assisted Living or Home Care (CNA certification is a plus!)
    Ability to pass a multiple-choice Nursing Assistant knowledge quiz
    Reliable vehicle and ability to travel up to 35 miles from home
