    Our company is an established home healthcare company that has been servicing patients in Illinois  seeking a high-energy, self-initiating, and caring RN who can work as a field nurse in Chicago North Side 60614 and the following neighbourhood: Humboldt, Jefferson Park, Logan Square, Lincoln Park, Portage Park. Albany, Belmont, Austin,   Your organizational skills and strong understanding of home health, geriatric care, and previous experience in home health will be highly valued and very well compensated.

    Responsibilities and Qualifications:

    As a Registered Nurse RN you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

    Additional responsibilities of our Registered Nurse include:

    Provide admission, case management, and follow-up skilled nursing visits for home health patients.Confer with the physician and patient in developing a plan of care based on patient's medical condition and assessment findings at the time of the Start of Care and with every re-certificationObserve and assess patient's overall health and community status; train/educate patient /family on disease condition and treatment plan; render hands on care as needed and do management and evaluation when appropriate.Coordinate with other disciplines involved in case to ensure that quality services are provided and there is continuity of care among providersUpdate physician on patient status routinely and as needed when patient has a significant change in conditionWork with families and get them involved in the plan of care in order to achieve maximum resolution to patient needs/problems.Create plan of care for home health aides and supervise them routinely to ensure that they are following the plan of care.Perform discharge planning and provide patient with the necessary information they require in order for them to be able to continue to manage on their own after home health services are discontinued.Get involved with QI projects and offer input to help improve delivery of service • Adhere to the Nursing Standards of Practice.

    This is a full time position but Part time or PRN are welcome to apply too. 

    QUALIFICATIONS: 
    1.    Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing. 
    2.    Minimum of one (1) year experience in nursing, preferred. 
    3.    Acceptance of philosophy and goals of this Agency. 
    4.    Ability to exercise initiative and independent judgment. 
    RESPONSIBILITIES: 
    1.    Understands and adheres to established policies and procedures. 
    2.    Implements the nursing care plan for each patient. 
    3.    Provides nursing services, treatments and diagnostic and preventive procedures as assigned. 
    4.    Initiates preventive and rehabilitative nursing procedures as appropriate. 
    5.    Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition. 
    6.    Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. 
    7.    Evaluates with registered nurse the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse. 
    8.    Maintains accurate and complete records of observations, treatments and care of patient. 
    9.    Participates in medical record audit as assigned. 
    10.    Attends staff meetings, patient care conferences and in-services as scheduled.
