Job Summary
As a Home Health Nurse, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
Following a physician-established plan of treatment and working under the supervision of the Director of Nursing, our Home Health Visit Nurses instruct patients and their families in proper treatment and help individuals and groups take steps to improve or maintain health.With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.Our nurses function as a Caregiver, Teacher, and Advocate; working in partnership with the family, our nursing staff brings a commitment to support and assist with the family in their caregiver role. As a caregiver, the nurse will provide ongoing assessment of our patient's condition response to treatments and developmental progress. As a teacher, the nurse will instruct the family on procedures, medications, safe use of equipment, and compliance with the treatments.

Essential Functions/Areas of Accountability:
Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.Recognizes and reports changes in client condition to the Case Manager and physician, as directed.Communicates information on a timely basis, consistent with agency policy and urgency of the situation.
