    Position Overview
    Provide telehealth therapy and support services for adults experiencing mental health and/or substance use challenges. You'll be part of a collaborative, multidisciplinary team while delivering services remotely via secure platforms.

    POSITION OVERVIEW

    This position provides leadership and managerial oversight for an Assertive Community Treatment (ACT) team. The ACT team is a multidisciplinary team that provides a wide array of services to individuals with severe and persistent mental illness and dual disorders. ACT services are provided in the community and include, but are not limited to: mental health counseling, substance use counseling, skill building, medication management, budgeting, healthcare practices, and accessing community resources.

    CORE RESPONSIBILITIES

    - Provide supervision and oversight to a multidisciplinary team, including:

    - Community Support Specialist

    - Nurses (RNs and LPNs)

    - Substance Use Specialist

    - Employment Specialist

    - Lead daily organizational staff meetings to review each client's status and ensure coordinated, shared-caseload service delivery across the team, consistent with ACT fidelity standards

    - Monitor and report fidelity measures (e.g., DACTS/TMACT) to ensure ongoing compliance with state and national ACT model standards

    - Maintain appropriate staff-to-client ratios and ensure the team delivers the full range of services directly, rather than through outside referral

    - Ensure the majority of client contacts take place in community settings rather than the office, in accordance with ACT fidelity guidelines

    ABOUT THE ROLE

    The Assessment Clinician, Substance Use plays a critical role in evaluating and diagnosing individuals to inform treatment planning and support services. This position provides substance use and mental health treatment services including orientation, screening, assessment and diagnosis, treatment planning, individual counseling, and group counseling.

    This ole involves conducting comprehensive clinical assessments using standardized tools and clinical interviews to gather relevant psychological, behavioral, and social information, and collaborating with multidisciplinary teams to interpret assessment data and develop individualized care plans that address the unique needs of each client. The Assessment Clinician is responsible for ensuring that all individuals receiving substance use treatment receive ongoing assessment and treatment planning in accordance with guidelines set forth by IDHS DBHR and other applicable rules.

    Additionally, the role requires maintaining accurate documentation and ensuring compliance with ethical and regulatory standards. Ultimately, the Assessment Clinician contributes to improving client outcomes by providing thorough and insightful evaluations that guide effective interventions.

    RESPONSIBILITIES

    - Provide substance use and mental health treatment services, including orientation, screening, assessment and diagnosis, treatment planning, individual counseling, and group counseling.

    - Conduct detailed clinical assessments and diagnostic evaluations using evidence-based tools and techniques.

    - Gather and analyze client history, behavioral observations, and collateral information to inform assessment outcomes.

    SKILLS

Proficiency in documentation ensures that all findings are recorded clearly and comply with regulatory standards.     MINIMUM QUALIFICATIONS

    - Bachelor's degree in human services, social work, psychology, or a related human service field.

    - Certification or licensure (CADC, LSW, or LPC) required within 24 months of hire.

    - Demonstrated experience conducting clinical assessments and diagnostic evaluations.

    - Strong knowledge of DSM-5 criteria, ASAM criteria, and standardized assessment instruments.

    - Excellent communication and documentation skills.

    - A valid driver's license, reliable means of transportation, and proof of automobile insurance.

    PREFERRED QUALIFICATIONS

    - Graduate degree in counseling, clinical psychology, social work, or other human service field.

    About the Role

    Heritage Behavioral Health Center is seeking a Team Lead, LPHA Supervisor to provide clinical leadership and oversight to our team of reviewing Licensed Practitioners of the Healing Arts (LPHAs). This role ensures that diagnostic assessments, treatment plans, and related clinical documentation meet Illinois Rule 132/140, CCBHC, and medical necessity standards across Heritage’s continuum of care. The Team Lead, LPHA Supervisor supports practice-level decision making through case consultation, partners with leadership across clinical programs to maintain continuity of care for high-needs clients, and collaborates closely with Heritage’s Safety Team to address risk and ensure client and staff safety.

    Core Responsibilities

    Provide direct supervision, clinical leadership, and day-to-day oversight to the team of reviewing LPHAs, including workload assignment, quality monitoring, and timeliness of assessment reviews.Oversee the LPHA review of diagnostic assessments, treatment plans, and related clinical documentation to confirm diagnostic accuracy, medical necessity, and compliance with Illinois Rule 132/140 and CCBHC documentation standards.Facilitate and participate in case consultations with clinical staff across programs, offering guidance on complex presentations, differential diagnosis, level-of-care determinations, and treatment planning for high-acuity clients.Partner with directors and program managers across clinical service lines, including crisis services, residential recovery, outpatient, substance use, and youth and family programs, to coordinate continuity of care for high-needs clients moving between levels of care.Collaborate with Heritage’s Safety Team on the identification, review, and follow-up of client and staff safety concerns, including risk mitigation planning and post-incident clinical review.Track and report on LPHA review quality, accuracy, and turnaround time, providing coaching, feedback, and corrective guidance to reviewing LPHAs as needed.Participate in CQI activities related to assessment quality, diagnostic congruence, and clinical risk, in coordination with clinical leadership.Stay current on changes to Illinois Rule 132/140, CCBHC requirements, and DSM-5-TR diagnostic standards, and ensure the reviewing LPHA team is trained accordingly.Maintain own caseload of assessment reviews and case consultations as needed to support team coverage and timeliness.


    Qualifications

    Must meet the definition of a Licensed Practitioner of the Healing Arts (LPHA) under Illinois Rule 132, Licensed Clinical Social Worker (LCSW).Active, unrestricted Illinois licensure required.Minimum of three years of post-licensure clinical experience in behavioral health required, including experience completing diagnostic assessments and treatment plan reviews.Prior supervisory, lead, or team coordination experience strongly preferred.Demonstrated knowledge of DSM-5-TR diagnostic criteria, medical necessity standards, and Illinois Rule 132/140 documentation requirements.Strong skills in clinical case consultation, risk assessment, and crisis-related decision making.Ability to work collaboratively across multiple clinical programs and with the Safety Team on shared client care and risk concerns.A valid driver’s license, reliable transportation, and proof of current automobile insurance are required.
