Position Overview: We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide patient-centered, trauma-informed primary care as part of a multidisciplinary team. This role focuses on addressing acute and chronic health conditions, promoting health equity, and delivering holistic care to individuals facing complex social and health challenges.
The Advanced Practice Provider (APP) provides quality primary healthcare services to special populations, emphasizing the promotion of health and the prevention of disease. This role addresses society's demand for accessible and affordable healthcare. The APP works as an integral member of a multidisciplinary team, managing acute and chronic health issues while collaborating with physicians, nurses, and other healthcare professionals. Individual accountabilities and work volumes will be established annually.
Essential Duties and Responsibilities:
Facilitate participant engagement and entry into the healthcare system by assessing primary healthcare needs, performing physical examinations, and ordering/performing pertinent diagnostic tests.Provide coordinated and continuous primary healthcare to clients across the care continuum through goal setting and care planning.Prescribe treatment modalities ( based on licensure) to meet client health and illness needs.Educate and counsel clients to foster therapeutic and caring relationships.Manage and evaluate care plans, modifying them as needed to meet individual client needs.Collaborate with multidisciplinary teams to provide high-quality care and community services.Coordinate with team members to ensure continuity of care.Participate in quality assurance activities, including protocol development, peer review, and evaluating standards of care.Maintain clinical competence through ongoing education and collaboration with colleagues.Document comprehensive and accurate data in client records and reports.Adhere to ethical and legal practice guidelines set by regulatory and governing bodies.Participate in professional development and mentorship activities.Qualifications:

Active Illinois PA licensure required.Graduation from an ARC-PA-accredited program.Preferred family practice provider. Successful completion of the PANCE exam.• Nurse Practitioners (NPs):

Summary: The Participant Engagement Specialist will provide advanced customer service and engagement at reception and registration, enhancing the participant experience and access to health care and other services within Heartland Alliance Health while promoting excellence. The individual in this position will handle complex support services including, but not limited to, scheduling appointments, telephone operations, cash collection, auditing and reporting. Essential Duties and Responsibilities:
Greets, welcomes and engages participants into services at assigned locations, including health centers and outreach locations.Registers and assesses participants for program eligibility and screens participants for other social service needs or insurance benefits and entitlements services.Explains service offerings, answers questions, and makes appointments for all services, including but not limited to, medical, dental, and behavioral health care. Answers incoming and makes outgoing phone calls to schedule and remind participants of appointments while ensuring other calls are routed to appropriate person/program in a timely manner. Creates telephone note in electronic health record to alert Providers and/or Nurses of Participant requests for call-backs if not immediately available.Assists in telephone and other initiatives to increase participant engagement, improve participant experience and service delivery. Educates participants regarding patient responsibilities, services, sliding fee scale, patient rights, grievance policy, confidentiality and release of information. Generates and updates the Program Participation Form and updates participant information as assigned. Maintains a thorough understanding of medical coverage, managed care organizations, and third-party liabilities, including Medicaid and Medicare. Verifies and updates third-party insurance. Maintains a strong familiarity with CPT codes and determines the amount needed at co-pay based on service rendered. Accurately assesses, explains, and collects co-pays and issues receipts to participants. Generates daily and/or weekly reports of collection data. Maintains an accurate daily log of co-pay activity. Coordinates patient registration activities with Front Desk, Case Management, and Billing Department. The Community Health Nurse provides quality primary health care service to special populations. The individual is a registered professional nurse prepared at the baccalaureate level as a community health nurse generalist, who synthesizes nursing and public health practice to promote and preserve the health of individuals, families and groups to improve the health of the community. Within the nursing process framework, the Community health Nurse performs comprehensive assessment and validation of the reactions and responses of individuals, families or groups to actual and potential problems and provides intervention, evaluation and/or referral to meet identified client needs. Summary: The Participant Engagement Specialist will provide advanced customer service and engagement at reception and registration, enhancing the participant experience and access to health care and other services within Heartland Alliance Health while promoting excellence. The Manager of Grants Development is responsible for contributing to all aspects of the organization's resource development strategy, including identifying, cultivating, soliciting, and stewarding funding from federal, state, and local government sources, private foundations, corporate partners, and individual donors. This position plays a critical role in ensuring the financial sustainability and growth of community health programs.
Essential Duties and Responsibilities:
Grant Strategy and Management
Identify and research funding opportunities from government agencies (HRSA, SAMHSA, CDC, state and local sources), private foundations, corporations, and other philanthropic entitiesManage a portfolio of grants throughout the grant lifecycle, including preparing applications, proposals, and reports across multiple funding streamsCreate and maintain a grants calendar tracking deadlines, reporting requirements, and renewal datesEnsure compliance with all grant requirements, regulations, and reporting standardsMonitor grant performance metrics and outcomes to demonstrate impact to fundersGrant Writing and Proposal Development

The Dental Hygienist, under differing levels of supervision, will provide dental health care services that include cleaning, polishing, and providing preventative instructions and treatments. The Dental Hygienist may also provide chairside assistance as needed. Individual accountabilities and work volume will be established through the development of annual Success Objectives and annual Performance Evaluations, within the framework outlined below.
Clinical Duties
Perform duties under “general supervision” which requires a previous exam within 12 months by a treating dentist but does not require the dentist to be present within the facility or location of care including, but not limited to: Oral Prophylaxis (dental cleaning)Application of FluoridePlacement of SealantsPlacement of SDF Exposure and processing of x-rays to teeth and surrounding structuresAny Procedures performed by a Dental Assistant

Acts as a preceptor and provides onboarding, orientation, and training to new dental personnel, dental assistants, interns, and volunteers on dental hygiene procedures, protocols, infection control, and clinic operations.Offers mentorship, coaching, and ongoing guidance to support the professional development and skill growth of team members and trainees.Provides direction and oversight to dental assistants during chairside assistance and routine procedures to ensure proper workflow efficiency.Serves as a consultant and resource for specialized patient populations, sharing expertise with other dental care providers and community partners.Contributes to team performance by identifying training needs, providing feedback, and supporting quality improvement initiatives. Ability to apply concepts of basic algebra and geometry. Reasoning Skills: High reasoning ability; ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Language Skills: Ability to read and interpret documents such as safety rules, operation and maintenance instructions, and procedure manuals. Ability to speak effectively before groups of customers or employees of organization.Computer Skills: Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance Health.Other Skills and Abilities: A minimum of one year hygiene experience. Previous experience in handling specialty referrals, maintaining dental records, charting and documentations is preferred. Team players with the ability to multitask and have strong communication, computer, and customer service skills are desired.
