    About the Role

    We're looking for a Logistics Specialist I to join the Home Delivery team. In this role, you'll help ensure patient deliveries run smoothly by resolving delivery issues, managing daily logistics, and supporting customer-facing processes.

    You'll work closely with a team of six and be cross-trained across several areas of the business, including:

    Daily case managementSWAP ordersVacation ordersAcute & Critical Care supportSkilled Nursing Facility (SNF) operationsHigh Touch reportingPTO and team coverage

    We're seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team. In this role, you'll help patients regain mobility, improve functional independence, and enhance their quality of life through personalized therapy plans delivered in the comfort of their homes.

    Schedule

    Required Qualifications

    Responsibilities

    Perform comprehensive physical therapy evaluations based on physician referrals.Develop individualized treatment plans with measurable goals to maximize patient function and independence.Provide skilled physical therapy interventions in the home setting.Reassess patient progress and modify treatment plans as needed.Complete accurate and timely OASIS assessments and documentation in HomeCare HomeBase (HCHB).Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care.Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.Recommend and train patients on adaptive equipment to improve function and independence.Maintain accurate clinical, treatment, and billing documentation in compliance with agency, state, and federal guidelines.Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with Alabama practice standards.Participate in interdisciplinary team meetings, staff development, and continuing education opportunities.

    Responsibilities

    Conduct comprehensive patient assessments and develop individualized care plans.Deliver skilled nursing services in the home setting while promoting health maintenance and disease prevention.Complete accurate OASIS assessments and maintain timely documentation in HomeCare HomeBase (HCHB).Coordinate patient care with physicians, therapists, case managers, and interdisciplinary healthcare teams.Monitor patient conditions and communicate changes to the physician to ensure appropriate interventions.Administer and monitor medications, reconcile physician orders, and ensure compliance with medication management policies.Educate patients and caregivers on medications, disease management, treatment plans, and home safety.Supervise and support Licensed Practical Nurses (LPNs) and other clinical staff as appropriate.Coordinate hospital admissions, discharges, and follow-up care to ensure smooth transitions.Maintain compliance with agency policies, infection prevention standards, and regulatory requirements.Participate in the rotating on-call schedule to provide after-hours patient support.
