    Overview

    HealthDrive is looking for a full-time Primary Care Nurse Practitioner to manage the medical needs of skilled nursing residents in both the sub-acute rehab and long-term care units (in the Orange/Newark areas). The Nurse Practitioner will diagnose and treat health conditions, promote good health practices, and work collaboratively as a member of the medical staff. The APRN will also establish and build strong working relationships with administrators and staff throughout their designated building and will act as the primary point of contact for emergencies and routine medical visits. The Nurse Practitioner will be responsible for conducting physical examinations, interpreting lab tests, and overseeing patient health.

    Responsibilities

    Clinical Care

    Conduct comprehensive palliative care assessments for residents with serious, chronic, or life-limiting illnesses.Evaluate and manage symptoms including pain, dyspnea, anxiety, depression, nausea, fatigue, insomnia, and other distressing conditions.Develop individualized care plans in collaboration with residents, families, and facility staff.Provide ongoing follow-up visits and monitor disease progression and treatment effectiveness.Order, interpret, and review diagnostic tests, laboratory results, and other clinical data as appropriate.Coordinate care with primary care providers, specialists, hospice agencies, and facility teams.

    Goals of Care & Advance Care Planning

    Lead goals-of-care conversations with residents and families to ensure care aligns with patient preferences and values.Facilitate discussions regarding prognosis, treatment options, quality of life, and end-of-life care planning.Complete and review advance directives, healthcare proxies, MOLST/POLST forms, and code status documentation.Provide education and emotional support to residents and families facing serious illness.

    Medication Management

    Review and optimize medication regimens with an emphasis on symptom management and deprescribing when appropriate.Prescribe medications and treatments in accordance with state regulations and organizational policies.Monitor for medication effectiveness, adverse effects, and potential drug interactions.Collaborate with pharmacists and facility clinicians to improve medication outcomes.

    Interdisciplinary Collaboration

    Participate in interdisciplinary care conferences and family meetings.Educate facility staff on palliative care principles, symptom management, and communication strategies.Serve as a clinical resource for nursing staff regarding complex palliative care cases.Collaborate closely with social workers, therapists, chaplains, physicians, and hospice partners.

    Documentation & Compliance

    Maintain timely, accurate, and complete clinical documentation within the electronic medical record.Ensure compliance with federal, state, and facility regulations.Document goals-of-care discussions, treatment recommendations, and care plan updates.Support quality improvement initiatives related to palliative care outcomes.

    Overview

    HealthDrive is looking for a part or full time (3-5 days per week) Attending Physician to handle new admissions and collaborate with Nurse Practitioners that are attending to skilled nursing residents in both the sub-acute rehab and long-term care units. The Attending Physician will promote good health practices and work collaboratively as a member of the medical staff. This is an on-site position (NOT remote).

    Responsibilities

    The physician will serve as the Attending Physician for skilled nursing facilities/rehabs and will participate in administrative and quality initiatives for assigned facilitiesAttending Physician will attend and participate in monthly quality assurance/regulatory meetings and quarterly staff meetingsThe Attending Physician will participate in quality improvement initiatives, provide guidance to facility staff, oversee clinical care plans, compliance with state and federal regulations, and supervise facility clinical staffThe physician collaborates with Nurse Practitioners to manage the daily care for each resident at each facility. Each facility will have a full-time NP assigned to the facility depending on the acuity and volume of the patient populationThe Attending Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs/Rehabs

    Qualifications

    Candidate must hold a valid medical license in the state of practiceCandidate must hold a valid DEA/Controlled Substance License in the state of practiceCandidate must have completed a valid U.S. residency programCandidate must have completed a medical degree (MD or DO) from an accredited institutionValid driver’s licenseAbility to work both independently and collaboratively with on-site staffEffective communication skills with the geriatric or Medicare populationExcellent written, verbal, interpersonal and organizational skillsAbility to use email, the internet, and to learn other healthcare related software

    Physical Requirements

    Must be able to move inside a facility frequently and walk briskly in order to see patients, meet with managers/facility staff and attend to patient needsAbility to document in an EMR system on a laptop computerAbility to ascend and descend stairs in a facilitySome ability to position oneself to obtain records, tools, medical equipment and other items that may be in locations that are below the knee or above the shoulderEffectively communicate with patients, nurse practitioners, and facility staff in EnglishMust be able to lift at least 25 pounds
