    Responsibilities

    The Nurse Practitioner will manage the medical needs of skilled nursing residents in both sub-acute rehab and long-term care units.

    Day-to-Day Responsiilities:

    Provide primary and preventative care for residents in an assigned facilityOrder and study diagnostic tests (blood tests, X-rays etc.)Diagnose and treat health conditions and illnessesPropose treatments for chronic and infectious diseasesPrescribe medicationsMaintain accurate records and schedulesInform patients about their health conditions and prescribed medicationsBuild strong working relationships and work collaboratively with on-site medical staff.Oversee patients’ overall health

    Skills:

    Experience in one of the following Nurse Practitioner specialties: gerontology, acute care, cardiology, internal medicine, or endocrinology.Effective communication skills with the geriatric or Medicare populationNursing experience in ICU/ER/SNF a plus

    Qualifications

    Master’s Degree from an accredited college/universityState professional Nurse Practitioner licenseBoard Certification RequiredDEA/Controlled Substance LicenseValid driver’s licenseAbility to work both independently and collaboratively with on-site staff.Excellent written, verbal, interpersonal and organizational skills.Ability to use email, the internet, and to learn other healthcare related software.Ability to change work sites within a local area if needed

    Responsibilities

    Role Overview & Responsibilities for Podiatrists:

    Conduct comprehensive evaluations and regular podiatry care. Traditional daily patient care will include nail trimmings, corn & callous care and occasional wound care.Complete required patient and clinical documentation in our proprietary electronic medical record system.Responsible for applying proper coding guidelines and populating appropriate billing codes for services provided to patients.Adheres to OSHA guidelines regarding patient care, disposal of materials, and sterilization of equipment.

    Qualifications

    Qualifications:

    Podiatry background with proficiency in the assessment, treatment, management and evaluation of foot conditions and disorders.

    Ability to work independently on a daily basis.

    Excellent written, verbal, interpersonal and organizational skills.

    Ability to use EMR, text, email, the internet and to learn other healthcare related software.

    Self starter attitude with the desire to deliver quality, thoughtful and attentive care.

    Physical Requirements for Podiatrists:

    Ability to stand, to bend, and to lift.

    Ability to transport mobile podiatry equipment (maximum of 15 lbs.).

    Ability to assist patients with shoes and socks.

    May be occasional required to transport patients in wheelchairs.

    Education & Qualifications:

    D.P.M.Completion of a podiatric residency program.Current State professional license.Valid driver’s license.

    Travel:

    This is a mobile position. Daily travel is based on the patient care requirements for the specified service area; your Practice. A valid driver’s license and the ability to drive significant distances on occasion is required.

    Overview

    HealthDrive is looking for a Nurse Practitioner to manage the medical needs of skilled nursing residents in both the sub-acute rehab and long-term care units. The Nurse Practitioner will diagnose and treat health conditions, promote good health practices, and work collaboratively as a member of the medical staff. The APRN will also establish and build strong working relationships with administrators and staff throughout their designated building and will act as the primary point of contact for emergencies and routine medical visits. The Nurse Practitioner will be responsible for conducting physical examinations, interpreting lab tests, and overseeing patient health.
