Our providers work primarily in office based primary care as well as LTC/Nursing Facilities settings providing consultation, primary and follow-up long term care (if desired). We collaborate closely with our patients, their families, rehab staff, physicians, and hospital systems. The incoming NP will be covering mainly office based primary care but will potentially cover locations in the area from Indiana to Punxsutawney PA if so desire, 30 miles apart. This is a 4-5 day/week position.
    Job Responsibilities: Office based primary care Perform new admits, consultation, primary and follow-up care for long term care residents Round on patients, track patient progress, remove obstacles to functional progress, and improve quality of life. Engage with clinical staff on a regular basis and maintain a professional, collegial relationship with other facility physicians, caregivers, administrators, and directors.
