    Responsibilities

As a Family Centered Treatment (FCT) Specialist at Pinnacle Family Services, you'll deliver meaningful, evidence-based services where it matters most: in the homes and communities of the children and families you serve.

    What You'll Do

    As an FCT Specialist, you'll work with a caseload of children, adolescents, and families, guiding them through a structured, research-backed treatment model designed to create lasting change. Your work will be hands-on, relationship-driven, and grounded in best practices. Day to day, you can expect to:Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are — literally and figurativelyGuide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-beingServe as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person careDevelop, implement, and continuously refine individualized Service Plans in partnership with the families you serveConnect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time togetherMaintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provideParticipate in Quality Assurance activities that help make our programs stronger for every family we serve

    What to Expect

    This is community-based work, which means a flexible schedule that accommodates families' lives. Like most roles serving children and families in the community, this position includes 24/7 on-call availability for crisis support — because sometimes families need a steady, trusted voice outside of regular hours. In practice, crises don't define the day-to-day experience but knowing you're there when it counts is a meaningful part of the impact you make in this role.

    Your Path to FCT Certification

    FCT Certification is required for this role and must be completed within one year of hire.     Qualifications

As an Integrated Care Coordinator at Health Connect America, you’ll serve as a key connector for clients, providers, and community resources—ensuring comprehensive behavioral and physical health support, closing gaps in care, and promoting overall well-being. Your work will be hands-on, relationship-driven, and integral to both client outcomes and program success.

    What You'll Do

    As an Integrated Care Coordinator, you’ll oversee care planning, coordination, and monitoring for assigned clients, supporting both behavioral and physical health goals. Day to day, you can expect to:

    Conduct assessments and develop individualized, person-centered care plans based on client needs, health goals, and Social Determinants of Health (SDOH)

    Coordinate and monitor services across treatment teams, primary care providers, and community resources to ensure seamless care delivery

    Facilitate follow-up services for recent hospitalizations, life transitions, or other critical events to support smooth transitions of care

    Monitor client progress, track HEDIS gaps, and verify enrollment and payer status to ensure access to all required services

    Provide health education, guidance, and crisis response support to clients and their families as needed

    Maintain timely, accurate documentation of care coordination, assessments, and interactions in accordance with regulatory and agency standards

    Support clinic-based operations, including scheduling, reminders, vitals, chart documentation, and direct collaboration with Nurse Practitioners for integrated care clients

    Participate in marketing initiatives, outreach to new referrals, and education of prospective clients to promote engagement and program enrollment

    What to Expect

    This role requires strong organizational skills, clinical insight, and the ability to navigate complex care needs across multiple providers and settings. You’ll balance in-office responsibilities with community and client-focused engagement, supporting both ongoing wellness and crisis intervention as needed.

    As part of your role, you’ll collaborate closely with treatment teams, integrated care staff, and community partners, ensuring that every client experiences comprehensive, coordinated care that addresses both immediate needs and long-term health goals.

    Provide best practice, quality case management services, ensure that all documentation is completed per Payer, State, and COA standards and requirements. Assist in marketing new referrals.

    Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.If no wraparound facilitator is present with MYPAC, CSS will make 2 contacts per week by phone or face to face and document accordingly. Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral. Complete documentation within established time frames. Meet with clients weekly or bi-weekly for up to 12 months. (see above for MYPAC requirements). Attend and participate -weekly with Treatment Team meetings. Serve as a liaison with other professionals and agencies related to the client. Complete a recovery support plan with all individuals within the first 30 days of referral to services. Provide 24/7 on-call support for clients as per program requirements. Ensure compliance with all state regulatory bodies.

    QualificationsBachelor’s degree in human services discipline, including, but not limited to: Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc.) and experience working with children and families in case management type/ community resource position.     The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.

    Essential Duties & Responsibilities:Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements. FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA). Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus. Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule. Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards. Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician’s area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision. Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.Provide psychoeducation as indicated in the Service Plan. Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents. Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.Make referrals to services and resources on behalf of the families and children.Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs. Coordinate and oversee the initial and ongoing assessment activities. Convene the Treatment Team for Service Plan. Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process. Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations. Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members. Monitor and document the status of the consumer’s progress. Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
