    The Registered Dietitian, Diabetes Serves as a key member of the Comprehensive Diabetes Team of the Pediatric Subspecialty Endocrinology Diabetes Team, providing general diabetes education and diabetes nutrition education in accordance to ADA guidelines. This includes completing comprehensive patient assessments, formulating a plan of care in collaboration with other disciplines, and appropriate and accurate documentation in the medical record.

    Responsibilties:

    A day in the life of an Registered Dietitian, Diabetes at Hackensack Meridian Health includes:

    Completes a comprehensive diabetes educational assessment prior to initiating a plan of care. Education and counseling on diabetes and nutrition is provided to patients and/or family members. This is based on identified learning needs in a manner that demonstrates their understanding. Readiness to learn and barriers to learning are documented in the interdisciplinary education record. Performs diabetes educational assessment and develops a plan of care for patients utilizing ADA guidelines and recommendations.  Documents above in accordance with unit-based protocols, established institutional policies and procedures, New Jersey DOH regulations and accreditation agencies.  Proficient in the use of computerized educational documentation tool and uses tool to demonstrate ongoing educational progress and continuity of care. Maintains skills in computer software used for Diabetic Care and Education.  Contributes effectively as part of the comprehensive diabetes team. Communicates patient findings and shares significant information during the team conference in the comprehensive diabetic clinic service. Reports issues related to process and duties to team leader in order to promote efficiency and effectiveness of the comprehensive diabetes team. Facilitate patient flow and download blood glucose meters as needed. Collaborate with Clinician`s on Continuous Glucose Monitoring Systems including operation, sensor device insertion and downloading/interpretation of monitoring data. Develops curriculum based on ADA standards. Teaches patient centered classes on general diabetes education and diabetes nutrition. Maintains roster of clients. Schedules agreed upon number of educational classes per month. Contacts clients prior to class as a reminder in order to maintain attendance levels and provide services to as many patients as possible. Develops pre and post tests to evaluate effectiveness of instruction provided. Communicates results of pre and post test to manager and team leader on a quarterly basis. Provides accurate education to diabetes clients consistent with AADE7 Self Care Behaviors. Education content includes but is not limited to: A. Overview of the disease of diabetes, the role of insulin and glucose, and their effects on metabolism, B. Stress management C. Nutritional management D. Medication action, peak, onset and side effects, E. Insulin administration and self-testing, F. Sick day management, G. Hypo/hyperglycemic management, H. Foot and skin care, I. Lifestyle modifications. 14. Maintains Commission on Dietetic Registration re-certification requirements on a continual basis. Attends a minimum of one continuing education program annually. Documents professional development activities independently and informs supervisor of status. Maintains certification as a Certified Diabetes Care and Education Specialist. Responsible for ordering educational materials and supplies needed for the comprehensive diabetes clinic, classes, individual instruction, and general use. Maintains appropriate amount of stock on hand. Updates teaching materials as needed and outdated materials. Participates in facility/system-wide community-based activities. Assist in product incorporation and act as industry liaison for Diabetes supplies including meters, pumps, continuous glucose monitoring and insulin products. Promotes patient satisfaction through demonstrated commitment to providing excellent service by interacting in a friendly and helpful manner. Communicates potential patient satisfaction issues to manager and / or comprehensive diabetes team leader. Performance improvement: Completes data collection for PI activities as assigned. Assists in the development and implementation of changes in practice as a result of PI outcomes. Adheres to HMH Organizational competencies and standards of behavior.  Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 1 hour a day.
    Qualifications:

    Education, Knowledge, Skills and Abilities Required:

    Bachelor`s Degree in nutrition or dietetics from an accredited institution, meeting the didactic requirements of the Academy of Nutrition and Diabetes. Computer Literacy. Strong communication and counseling skills. Ability to work collaboratively in a multidisciplinary team.

    Education, Knowledge, Skills and Abilities Preferred:

    Experience with managing patients on insulin pumps and use of CGMS systems. May be required in some departments. One year of endocrinology, primary care of diabetes-related experience.

    Licenses and Certifications Required:

    Must currently be registered with the Commission on Dietetic Registration (CDR) and maintain 75 continuing education credits every 5 years in the area of nutrition counseling and medical nutrition therapy.

    Licenses and Certifications Preferred:

    The Manager, Workforce Management & Data Analytics is responsible for developing the strategy for workforce planning, healthcare technology, and performance analytics across both internal teams and vendor partners. Owns the reporting and analytics process, from ensuring data accuracy to creating executive-level dashboards and translating data into clear insights that guide business strategy to improve the patient experience. Uses advanced analytics, workforce management tools, and current industry best practices to ensure effective staff planning to deliver a high-quality, efficient service experience to patients within the PAC environment for the Hackensack Meridian Health (HMH) network. Balances daily operational leadership--which includes taking decisive action based on a deep understanding of how staffing and scheduling impact service levels and ensuring all service level agreements (SLAs) are met--with providing strategic, data-driven recommendations to senior leaders to inform long-term business decisions. Leads a team of analysts, oversees all data and reporting systems, and fosters a culture of data-driven improvement.


    Responsibilties:

    A day in the life of a Manager, Workforce Management & Data Analytics at Hackensack Meridian Health includes:

     

    Lead the overall strategy for workforce planning and analytics, designing call flows, managing Interactive Voice Response (IVR)/Intelligent Virtual Assistant/Agent (IVA) tools, and advising senior leadership on long-term capacity planning while overseeing vendor performance. Manage the complete data analytics function by setting data governance standards and overseeing all reporting, while using advanced analytics and Artificial Intelligence (AI) to analyze data from key systems--including WFM software, EMRs (i.e., EPIC), and Knowledge Management--as well as patient surveys to drive performance improvements. Monitor and react to SLAs by managing and performing recovery actions to ensure a high level of service.     The Director of Quality Initiatives and Improvement (DQI) leads all hospital-based work to eliminate preventable harm to patients, family members, and staff and to attain unsurpassed clinical and patient-reported outcomes for assigned hospital(s). Builds organization-wide participation in improvement through transparency, collaboration and mutual learning. Collaborates with leaders to achieve the Hackensack Meridian Health (HMH) quality goals and eliminate preventable harm. Facilitates the redesign of patient care processes; leads root cause analyses process, analyzes data to inform and guide improvement efforts, and develops organizational capacity for improvement through teaching and mentorship. While directing the assessment, planning, implementation, monitoring and evaluation of best practices, this position ensures that cost effective, quality care is administered to all patients in accordance with the hospital performance improvement (PI) plan, while complying with the standards of all regulatory and accrediting agencies. Provides leadership in the implementation and integration of the Department of Patient Care Performance Improvement Plan in addition to ensuring Joint Commission on Accreditation of Healthcare Organizations (JCAHO) & regulatory compliance.     Responsibilties:

    A day in the life of a Director of Quality Initiatives and Improvement (DQI) at Hackensack Meridian Health includes:

     

    Provides leadership to all safety and quality improvement activities at a hospital including committee meetings, medical staff peer review, root cause and apparent cause analyses, event management, morning safety report, follow up of ONELink event reports, and specific improvement cycles including optimization of STEMI care. Provides leadership to local high reliability organization (HRO) transformation. Engages all levels of leadership, caregivers and staff in advancing patient safety through HRO training, morning safety huddles, and joint event management with the departments of Human Experience and Risk Management.Oversees the Nurse Practitioner service and ensures they are deployed in an efficient manner to further improve our quality metrics, improve throughput and LOS and support patient experience improvement efforts. Develops and oversees organizational quality initiatives and the monitoring of quality priorities. Presents quality data results with analysis and recommendation to a variety of organizational committees and councils including Department of Patient Care to enhance achievement of HMH quality goals.     The Outcomes Analyst for the Women's Service Line functions as a key member of the Department of Patient Safety and Quality, leveraging comprehensive data analysis to drive measurable improvements in patient safety and quality of care for the maternal health population. Working in close collaboration with the OB (obstetrics) and Maternal Health nursing teams, physicians, and departmental leadership, this role is responsible for identifying trends, measuring performance against national benchmarks, and creating customised reports, dashboards, and graphs. Transforms complex data into actionable insights, support evidence-based practice changes, and audit data for validity and accuracy. As a key resource for the Network's Women's Service in its journey to becoming a Highly Reliable Organisation (HRO), this position will support the leadership team in implementing and sustaining strategies that foster a culture of safety and excellence within the obstetrics and maternal health department.

     

    Prior experience working within Maternal Heath Data Analytics is preferred.  This includes knowledge of maternal health key metrics and publically reported data such as US News & World Report.  Will also be responsible for monitoring inernal dashboards and collaborating with the team to develop quality improvement processes.

     

    This position is based in Edison, but will require travel throughout the network.

    The Patient Care Technician functions as a caregiver for a specific patient population in order to achieve desired outcomes. The Discharge Summary reflects the resident's current status, outcomes achieved, and recommendations for further services or interventions, as appropriate. Documentation of Re-Evaluations: Re-evaluations are completed in a timely manner. Goals and treatment plan updated as per departmental policy. Effective Communication: Develops a rapport and mutual respect with the resident to foster an optimum environment for goal achievement. Provides education to residents/families based upon the identified learning needs, in a manner that the residents/families can demonstrate their understanding. Evaluation Information Gathering: Gathers all information necessary to properly evaluate the resident's needs as per department policy, including testing and consultations with other health care professionals, as appropriate. The content of the evaluation contains data as outlined by departmental policies and is completed within time frames as specified in departmental policies. Other Duties As Assigned: Handles special projects and rush assignments as needed, in a timely manner. Patient Rapport: Develops a rapport and mutual respect with the patient/caregiver and team members to foster an optimum environment of goal achievement. Patient/Caregiver Education: Provides and documents education to patients/caregivers based upon identified learning needs in an understandable manner. Plan of Care: Develops and implements an individualized treatment program based on established goals.  Productivity: Completes patient treatments within acceptable departmental productivity range and documents on daily charge sheet. Prosthetic/Orthotic Devices: Demonstrates appropriate judgment in ordering and measuring the resident for a prosthetic/orthotic device. Assesses the fit and function of the device and the need for adjustments to ensure proper utilization. Develops with the resident and other personnel, as appropriate, a wearing schedule to begin functional use. Educates the resident/family about the proper fit and function of the device and signs of malfunction which would indicate the need for intervention.  Resident Goal Setting: Formulates functionally based/objective short and long term goals. Incorporates evaluative findings in establishing time oriented functional and measurable short and long term goals.  Respect of Resident's needs: Administers the evaluations in a manner appropriate to the resident's disability, demonstrating respect for the resident's needs. Safety and Body Mechanics: Therapy is provided in a safe environment, utilizing proper body mechanics.  Treatment Program: Develops and implements an individualized program of treatment focused on identified resident needs. Focuses on achievement of the established goals. Participates in the ongoing development of the discharge plan that was initiated at the time of initial evaluation. Provides the appropriate assistive/adaptive devices and durable medical equipment to the resident to improve their independence in function. Ensures the proper utilization and clinical appropriateness of the device/equipment and educates the resident/family about its use and safety. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.
    Qualifications:

    Education, Knowledge, Skills and Abilities Required:

    Graduate of an APTA accredited program or equivalent. Standing, walking and/or continuously moving around. Kneeling, stooping, crouching, crawling or working in other unusual physical positions. Climbing (including ladders, stepping stools, and/or stairs) or balancing. Stretching and/or reaching to retrieve/replace object.  Pushing, pulling, lifting and/or carrying heavy objects (20 lbs. or more) without aid of physical devices. Dexterity and eye-hand coordination to perform activities which involve holding, positioning, manipulating, typing (keyboarding or data entry), etc. Visual ability to monitor instruments and computers, distinguish characters (letters, numbers, etc.) recognize colors, observe physical characteristics, etc. Hearing ability to recognize sounds, sound patterns and/or other auditory stimuli. Physical ability to react/respond quickly in emergencies, including life threatening situations. Excellent written and verbal communication skills.Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. 

    Licenses and Certifications Required:

    Physical Therapist License. AHA Basic Health Care Life Support HCP Certification.

    The Physical Therapist Per Diem evaluates, plans and implements a goal oriented Physical Therapy treatment program that focuses on restoring function and preventing further disability as a result of trauma, disease, amputation, congenital defects, surgery and other debilitating illness.

     

    The ideal candidate will have experience in an ambulatory outpatient physical therapy setting.

     


    Responsibilties:
