This position will provide support to patients of the Residential Treatment Facility throughout their treatment. The Patient Engagement Coordinator (PEC) is responsible for engaging patients on a daily basis by providing activity services, crisis intervention, and connecting with the various interests of the patient as a means of engagement. All interactions will be documented in the patients EMR. PECs help guide the patient in appropriate behaviors, active participation in treatment and reflection on the benefits of their participation in programming. They encourage patients to attend all groups, medical appointment, and clinical appointments.
Essential Job Functions
Responsible for completing EMR notes during the proper shift location. This should be completed in a clear and timely manner.Ensure patients are acting appropriately and respectfully during groups and are not leaving groups for unexcused reasons. If a patient does leave, their name should be removed from the attendance sheet unless they have attended the majority of the group and/or had an appointment.Review patient handbook and assist in orienting patient to the programProvide educational and support groups for patientsAssist with the transport of PHP patients to off-site activitiesProvides individual support to detox patients by meeting with them on a daily basis, reviewing programming requirements and expectations, and providing concierge service to help engage detox patients in the program.Assist with AMA (Against Medical Advice) discharge rescuesAvailable to patients for crisis intervention Will practice individualized coping skills with patients Assist clinical department in the planning and execution of all engagement activities throughout the month Document on a weekly basis the number of patients with whom they intervened; noting the number of patients who left AMA and the number of patients who were successfully retained in treatment. Provides services based on specific skill set to engage patients For example, Personal Trainers will provide services surrounding physical fitness activities; a skill set of arts and music will provide engagement activities surrounding these modalities; a skill set of gardening will engage patients in creating and tending to the onsite garden. Coordinates activity services based on individualized skill setPatient Engagement Coordinator who are assigned concierge care will assist with crisis intervention with patients and be engaged with the community to promote positive interactions, decrease AMA potentials with patients and provide additional attention to those patients who are struggling with engagement in treatment. This will include services such as assisting patients with getting to groups, meeting their appointments, join in on campus activities and attempt to remove barriers to treatment for patients.

Full Time - Recovery Support Specialist
Our Recovery Support Staff team works hands-on with clients on a non-clinical basis throughout the duration of the clients stay. RSS is tasked with 24-hour direct supervision of clients, holding each client personally accountable through the enforcement of policy and procedure. RSS serves each client by offering growth opportunities, encouragement and support based upon the application of the 12 Steps into real life situations. Other RSS duties include the observation of client medication administration, transportation of clients to and from 12 Step meetings, medical appointments and the gym, documentation of observed behavioral issues and assisting in the new client admissions process.

The Recovery Case Manager (RCM) will meet with their clients regularly on a one-on-one basis. The RCM is the primary contact for a client's family, loved ones, and referral sources. They are responsible for the planning and implementation of client’s aftercare. RCM’s also serve to assist clients with their outside needs (e.g. court hearings, application for unemployment benefits, FMLA).
Responsibilities:
Help hold clients accountable for their actions in the community and the completion of their work Facilitate and provide timely documentation Monitor compliance on client contracts, phone policy, “buddy” relations. Ensure that all pertinent information is notated on RSS office board at shift change Ability to work in a fast-paced, dynamic environment Ability to communicate effectively in written, electronic, and oral form Ability to make sound decisions and/or find alternative solutions Make weekly calls to the client’s family (more communication may be required). Behavioral Health Tech – Residential Treatment Facility
Granite Recovery Center is looking for a Behavioral Health Technician provides direct support to the clients and the Clinical/Operations Team at the facility. The Behavioral Health Technician is responsible for monitoring and documenting self-administration of medication and observing, collecting, and testing urine samples submitted by clients in accordance with policy and procedure. A Behavioral Health Technician is required to fill out the required forms for the testing, report positive test results, and submit forms accurately and in a timely manner.
Role and Responsibilities:
Provide supervision and support to clients at the residential treatment center.Assist and monitor self-administration of medication.Offer clients guidance about recovery using the 12-step model.Conduct random urinalysis drug testing 2-3x weekly and as outlined procedurally.Monitor all drug tests by observing samples being provided via same sex staff to client interaction.Document all results of each sample provided and have client sign and date the form used.Maintain records of urine submissions, and client data related to self-administration of medication.Contact the Clinical Director and residential director immediately should a conflict arise regarding U/A process or self -administration of medication process.Report any unusual client behavior or suspicious activity to the Clinical Director and Residential Director.Maintain high standards of ethical and professional conduct and adhere strictly to confidentiality requirements.Comply with all policies and procedures.Adhere to established standards of hygiene and universal infection control procedures in handling all samples and supplies.Qualifications and Education Requirements:

Full Time - Recovery Case Manager

The Recovery Case Manager (RCM) will meet with their clients regularly on a one-on-one basis. The RCM is the primary contact for a client's family, loved ones, and referral sources. They are responsible for the planning and implementation of client’s aftercare. RCM’s also serve to assist clients with their outside needs (e.g. court hearings, application for unemployment benefits, FMLA).
Responsibilities:
Help hold clients accountable for their actions in the community and the completion of their work Facilitate and provide timely documentation Monitor compliance on client contracts, phone policy, “buddy” relations. Ensure that all pertinent information is notated on RSS office board at shift change Ability to work in a fast-paced, dynamic environment Ability to communicate effectively in written, electronic, and oral form Ability to make sound decisions and/or find alternative solutions Make weekly calls to the client’s family (more communication may be required). This communication will be logged in the EMR. Phone calls will address client progress, behavior, contracts, growth opportunities, programming, and aftercare. RCM must be prepared to discuss enabling, codependency, and/or Al-Anon. RCM is responsible for scheduling all calls and face-to-face meetings with family members and/or loved ones Family calls are to be within the first 24 hours of admission Referral calls are to be made within the first 24 hours of admission. Calls to referral sources are to be made weekly Meet with clients at least once a week to monitor the treatment process Responsible for executing step downs and discharges Uploading/delegating current ROIs and aftercare plans into EMR RCMs are expected to attend the following meetings: Morning Meeting, RCM Meeting, Supervision, when necessary, Safety Squad and Quality Assurance Client engagement is not limited individual case load Introductions are to be made with new clients on caseload within 48 hours of admission; interaction must be logged in the EMRQualifications:
High school diploma or GED required Extensive experience within Mental Health and Substance Use Disorder industry Time management skills and the ability to work on multiple projects simultaneously Coordinate with all departments and oversees clients stay at GMTC Creation of an after-care plan that realistically meets the needs of the client Vetting of client visitors and phone calls Must meet then maintain all pre-employment requirements which includes reference checks, applicable state and national required background screening, 10 panel urine drug screen, DMV check, insurance (if applicable), and license/credential verificationsBenefits:
