    POSITION SUMMARY

    As a Caregiver, you will provide non-medical support and companionship to elderly clients in their homes. The caregiver is responsible for assisting clients with their daily activities of living including but not limited to – bathing, grooming, toileting, transfers, meal preparation, transportation, light housekeeping, pet care etc. The caregiver duties are not limited to those listed in the job description and may change depending on the needs of the client. The Caregiver will mostly work independently of co-workers and/or supervisors and therefore must balance good judgement and initiative while adhering to Golden Years Policies and Procedures. This role is ideal for someone who is empathetic, dependable, and passionate about helping others live with dignity and comfort.
    PLACE OF EMPLOYMENT

    Caregivers work in the client’s home or places of residence (assisted living facilities, nursing homes, etc.). Caregivers are responsible for their own transportation to and from work sites. 

    RESPONSIBILITIES

    ·       Provide companionship and emotional support to elderly clients in their place of residence·       Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support·       Help with light housekeeping such as dishes, laundry, vacuuming, etc.·       Provide medication reminders (no medication administration)·       Assist with errands, transportation to appointments, and grocery shopping·       Follow individualized care plans and communicate any changes in client’s needs to supervisors·       Maintain accurate and daily care notes and documentation·       Ensure a safe and clean environment for the client
    QUALIFICATIONS

    ·       Must be 18 years of age or older·       Reliable transportation to and from the job site·       Valid US Work Authorization·       Ability to pass a criminal background check, and all required registry checks ·       Valid CPR and First Aid Certification ·       Strong interpersonal and communication skills·       A work style that is respectful, honest, and transparent·       Patience, reliability, and a commitment to supporting Golden Years clients·       Able to read and understand instructions, perform electronic record keeping, and write reports and daily notes as required·       Effectively read, write, and communicate verbally in English·       Ability to lift 50 lbs. and provide physical support as required

    POSITION: Direct Support Professional

    REPORTS TO: Residential Administrator

    POSITION SUMMARY

    Direct Support Professionals (DSPs) are responsible for supporting individuals with intellectual and/or developmental disabilities in achieving greater independence and enhancing their quality of life. DSPs will play a vital role in providing hands-on support in daily living, community integration, and personal development. DSPs will empower the people we support in living self-directed lives and developing the skills necessary to live the lives they deserve and desire.

    RESPONSIBILITIES

    · Assist individuals with daily living activities such as bathing, dressing, grooming, toileting, and meal preparation.

    · Encourage and empower the people we support to live as independently as they desire

    · Promote social skills, community involvement, and individual choice

    · Assist the people we support in gaining meaningful access to the community in the ways in which they desire

    · Utilize the Person-Centered Support Plan (PCSP) to assess the supports needs and provide the level of support dictated in the PCSP

    · Transport and accompany the people we support to appointments, errands, community events, and/or recreational activities

    · Provide support in medication administration and complete appropriate documentation (if certified to do so)

    · Provide guidance on a range of developmental goals. Goals may include community participation, financial budgeting, exercise, nutrition, vocational skills, and/or interpersonal skills.

    · Advocate for the rights, dignity, and preferences of each person supported

    · Adhere to a person-centered approach with working with the individuals we support

    · Monitor and ensure a safe, clean, and supportive environment for the people we support

    · Implement any specialized plans (behavior, nutrition, etc.)

    · Communicate effectively with other team members, family and/or natural supports of the person supported, other providers, and other members of the person’s team

    · Collaborate with supervisors, team members, families, and other member’s of the person’s team to ensure the highest quality of care

    · Complete all required documentation within the required timeframes

    QUALIFICATIONS

    · Must be 18 years of age or older

    · Reliable transportation to and from the job site

    · Valid US Work Authorization

    · Ability to pass a criminal background check, and all required registry checks

    · Valid CPR and First Aid Certification from an approved entity

    · Strong interpersonal and communication skills

    · Patience, reliability, and a commitment to supporting people with intellectual and developmental disabilities

    · Able to read and understand instructions, perform electronic record keeping, and write reports and daily notes as required

    · Effectively read, write, and communicate verbally in English

    · Must be able to complete electronic training required by the Department of Disability and Aging in the Relias system

    Golden Years provides supports and services to adults with intellectual and developmental disabilities in Tennessee. We offer Supported Living and Family Model Services to the people we support through the ECF Choices and Choices Program.

    About the Family Model Program
    The Family Model Program is an opportunity for you to provide care to an adult with an intellectual and/or developmental disability in your home. Similar to the Foster Care model for Children, the Family Model Program offers the people we support an opportunity to live in a family home and receive the support they need to be as independent as they desire.

    General Duties
    Implement the individuals Person-Centered Support Plan
    Assist with Daily Living Skills including but not limited to, meal preparation, feeding, personal hygiene/grooming, bathing, as necessary.
    Administer Medication(s) as necessary
    Complete required daily/monthly documentation
    Provide a safe and nurturing home to an adult with intellectual and/or developmental disabilities

    Requirements
    Must have a HS Diploma
    Must have a home that meets the requirements for the program. Home will be inspected by Agency Staff to determine suitability.
    Current CPR/First Aid Certification
    Medication Administration Certification (can be obtained after hiring)
    Active homeowners/renters insurance
    Valid Drivers License and automobile insurance
    Must have reliable transportation and the ability to transport the client as necessary
    Experience working with adults with Intellectual/Developmental Disabilities

    Pay
    $50/day - $195/day based on the level of the client.
