    Description:
    This position is responsible for the coordination of care for patients who have heart transplants.
    Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, monitoring for organ rejection, monitoring for infection, and other routine patient monitoring.
    Experience managing other organ transplant recipients is preferred.
    Knowledge of immunosuppressive therapies is preferred.
    This role involves the assessment of post-heart transplant graft function and general health status.
    The Care Coordinator will collaboratively determine the need for additional assessment or intervention.
    Changes to medication regimens or other regimens will be coordinated with the attending physician or advanced practice provider and pharmacist.
    Frequent oversight of laboratory results is required to detect any changes that may need immediate action.
    Timely communication with multidisciplinary team members, patients, and families regarding patient care issues is critical.
    Facilitating patient activities with consulting departments and physicians is part of the role.
    Assistance will be provided to other nurses and staff in assessing, planning, implementing, and evaluating care of the transplant patient.
    Assessment of patient and family educational needs is necessary, and these needs will be communicated to appropriate services such as financial counseling, social work, and nutrition.
    The Care Coordinator will ensure coordination of all post-heart transplant patient care across the continuum, following provider orders and protocols.
    The role may require floating to other departments, units, or facilities based on job specifications.
