    Job Summary:

    Duties and Responsibilities:

    Job Summary:

    A Peer Mentor should have the willingness to share lived experience with purpose and intent: good listening skills: nonjudgmental ability to foster self-sufficiency; the desire to work in a natural family and youth setting; ability to establish trusting relationships through compassion, embracing of diversity and empathy and insight with TAYA.

    Duties and Responsibilities:

    Support, coach and train youth in connecting with community resources and services that help sustain the youth’s optimal function in the community.Attend and coordinate youth advisory group meetingAttend and participate in all training sessions and group meetings.Work both independently and as part of a group with fellow peer mentor’sProvide outreach to the community to engage in Transitional Age Youth and Young Adults (TAYA); provided information and linkages to mental health, substance use, primary healthcare, supportive housing employment, education, social support, BenePlan, and DTA.Work as a team with Recovery Coach to meet the co-occurring needs of TAYAAttend System of Care and other system meetings to infuse youth and young adult’s voice and perspective.Meet individually with youth and family and summit report to the supervisor or coordinator.Collect data to support quality of improvementMaintain Social Marketing strategies develops through the grant

    Minimum Qualifications:

    GED or high school diploma and 2 year experience working with children/adolescents age youth OR Bachelor’s degree in human services field from an accredited university and one year experience working with children/ adolescents age youthRequires a current driver’s license and reliable vehicle for transportationAbility to work effectively with other community agenciesRequires good organization skills and comfort negotiating multi-level systems on behalf of client and program.Experience overcoming difficult situations and preferably personal experience with mental health services. Lived experience is defined as: the personal experience of living with and overcoming a mental health/ substance abuse/ traumatic life concern which has caused difficulties in one or more areas of life.     The Supportive Housing Case Worker provides on-going case management services to individuals and families in recovery from substance use disorder. These services are provided either in the office, participants home, or/and in the community to support recovery and housing stability.

    Duties and Responsibilities:

    Job Summary:

    The Care Coordinator facilitates care planning and coordination of services for youth, with serious emotional disturbance (SED), under the age of 21. Care Coordinators ensure that necessary services are accessed, coordinated, and delivered in a strength-based, individualized, family/youth-driven, and ethnically, culturally, and linguistically relevant manner. Services and supports, which are guided by the needs of the youth, are developed through a Wraparound planning process that results in an individualized and flexible plan of care for the youth and family. When providing care coordination, the Care Coordinator and a Family Partner work together with youth and their family while maintaining their distinct functions.

    Duties and Responsibilities:

    Submit detailed progress notes that follow federal, state, and agency requirements.Conduct comprehensive home-based assessments and behavioral health assessments inclusive of the age appropriate version of the Massachusetts Child and Adolescent Needs and Strengths (CANS) that occurs in the youth’s home or another location of the family’s choice.Maintain a caseload of 11-14 families.Oversees the assembly of the Care Planning Team, and guides the development of an Individual Care Plan and a risk management/safety plan.Monitors the provision and quality of services provided to the family and is the liaison when new services/resources are needed to be sought or developed.Provides or arranges for transportation for clients to appointments, crisis/respite services, etc. if needed.Provides or secures support and crisis/emergency services for the youth/family.Collaborates with other necessary individuals the youth and family may have contact with, in order to properly coordinate care.Identifies community resources and development of natural supports for youth and parent/caregiver(s) to support and sustain achievement of the youth’s individual care plan, goals and objectives.

    Minimum Qualifications

    Minimum Experience Required: 1 year.Bilingual and Bicultural encourage to apply - Portuguese & English - Spanish & EnglishDegree Required: Must possess an MCE approved Master’s or Bachelor's Degree in one of the following: Family Therapy, Counseling and Psychological Services, Human Services, Community Psychology, etc.Driver’s License and proper auto insurance required.Awareness and sensitivity to contextual variables such as race, culture, gender, sexuality, disability, economics and lived experience.Bilingual Spanish/English
