    Job Summary and Qualifications
    Position Objective:

    The RN Behavioral Health is primarily responsible for adhering to the hospital's Standards of Patient Care to deliver safe and quality patient care. The RN performs all treatments and procedures as outlined in the Standards Manual and applicable regulatory and professional entities/ guidelines, standards, protocols and procedures within the scope of RN practice.

    Patient Population Served :

    The Geropsychiatric Unit RN is expected to demonstrate the knowledge and skills necessary to provide care based on the physical ability, intellectual development, psychosocial, education and safety needs of the geriatric patient.

    Treating patients 55 years and older for acute psychiatric needs

    Essential Position Functions :

    The following functions must be able to be carried out by the incumbent with or without reasonable accommodation:

    Assessment

    Interviews patient and caregiver/family on admission to obtain a bio-psycho-social history relevant to reason for admission.

    Assesses the patient's physical. emotional and psychosocial status.

    Reviews all test results including neuropsychological, lab, x-ray for ongoing assessment and reporting to physicians.

    Assesses patient and family/caregiver for discharge needs.

    Assesses patient status on an ongoing basis.

    Communicates patient needs/assessment to physicians/APRN, GPU social worker.

    Assesses for developmental needs.

    Planning of Care

    Develops a written nursing care plan with patient and family input that is individualized to patient needs, supportive of clinical diagnostic presentation and supportive of all skills and abilities.

    Coordinates and documents the interdisciplinary plan of care.

    Directs the patient's participation in the therapeutic milieu and bio-psychosocial program.

    Develops teaching plans to meet the learning needs of patients, family, and caregiver relevant to discharge needs.

    Implementation of Interdisciplinary Plan

    Acknowledges, validates accurate transcription, coordinates and implements physician orders.

    Applies the interdisciplinary plan of care, monitoring for effectiveness.

    Administers medications and monitors for desired, side or adverse effects.

    Provides instruction to patient.

    Applies behavioral and supportive therapeutic approaches with individuals and groups to accomplish

    clinically sound behavior management outcomes.

    Evaluating Plan of Care

    Reviews and updates the interdisciplinary treatment plan in collaboration with the interdisciplinary team, the patient and the family.

    Communicates ongoing care needs to caregivers regarding discharge planning (nursing homes, group homes, families, etc.)

    Establishes a therapeutic relationship with the patient focused on empathy and clinically appropriate, evidence based treatment modalities.

    Applies principles and techniques of crisis prevention and intervention.

    Maintains a safe therapeutic environment for patients, families, staff, and corrects hazards promptly. Adheres to hospital safety policy and procedure.

    Provides patient education to patient/family minimally regarding the hospitalization disease process and medications.

    Documents all pertinent information on the patient's medical record in an accurate, detailed manner adhering to policy and format for charting. Documentation is relevant, legible, clear and concise.

    Assumes Charge Nurse role upon demonstrated knowledge of responsibilities when assigned.

    Participates actively in department meetings: patient care rounds, interdisciplinary staff meetings and nursing staff meetings.

    Supports Department's customer service efforts. Provides service excellence to patients and their families, medical staff, peers, support services and other hospital personnel. Provides immediate and satisfactory problem resolution for valid customer complaints or refers complex customer service problems to the Clinical Coordinator.

    Demonstrates knowledge of, and support for, hospital Mission and Values Statement, department goals and objectives. Assists in development and implementation of department mission and values statement, goals and objectives.

    Develops and maintains professional relationship with physicians, peers, and medical support services, demonstrating an internal customer service attitude of openness, constructive problem solving and respect for all.

    Follows the principles of infection control as outlined in the Policy and Procedure manual including the initiating of airborne droplet and strict precautions as soon as patient=s symptoms/lab values indicate they are needed.

    Participates in the department's Performance Improvement program by collaborating with the department's PI representative in developing and implementing effective PI programs.

    Assumes responsibility for own professional behavior, development and on-going education with self- awareness of strengths and areas for improvement. Self-initiates learning/growth activities. Keeps Clinical Coordinator informed of mandatory education/competency compliance.

    Demonstrates a commitment to containing costs and efficient use of material resources.

    Non-Essential Position Functions :

    Performs other duties as assigned or as judgment of necessity dictates. Participates in hospital/unit committees.

    What qualifications you will need: Graduate of an accredited school of nursing.

    Current licensure as a Registered Nurse in the State of New Hampshire or a compact state.

    Active BLS

    Active Nonviolent Crisis Intervention Certification within 90 days

    Job Summary and Qualifications The Emergency Department Registered Nurse provides professional nursing care within the Emergency Department and coordinates patient care planning with other disciplines.

    Patient Population Served :

    The Emergency Department Registered Nurse will demonstrate the knowledge and skills necessary to provide care based on the physical ability, intellectual development, psychosocial, educational and safety needs of the neo-natal, pediatric, adolescent, adult and geriatric patient.

    Essential Position Functions :

    The following are essential job functions that must be able to be carried out by the Emergency Department Registered Nurse with or without reasonable accommodations. The Emergency Department RN follows the Nursing Process of assessment and priority setting, planning, implementation and evaluation of all ED patients.

    Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.

    Has the ability to perform a head-to-toe assessment and reassessments on all patients as per FMH Standards for assessment & reassessment.

    Has the ability to monitor hemodynamic status of patient and correctly interpret the results.

    Demonstrates knowledge of cardiac monitoring, identifies dysrhythmias and treats appropriately.

    Accurately assigns triage categories.

    Maintains accurate and continued nursing documentation including patient histories, conditions, treatments, responses and assessment of changes.

    Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and families. Functions as a liaison between administration, patients, physicians and other healthcare providers.

    Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patients served. Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group.

    Interacts professionally with patient and family and involves patient and family in the formation of the plan of care.

    Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family in teaching as appropriate.

    Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition.

    Prepares, hangs, monitors and discontinues intravenous fluids

    Adheres to established departmental policies and procedures, objectives, performance improvement initiatives and safety, environmental and infection control standards.

    Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with Hospital standards.

    Assists in cost containment through the appropriate ordering and conservation of supplies and equipment.

    Cooperates with other personnel to achieve departmental objectives; maintains good employee relations and interdepartmental objectives.

    Responds promptly to changes in workload, adjusting assignments as necessary to achieve optimal productivity and efficiency.

    Provides resource information to other staff.

    Demonstrates the ability to assist physicians with procedures in the Emergency department.

    Can perform the fundamentals of dispatching ambulances when necessary.

    Can receive prehospital reports utilizing the radio and/or telephone communication.

    Supervises unit coordinators, ED technicians and delegates and monitors where appropriate.

    Orients new employees to the Emergency Department.

    Assists the Director of Emergency Services in the development, implementation and evaluation of performance improvement indicators to improve patient outcomes and the work environment.

    Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops; shows responsibility for own professional practice and ongoing education.

    Performs other related duties as assigned or requested within the scope of the NH Nurse Practice Act.

    Job qualifications:

    Education/Experience:

    Graduate of an accredited school of nursing with 1 year Registered Nurse experience in a critical care specialty preferred.

    Current New Hampshire State Registered Nurse License or Compact State License Current Basic Life Support certification (BLS)

    Advanced Cardiac Life Support (ACLS) Certification within 6 months of hire. Pediatric Advanced Life Support (PALS) Certification within 1 year of hire. Emergency Nurse Pediatric Core Course (ENPC) Certification recommended. Trauma Nurse Course (TNCC) recommended.

    Certified Emergency Nurse (CEN) recommended.

    Membership in the Emergency Nurses Association (ENA) encouraged

    Assessment of In-patients

    Interviews patients and caregiver/family on admission to obtain a bio-psycho-social history relevant to reason for admission.

    Examines the patient's physical status.

    Reviews all test results including EKG, lab, x-ray for ongoing assessment and reporting to physicians.

    Assesses patient and family/caregiver for discharge needs.

    Re-assesses patient status at established time frames.

    Communicates patient needs/assessment to physicians and interdepartmental services involved in the patient's care.

    Ensures thorough, accurate documentation of relevant data obtained during the assessment process.

    Ensures sensitivity towards patients and caregiver/family by eliciting patient's physical and emotional concerns, being a receptive listener, and providing an environment conducive to interviewing and the delivery of care.

    Assesses information received from computerized cardiac and pulmonary monitoring equipment.

    Planning of Care

    Develops a written nursing care plan with patient and family input that is individualized to patient needs, strengths and weaknesses.

    Coordinates the interdisciplinary plan of care.

    Directs the patient's and caregiver/family's participation in the planning and delivery of care.

    Develops teaching plans to meet the learning needs of patients, family, and caregiver relevant to interdisciplinary discharge needs.

    Accepts the role of the Admissions Nurse and admits patients in collaboration with the unit to which it is assigned.

    Maintains a safe and therapeutic environment for patients, families, staff; corrects hazards promptly. Adheres to hospital safety policy and procedure.

    Documents all pertinent information on the patient's medical record in an accurate, detailed manner adhering to procedures for charting.

    Job Summary and Qualifications

    The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

    What you will do in this role:

    Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care.Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team.Collaborates with subject matter experts and other managers to create an environment of teamwork that supports improved outcomes and service.Supports a patient-first philosophy and engages in service recovery when necessary.Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives. Provides recommendations related to interviewing, selecting, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure a competitively better organization.Assists with staff scheduling. Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.Supports proper inventory control and assists with managing supplies and equipment.
