Fraser is currently hiring a Designated House Coordinator to join our Community Living team. As a DHC, you will provide site-specific supervision and leadership to assure continuous quality improvement in active treatment service development and implantation at one of our Supervised or Supportive Living locations.
Requirements:
Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.
HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experienceBA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilitiesResponsibilities:
Manage staff coverage to meet the needs of the clients servedCoordinate completion of all household repairs and other upkeepCoordinate the activities of daily living including personal care, hygiene, and groomingProvide direct cares to our clientsAssure documentation for all areas of client care are completed Collaborate with medical professionals to coordinate health and medical needs for each clientCoordinate weekly, meal planning, and nutrition for clientsFraser Offers:
