Position Summary:
Is an ambassador of Four Seasons mission to enable patients to take full advantage of their hospice benefit by performing 2-4 admissions daily depending on length of shift and complexity of patient. May also perform patient visits, if necessary, providing direct nursing care. At times they may receive assignments that are a combination of admissions & patient visits – adjusted accordingly depending on the mix of each. Duties will include performing a comprehensive assessment of patients to identify needs.
Essential Duties & Responsibilities:Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens. Record and report the results of the nursing assessment. Plan, initiate, deliver, and evaluate appropriate nursing acts. Teach, assign, or supervise other personnel in implementing the treatment regimen. Collaborate with other health care providers in determine the appropriate patient and family-centered plan.Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen. Provide teaching and counseling about the patient's health. Report and record the plan for care, nursing care given, and the patient's response to that care. Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services. Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.
