    Position Summary:

    Is an ambassador of Four Seasons mission to enable patients to take full advantage of their hospice benefit by performing 2-4 admissions daily depending on length of shift and complexity of patient. May also perform patient visits, if necessary, providing direct nursing care. At times they may receive assignments that are a combination of admissions & patient visits – adjusted accordingly depending on the mix of each. Duties will include performing a comprehensive assessment of patients to identify needs.

    Essential Duties & Responsibilities:Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens. Record and report the results of the nursing assessment. Plan, initiate, deliver, and evaluate appropriate nursing acts. Teach, assign, or supervise other personnel in implementing the treatment regimen. Collaborate with other health care providers in determine the appropriate patient and family-centered plan.Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen. Provide teaching and counseling about the patient's health. Report and record the plan for care, nursing care given, and the patient's response to that care. Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services. Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly. Knowledge, Skills & Abilities:

    Must be willing and able to care for patients with infectious disease processes unless approved accommodationsKnowledge of Federal, State and Local regulations and Medicare Conditions of ParticipationKnowledge of HIPAA privacy rules and regulationsKnowledge of electronic medical records systemsDemonstrate basic data entry skills Demonstrate excellent organization skillsDemonstrate excellent interpersonal skillsDemonstrate excellent written and oral communication skillsAbility to work under pressure to meet deadlinesAbility to take initiative and utilize innovative techniques and ingenuityAbility to interact with various departments throughout the organizationAbility to work flexible hours, if neededAbility to participate as a team player to coordinate projectsAbility to plan, prioritize and coordinate multiple projectsDemonstrate skill/expertise in pain and symptom managementDemonstrate ability to effectively collaborate and facilitate the work of a groupDemonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation Minimum Qualifications:

    Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneelingFlexion/Extension of arms/wristHand dexterity to include gripping, pushing/pulling and writingLifting/carrying of 50 lbs. often Intent & Function:

    Location: Serves Macon, Jackson, Clay, Swain, Graham, and Cherokee counties,

    Position Summary:

    Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

    Essential Duties & Responsibilities:Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation Serve as first responder for symptom-based calls from current patients after normal office hoursProvide guidance to patients and families as needed or communicate need for case manager or designee follow up promptlyParticipate in implementation and coordination of patient’s hospice careAssist with emergency admissionsAssess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens. Record and report the results of the nursing assessment. Plan, initiate, deliver, and evaluate appropriate nursing acts. Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
