Job Description
Under the direct supervision of the IHART Program Director, the Behavioral Health Specialist (BHS) will be responsible for mental health and substance use assessment, wellness services, and crisis prevention. BHS will follow the principles of harm reduction, trauma-informed care, and recovery and wellness to engage clients in therapeutic interventions, link them with appropriate social and mental health services in addition to providing linkages to shelter, public benefits and other services to support them on their path to wellness.
Specific Responsibilities
Works a flexible, full-time schedule to accommodate program/client needs. Evening and weekend hours are required to accommodate a 12/7 day a week schedule. This is a community based highly mobile position.Performs intake and comprehensive assessments with adults who may have psychiatric and dual diagnosis disabilities (substance use disorders).Provides clinical case management services including evaluations, service plan development, assisting clients with accessing housing, financial, medical, and other needed services. Coordinates with other service providers.Provides services to address underlying mental health concerns such as depression, anxiety, complex trauma, and addiction within the context of a harm reduction approach and strength-based client centered work.Provides crisis prevention planning and crisis intervention and support, as appropriate.Conducts aggressive community outreach and engagement to safely provide services to clients sagely within the community, as needed.Maintains clinical records after each client contact or service, in accordance with Felton Institutes mandated standards and Antioch City Council requirements.Contributes to team meetings and case conferences through presentation of cases, discussions of clinical issues, and exchange of information and ideas.Other duties and responsibilities may be assigned as required to meet program goals and objectives.Minimum Requirements and Qualification
Master’s degree in social work, Counseling, or Psychology.License or license eligible preferredAbility to work cooperatively on a multidisciplinary team, and to work with clients of diverse ethnic and cultural backgrounds.A minimum of 3 years of case management experience with an adult population psychiatric and substance abuse diagnoses preferred.Understanding of DSM-5 diagnoses, and appropriate treatment interventions plan that includes service linkage, as well as intervention and personal change goalsWorking knowledge of a variety of computer software applications (i.e., Microsoft word, Excel, Access, PowerPoint, Adobe Acrobat)Strong organizational and time management skills.Strong communication (both verbal and written) and interpersonal skills.Ability to maintain proper documentation as determined by programmatic requirements.Ability to prioritize projects and follow through.Additional Information:

Program Description
Felton Institute has been providing comprehensive early care and education services in San Francisco since 1971. Felton now has ECE programs in SF’s Mission, Bayview and Visitation Valley neighborhoods; and offers a school-readiness program for infants, toddlers and preschoolers (2 weeks to 6 years old). Felton’s ECE programs include nine (9) Infant / Toddler classrooms and ten (10) preschool classrooms, which serve approximately 350 children, full day/ year round. These multicultural, educational programs provide a nurturing, developmentally challenging, and inclusive environment for very young children of low/moderate income families. Job Description
Responsible for the assessment, teaching and implementation of identified developmental goals for children with special needs; in collaboration with other classroom teachers implements individual and group activity plans for all children as needed; Makes recommendations to Head Teacher regarding to curriculum planning, modifications in order to meet the needs of individual children, writes and provides anecdotal and progress notes; attends meetings; record keeping; work with parents.

Specific Responsibilities

Keeps all children within vision in order to be accountable for safety. Follows protocols and procedures at all times on a consistent basis.Center assistant duties such as meal preparation, food distribution, inventory management, and restocking of supplies as needed. The Children, Youth, Family, and Transitional Age Youth Services Division (CYFTAY) offers an array of services to low-income and underserved clients with multiple challenges and abilities: Infants, toddlers, and preschool children, including those with special needs. Youth: including transitional age youth, pregnant| parenting teens and youth in the foster care system. Families: including low-income, immigrant, monolingual, and homeless populations. ECE Program(s) Description
Felton Institute has been providing comprehensive early care and education services in San Francisco since 1971. Felton’s ECE programs include 4 infant, 7 toddler, and 11 preschool classrooms, which serve approximately 400 children, full-day/ year-round. These multicultural, educational classrooms provide a nurturing, developmentally challenging, and inclusive environment for very young children of low/moderate-income families. We are committed to inclusive, positive environments that build on children’s positive self-esteem and self-image. Our newest ECE program and the first co-located on-site with affordable housing, the Solmar Learning Center is located in the Mission neighborhood and serves 42 preschool children ages birth-6 years old. The program is open year-round, Monday-Friday 7:30 am-5:30 pm.
Job Description
The Program Supervisor is responsible for overseeing the operations of the Program including planning, organizing, coordinating, and directing the daily operations of the Felton Learning Center Areas of responsibilities include, but are not limited to: staff developing, supervision, and evaluation of the performance of Head Teachers; Supporting the training of all classroom staff; supporting program administration, including strategic planning, visioning, and program compliance. In addition, responsibilities include demonstration teaching, providing consultation, team building, problem-solving, collaborative instructional support, positive and nonevaluative feedback, preparing and planning training. The Program Supervisor is responsible for coaching and supervising the Headteachers, in two (2) main areas: (1) Best practices in Professional ethics, teacher/child, parent/teacher, teacher/teacher relationships, supervision, developmentally appropriate activities, curriculum implementation, observation and evaluation of children. (2) Inclusion of children with special needs in areas of Curriculum, planned activities, individual needs, and identification of unmet needs for the child.

Lastly, the Program Supervisor will be tasked with leading the implementation of Felton’s continuity of care model, dual language programming, and the intentional implementation of Learning Stories across the site. He/she must be a self-starter; have strong organizational skills, and a high level of independent judgment required in decision making and duty performance. The ideal candidate will be comfortable collaborating with individuals, families, and Community Based Organizations to implement and successfully manage the site. He/she meets with supervisors and/or directors regularly, and performs other related duties as assigned.

Job Description
Bridge Housing is intended to address the immediate shelter, and treatment needs of people who have serious behavioral health conditions and are experiencing homelessness. The site management and support services team are responsible in maintaining a 80-bed transitional recovery and community re-entry housing facility for those suffering from mental illness and/or substance use disorder who are experiencing homelessness. The target population often suffers from complex and co-occurring medical conditions and other characteristics that can challenge their ability to navigate mental health, physical health, and substance use treatment in the Santa Rosa community. Services include 24/7 medical and clinical staffing with a department-assigned Housing Coordinator who will provide “gatekeeping” to assure appropriateness of all referrals. Referrals must be clients with a mental illness and/or co-occurring substance use disorder and are either precariously housed or are unsheltered.

Division and Program Description
Felton Institute’s Early Psychosis award winning program (re)MIND® (formerly PREP) was originally developed in partnership with the University of California San Francisco (UCSF). Felton’s (re)MIND® is an early psychosis coordinated specialty care model for early detection and treatment of schizophrenia spectrum disorders. (re)MIND® core evidence-based practices include Cognitive Behavioral Therapy for Psychosis (CBTp), Strength-Based Care Management, Multi-family Group Therapy, Peer Support, Algorithm-Based Medication Management and Individual Placement and Support (IPS) model of supported education and employment. The (re)MIND® model has two additional components, Felton BEAM and BEAM UP, for individuals diagnosed with bipolar or mood disorders with psychosis and for individuals at clinical high risk of developing psychosis, respectively. Funded through partnerships with county behavioral health departments, the Felton Early Psychosis Division operates outpatient coordinated specialty care for early psychosis programs in San Francisco, Alameda, San Mateo, and Marin, and offers excellent training and workforce development.
Job Description
The Clinical Care Manager I works as part of a multidisciplinary team and is responsible for coordinating participant care within the team and with community partners. This includes evaluating the strengths and needs of participants; identifying treatment goals collaboratively with participants and team members; and providing therapeutic interventions that target psychosis, mood, and additional issues that may impact recovery (such as trauma, substance use, interpersonal problems, and environmental stressors, etc.). The Clinical Care Manager I is expected to provide services to participants in both the office and in the community (homes, schools, libraries, etc.). The Clinical Care Manager I reports directly to the (re)MIND® Program Manager.
Specific Responsibilities
Meet regularly with assigned participants and their families in the community and at the office.Conduct assessment interviews and diagnostic evaluations with participants and parents to develop care plans.Provide evidence-based treatment intervention, including Family Psycho-Education, Multi-Family Groups, Case Management, CBT, DBT, MI, and Crisis Intervention.Assist participants with accessing needed services that may include medical, benefits, employment, housing, etc...Collaborate closely with community providers; make referrals to other community and mental health agencies.Maintain clinical records within 24 hours of each participant contact or service, in accordance with mandated quality assurance standards for MediCal and Medicare billing as well as in compliance with County Behavioral Health documentation standards.Meet all deadlines and comply with agency's documentation requirements and productivity standards.Work as part of a multidisciplinary team. Under the supervision of the Operations Supervisor, the Administrative Assistant serves as the primary front desk support for the CYFTAY Division. This entry-level role is responsible for providing excellent customer service to families, staff, visitors, and vendors by maintaining consistent front desk coverage and supporting day-to-day office operations.

Job Description
The Speech Language Pathologist is a member of Felton’s interdisciplinary Early Intervention Team. Under direct supervision of the Early Intervention and Inclusion Director, the Speech—Language Pathologist is responsible for supporting the speech, language, and communication needs of both children enrolled in the Early Intervention Programs at Felton, and non-enrolled children referred by Golden Gate Regional Center for community-based services. The SLP serves children through both GGRC and SFUSD contract agreement, and is responsible for understanding and abiding by the requirements of both of these funders. Felton’s Children, Youth, Family, and Transitional Age Youth division offers comprehensive, early care and education, family support, and mental health services across San Francisco to over 400 families.
Via a two-generation approach, Felton’s Family Support services promote strong families and optimal development for their children by focusing on the Strengthening Families Framework. Felton Institute is seeking a passionate and experienced Early Interventionist to join our team. In this role, you will lead efforts to enhance Felton’s coordinated and integrated system of care for children with special needs aged 0-5. This role will lead the implementation of developmental playgroup services for children, primarily aged birth to three, who demonstrate developmental concerns but do not qualify for formal early intervention programs. This role will focus on creating inclusive and culturally responsive play environments that support children's developmental milestones and promote learning gains in a joyful and creative atmosphere.
Brief Overview:
The Early Interventionist will lead developmental playgroup services for children who may be experiencing developmental concerns or delays, particularly those from low-income communities of color. These playgroups aim to provide accessible, culturally responsive, and developmentally appropriate play opportunities where diverse identities can be explored and reflected. They are intended for children at risk for developmental issues or exhibiting mild to moderate delays, who may not meet the requirements for formal early intervention programs. The playgroups will focus on building developmental skills in communication, fine motor, adaptive, gross motor, and problem-solving areas for children from birth to 5 years old. Applicants should consider how parents will be engaged and involved in supporting their children's development both during play sessions and at home. Salary Range: $32-$34 per hour (DOE)
