Job Summary We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. The LPN will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standards of care in our long-term care and rehabilitation facility. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician. Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed. Collaborate with the rehabilitation team to support residents’ recovery goals, including physical, occupational, and speech therapy plans. Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations. Provide emotional support and education to residents and their families. Ensure a safe and clean environment, adhering to infection control protocols. Qualifications Current and valid LPN license Strong clinical skills, including medication administration and wound care. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Compassionate, patient-focused approach to care. BLS/CPR certification required.     RN Shift Supervisor General Purpose To plan, organize, develop and direct the overall operation of the Nursing Services Department on a particular shift in accordance with current federal, state and local standards governing the facility, and as may be directed by the Director of Nursing, to ensure that the highest practicable level of quality care is maintained. Qualifications Must be a Registered Nurse in good standing and meet all applicable Federal and State licensure requirements. At least 3 years of Nurse Management/Supervision experience required Must have previous experience in a hospital, long-term care facility or other healthcare related facility in a position of responsibility. Must be knowledgeable in general nursing standards of care and medical practices. Must possess the ability to communicate effectively and deal tactfully with personnel, residents, family members, visitors, government agencies / personnel, and the general public Must possess the ability to make independent decisions when circumstances warrant such action Excellent nursing assessment, documentation, and written/verbal communication skills. Computer literacy and knowledge of electronic documentation. Display professionalism both in appearance and in actions. Must possess strong leadership and interpersonal skills. Essential Job Functions (not all inclusive) Assist Director of Nursing (DON) in organizing, implementing, evaluating and directing the programs and activities of the nursing departments. Assist in developing and implementing methods for coordinating nursing services with other resident services, such as activity programs. Implement plans of action to correct deficiencies. Complete medical forms, reports, evaluations, studies, charts, etc. Supervise resident’s care, conduct compliance rounds throughout the day to assure quality of care and to address problems, as necessary. Make appropriate documentation. Conduct tour and obtain essential information for possible admission. Inform staff of new admissions, ensure that rooms are ready; greet new admissions and escort to rooms. Ensure that any equipment, medication, treatment and other supplies are available. Supervise Nursing Assistants, Charge Nurses, LPNs and Med Tech, if applicable. Assist in counseling, evaluating, and terminating nursing services personnel with guidance from Administrator and DON. Ensure schedules and work assignments, including vacation and holiday schedules, are implemented to ensure adequate staffing, including replacing call-ins. We are in search of a qualified Assistant Director of Nursing- ADON : Assists the Director of Nursing with management of the nursing department. Assists Director of Nursing in ensuring compliance with regulations for documentation from physicians and nursing personnel and ensuring that all regional policies/procedures are implemented. Make rounds daily to direct the work of employees and to observe for hazards, work duty problems, and ensure services are delivered to clients consistent with needs. Assists with Continuous Quality Improvement and corrective follow-up actions. Follows up daily on each unit to ensure changes in status are reported and recorded and that services are provided. Under the direction of the Director of Nursing, completes chart audits and reports results and progress, and makes recommendations for improvement to the Director of Nursing. Ensures proper infection control techniques are utilized. Maintains effective communication with families, residents and facility staff. Qualifications: Current R.N. license by State required with experience in the long term care or working with the geriatric population preferred. Must have at least three years nursing experience and one year supervisory experience. Clerical ability is necessary to read reports and utilize data accurately for other purposes. Skill in organizing and planning programs and managing personnel to provide nursing service for residents. Ability to plan and direct the department, coordinating with other departments.
