    Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems.     Essential Duties and Responsibilities:

    • Check in patients that are presenting for Methadone dosing. Review file for all HOLDS and instruct patient to follow through with drug screening, treatment plans and any other clinical requests.
    • Answer incoming calls, greet callers and visitors to the facility, provide information, transfer calls or take messages as necessary.
    • Answer simple questions about company services.
    • Operate communication systems, such as telephone, intercom, two-way radio, or public address.
    • Page individuals to inform them of telephone calls, using paging or interoffice communication.
    • Greet visitors courteously; determine their needs; log visits on visitor log sheet; provide visitor badges; and direct visitor to proper person/office.
    • Check in patients and visitors; make copies of required documents that patients bring in for clinical review.
    • Relay or route written or verbal messages.
    • Route emergency calls appropriately.
    • Notify appropriate staff of incoming deliveries or presence of repair or maintenance personnel.
    • Operate office machines and copy equipment.
    • Assist with toxicology collection if needed.

     



    Education and/or Experience: High School diploma or GED equivalent with at least 2 years of administrative/office manager experience. Experience in customer service strongly preferred. Basic knowledge of clerical practices and procedures, data and administrative management practices and procedures. Computer skills and knowledge of office software packages.

    General Duties:

    Mental Health Assessment

    Prompt scheduling and completion of an initial Mental Health Assessment for all mental health clients. This Mental Health Assessment shall be reviewed and approved by an LPHA.Completion of The Level Of Care Utilization System (LOCUS) functional assessment for all mental health clients 18 years of age and older.Documentation of an Admission Note summarizing the clients' identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, the medical necessity of treatment and treatment plan recommendations.Re-assessment per Medicaid Title 132 guidelines for ongoing patients.

    Treatment Planning

    Construction and documentation of a timely individualized treatment plan in conjunction with the client, and their guardian, in the event they have a court-appointed guardian or are a minor, addressing the clinical needs identified in the assessment process, coordination of care and discharge planning needs. This TP shall be reviewed and approved by an LPHA.Timely revision/modification of the TP as the client moves through the treatment process.

    Mental Health Therapy and Case Management

    Provision of individual, group, couples/family counseling as driven by the needs identified in the assessment process and as indicated in the individualized treatment plan.Timely documentation of therapy in progress notes summarizing for each session; goals and objectives addressed, clinical interventions, client response to interventions, and continued therapy plan.Provision of mental health case management to facilitate linkage to and ongoing coordination and support for needed services and supports in patients' community environment.Crisis intervention when indicated including immediate coordination of indicated service given nature of crisis addressed.

    Essential Duties:

    · Assist in coordinating agency wide gambling outreach services and activities ensuring alignment with the Illinois sponsored “are you really willing” campaign. This includes ensuring gambling outreach events are scheduled and staffed for the service areas of Cook County, Sangamon County and Will County.

    · Provide problem gambling education, screenings and referrals throughout the community served (Cook County) including but not limited to activations arranged as part of the Illinois sponsored “are you really winning” campaign.

    · Provide “brief intervention” to individuals identified to have concerns regarding gambling whether the individual accepts a treatment referral or not.

    · Provide problem gambling education, screening, brief intervention and referral to Family Guidance Centers' patients as appropriate.

    · Distribute gambling education materials to organizations in the community, for example, behavioral health organizations, social service organizations, establishments that have gambling such as video gaming terminals etc.

    · Conduct direct outreach to individuals experiencing substance use, homelessness, or other barriers to care by providing harm reduction education, distributing supplies, and describing FGC programs in accessible language to increase awareness of available services.

     

     

    Work Schedule: Monday - Friday (8:00a - 4:30p); with flexibility to support program needs

    Start Date: July 1st, 2026

    Work Location: Cook County

    Pay Rate: $22.00 per hour

    Education and/or Experience: High School Diploma or equivalent. Associate's degree preferred. Experience in behavioral health, community health outreach, substance use harm reduction, withdrawal management programs, medication-assisted treatment, and/or community relations required. Lived experience with substance use and/or problem gambling and successful recovery. Certification as a Community Health Worker or Peer Support Specialist preferred.
