The Human Resources Assistant is a part-time, non-benefit-eligible support position (up to 20 hours per week) responsible for providing clerical and administrative support to the Human Resources department. The position is based primarily at the Hunting Park site with one weekly visit to the Kensington-Allegheny (KA) site. Core responsibilities include managing HR documentation, maintaining employee records, supporting compliance activities, and assisting with employee engagement initiatives. The position reports directly to the Director of Human Resources and is expected to independently manage daily tasks in a timely and organized manner.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Compliance-Critical Administrative Responsibilities
Assist with onboarding documentation, including I-9 completion and verification, W-4 and state tax forms, and required clearances; ensure personnel files remain complete, current, and compliant.Compile and submit monthly HR compliance tracking reports, including visa/work authorization status and employee attendance reporting.Track employee attendance at mandatory trainings and events for compliance reporting purposes; flag gaps to the HR Director.Create and maintain new hire folders; update and organize existing employee files as directed; maintain strict confidentiality of all records at all times.Support HR compliance activities related to HRSA requirements, UDS processes, and FQHC regulatory standards as directed.Sign and uphold EHC's HIPAA/Privacy policies; protect the confidentiality of all employee records, including medical, immigration, disciplinary, and compensation information, in full compliance with applicable law and EHC policy.General Administrative & Operational Support

Read LessThe Patient Satisfaction Survey Administrator will work with Esperanza Health Center under a 6 to 7-week contract for Summer/Fall 2026, exact dates yet to be determined. As the administrator, you will administer an approximately 30-question survey to patients in waiting rooms at all of Esperanza’s sites to assess patients’ experience and perception of Esperanza Health Center’s medical services, in an effort to improve access to care and quality of care.
Project Responsibilities:
• Distribute and collect paper surveys in the waiting rooms. Our target is to collect at least ten surveys for each clinician who has a patient panel and a total of about 400 surveys. The number of surveys at each site should be roughly proportional to the number of clinical hubs at that site.
• Assist patients in completing the survey by answering questions, explaining any confusing language, and, when necessary, reading survey questions to a patient and recording their answers.
• Develop a system for keeping track of survey progress by a clinician and by site, and utilize the schedule to plan which site and times to work in order to reach the goal of ten surveys per clinician.
• Enter survey data into EpiInfo, either on an ongoing basis or after the surveys are completed.
• Transfer tabulated data into an Excel spreadsheet and update graphs.
Analyze data, observe trends, and refine survey administration processes for future years.Qualifications:

Strategic Leadership

POSITION SUMMARY: As a Care Team RN, this individual provides direct patient care in a care team through participating with the clinician in visits (especially acute co-visits, new patient exams and hospital follow-up visits), performing RN procedures, addressing needs of walk-in and call-in patients, providing patient education, integrating spiritual care, and coordinating care for complex patients on the care team.

JOB SUMMARY
The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health
professionals to provide comprehensive case management services to patients seeking support with substance
use. Services include referral and linkage, individualized care management, and support for adherence to
available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use
Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma-
informed approaches.

WHAT YOU WILL BE DOING
• Receive and manage patient referrals; collaborate with departmental staff to facilitate and expedite initial
screening and intake.
• Provide ongoing support and care management to individuals engaged in substance use, utilizing a
culturally sensitive, strength-based, trauma-informed approach to build and maintain a therapeutic
alliance.
• Collaborate with patients and care teams to develop individualized care management plans that address
identified needs and support progress toward patient-identified goals.
• Identify and connect patients to culturally appropriate resources across EHC and the broader community;
provide referrals to outside agencies as appropriate.
• Monitor and evaluate each patient's recovery progression; communicate progress to the care team
regularly and flag concerns as they arise.
• Address and amend care plans as needed in collaboration with patients and care teams, utilizing clinical
assessment skills to match patients with the appropriate level of care, including referrals to higher levels
of behavioral health care when indicated.
• Provide crisis intervention, crisis management, and safety planning as appropriate.
• Document all patient progress and services provided accurately and in a timely manner in the Electronic
Medical Record (EMR).
PHYSICAL DEMANDS AND WORK ENVIRONMENT
The Intensive Care Manager position involves regular in-person patient contact in a clinical outpatient setting. The
following physical demands are representative of those required to perform the essential functions of this role:

This position is a member of the primary care provider team providing brief, immediate behavioral health support and interventions as part of integrated primary care services and makes referrals to outside agencies when appropriate for optimum patient care. This position works normal business hours in the clinic on Monday-Friday.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Clinical Supervision:
Directly supervises team of BHCs at all 3 sites as clinically appropriate .Responsible for evaluating performance of BHCs.
Collaborates with HR and the department director to oversee recruiting, hiring and training of new BHCs
Directs the Coordinator to approve scheduling, paid time off, and time reporting issues.
Provides oversight of clinical issues related to BH services that are provided by BHCs in the context of primary care including clinical quality management, tracking and strategizing related to grant deliverables, clinical and ethical standards within the BH disciplines.
Participates in strategic planning and vision casting for the Behavioral Health Department and in collaboration with the Behavioral Health Director.
Facilitate department meetings and risk management trainings
Participate in external meetings with the Health Federation and other referral partners as needed.
Provides clinical supervision( individual and group) hours for pre-licensed behavioral health associates.
2. Behavioral Health Consultation(Direct Service Provision):
Works as part of the primary health care team, providing quick and accurate clinical assessments for patients in the exam room and patient care hubs using evidence-based treatment modalities.
Demonstrates appropriate clinical judgment in assessment and treatment, following established clinical care guidelines; provides appropriate referrals to outside agencies as indicated.
Designs culturally appropriate behavioral health interventions using evidence-based treatment modalities.
Appropriately documents each patient visit in EMR, including subjective and objective data, assessments, treatment plan and other documentation as necessary. Maintains compliance with all agency policies and procedures.
Effectively communicates with and educates patients regarding self-care; supports and facilitates patient goal-setting.
Responds quickly to urgent/emergent needs.
Provides holistic care by integrating faith and practice into patient care; provides effective spiritual intervention and referral to staff chaplains as indicated.
3. Other Responsibilities:
Meets with Director of Behavioral Health for supervision and case review.
Identifies and integrates community resources into practice, working collaboratively with community behavioral health agencies and others to optimize patient care.
Continues to expand personal and professional knowledge by attending area trainings, peer conferences and events related to duties as BHC.
Maintains licensure requirements.
Participates in departmental meetings and clinic meetings as required.

EDUCATION, SKILL, KNOWLEDGE AND QUALIFICATIONS
· Licensed in the Commonwealth of Pennsylvania as an LCSW, LPC, LMFT
· Strong oral communication skills
· Bilingual in Spanish and English required
· Demonstrated knowledge of trauma-informed care and providing culturally competent care.
· Experience providing clinical supervision, preferably in an integrated health setting.
· Knowledge and experience in treating co-morbid disorders
