    • Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
    • Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
    • Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic CM
    • Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
    • Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.
    • Maintain liaison with appropriate community agencies and organizations.
    • Accurately collect and document patient care data.
    • Develop treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care and achieve optimal wellness.
    • Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings. Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
    Additional Duties.Utilize available automated programs and information technology, communication, and management tools for proactive patient management and to facilitate patient engagement and enhance patient experience (i.e., MHS Genesis, TSWF, CarePoint and Patient Portal Secure Messaging).Communicate with patients utilizing asynchronous Secure Messaging (i.e., MHS Genesis Patient Portal) to improve communication and facilitate care through non- traditional means.
    Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care. Assist in coordinating a multidisciplinary team to meet the health care needs, including medical and/or psychosocial management, of specified patients. 5.1.6.2. Serve as consultant to all disciplines regarding CM issues. Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM; develop and implement policies and protocols for home health assessments and outcome measures.Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, databases for community resources, etc.Integrate CM and utilization management (UM) and integrate nursing case management with social work case management. Prepare routine reports and conduct analyses.Assist in establishing and maintaining liaison with appropriate community agencies and organizations, the TRICARE Lead Agent office, and the Managed Care Support Contractor.Maintain adherence to JCAHO, URAC, CMSA and other regulatory requirements. Apply medical care criteria (e.g., InterQual).Ensure accurate collection and input of patient care data and ensure basic CM budgetary management.Provide input on MTF CM resources and make recommendations to the Command as to how those resources can best be utilized.Collaborate with the multidisciplinary team members to set patient-specific goals. Develop treatment plans including preventive, therapeutic, rehabilitative,psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.Establish and implement mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.Provide nursing advice and consultation in person and via telephone.Ensure appropriate health care instruction to patient and/or caregivers based on identified learning needs.Alert physicians to significant changes or abnormalities in patients and provide information concerning their relevant condition, medical history and specialized treatment plan or protocol.Facilitate multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources.Develop and implement mechanisms to evaluate the patient, family and provider satisfaction and use of resources and services in a quality-conscious, cost- effective manner.Implement strategies to ensure a smooth transition and continued health care treatment for patients when the military member transfers out of the area. Develop a policy for, and assist with, region-to-region transfers.Facilitate screening and assist with transfers of Exceptional Family Member Program (EFMP) families.Plan for professional growth and development as related to the case manager position and maintenance of CM certification.Establish cost containment/cost avoidance strategies for case management and develop mechanisms to measure its cost-effectiveness.Assist with the CM interface or other database designed to support CM.Participate in video teleconferences (VTCs) and other meetings as required.Minimum Qualifications:Degree: Possess a Bachelor’s degree of Nursing from an accredited university. Core duties include, but are not limited to the following:Provides professional nursing care to patients undergoing surgery. Prepares and maintains operating room for all surgical procedures. Establishes and utilizes a checklist to monitor the physical plan of the operating room and equipment for conformity to all national standards for ensuring safety for the patient and all personnel. Assembles the type and quantity of material and special equipment needed for daily procedures and possible emergencies. Circulates for surgical procedures. Responds to surgical emergency requirements such as cardiac, pulmonary, or respiratory failure and hemorrhage.Maintains open communication lines, both interdepartmentally and interdepartmentally, to provide for total needs of the patient. Conducts preoperative and postoperative visits. Discusses operative procedures with patients and informs them of what to expect during surgery. Consults with the operating surgeon and establishes priorities of care for each patient entering the operating suite. Coordinates with other hospital units, which include recovery room, intensive care, obstetrics, and nursing units, on patients, progress and any special equipment or supplies that will be needed for the patient returning from surgery. • Directs and instructs personnel in preparing, sterilizing, and caring for operating room equipment and supplies. Maintains concise and complete records and reports. Coordinates an ongoing, in-service education program to continually update knowledge and skills of all assigned operating room nursing personnel.Initiate, perform and complete assigned duties in providing care to variable patient populations based upon the assessed needs of the patient and in relation to the proposed procedure.Complete assigned patient care based on the patients’ conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report and record relevant patient information.Observe, identify and respond to the patient’s needs for care, including medication, equipment-assisted care and patient/family education.Provides perioperative nursing care, for all types of patients admitted to the surgical suite for surgical intervention. The type of work may include, but not limited to, the following:Participates in and successfully completes two phases of Air Force IBHC training. Demonstrates core competency skills to IBHC independent practice within 90 days of starting. The 90-day threshold for demonstration of core competency skills will be extended if the Air Force is unable to provide training opportunities within 90 days.Phase I training is approximately four days onsite at a CONUS location. The candidate shall have a 100% pass rate (minimum of a "3" rating in each competency area of the clinical competency tool) before proceeding to Phase II.Phase II training will occur within 90 days of completion of Phase I training for a two day course. The candidate shall have a 100% pass rate (minimum of a "3" rating in each competency area of the clinical competency tool) in order to be deemed acceptable for the position.Provides brief (typically no more than four appointments) cognitive/behavioral interventions in primary care under the Air Force consultation model (i.e., per the Air Force Behavioral Health Optimization Program Practice Manual). Services will mimic the pace of primary care (i.e., patients seen for 15- to 30-minute appointments).May provide classes in format appropriate for primary care (e.g., stress management class, sleep enhancement class, chronic pain class).Works independently in the absence of direct oversight and adheres to Air Force values. Effectively uses appropriate communication format in addressing and resolving professional issues.Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.Must possess organizational, problem-solving, and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.Successfully consults with PCMs on-demand about general issues and specific patients.Ensures PCMs receive same-day verbal and/or written feedback on patients referred.Delivers brief presentations/trainings to primary care team members regarding behavioral health issues and IBHC services.Participates in team efforts to develop, implement, evaluate, and revise clinical pathway programs needed in the clinic.Provide concise documentation of care and recommendations in the patient’s electronic medical record within the allowable timeframe defined by the MTF standard after the patient is seen.Effectively manage a full daily patient load, as defined by the Mental Health Business Practice Standards and BHOP Manual of completing approximately 8-14 patient encounters per dayDuties may include supervision of providers who are in training or not fully licensed.Duties may include participating in Air Force-approved research projects relating to Primary Care Behavioral Health.Consults with medical personnel, legal authorities, and military commanders as required.Participates in military-specific training (e.g., use of the Air Force Guide for Suicide Risk Assessment, Management, and Treatment, pre-post deployment screening, PTSD, and combat stress, etc).Attends and participates in meetings during normal duty hours and professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, primary care staff meetings, and others required by applicable regulations, MTF guidance, or as directed by supervisor.Care Facilitator (CF) in a Patient Centered Medical Home (PCMH). The type of work may include, but is not limited to, the following:IBHCs who are assigned to MTFs that have between 3,000 and 7,500 adult enrollees in the Family Health Clinic will provide both IBHC and CF services per the BHOP Manual.Participates in and successfully completes Air Force care facilitator training. Demonstrates core competency skills to CF independent practice within 240 days of starting. The 240-day threshold for demonstration of core competency skills will be extended if the Air Force is unable to provide training opportunities within 240 days.Care facilitator training is an interactive computer-based training that is approximately three days in length. The candidate must complete all CF training requirements before engaging in CF patient care services.Contact referred patients to assess medication treatment adherence for depression, anxiety, and PTSD disorders.Use validated brief symptom inventories to assess current symptom severity.Assess barriers to treatment adherence and help patient problem-solve solutions to barriers.Provide patients with education regarding their behavioral health condition and treatment regimen.Maintain patient information in a patient registry.Maintain accurate and current notes in the electronic medical records of all patients contacted for review by the managing physician.Performs case management functions.Attends and participates in meetings during normal duty hours and professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meeting, professional staff meetings, commander's staff meetings, primary care staff meetings, and others required by applicable regulations, MTF guidance, or as directed by supervisor.Minimum Qualifications:Education: MSW Degree accredited by the CSWE.Experience: 1) Must have one year experience in the last three years in the evaluation and treatment of patients who have pain, psychological factors affecting their physical condition, or difficulty coping with medical problems. 2) Must have at least 12 months of direct hands-on experience in the last 24 months with computer operations, basic word processing, data entry, and use of an automated medical record system.Licensure; Current, full, active, and unrestricted LCSW license from any state.Life Support Certification: Possess and maintain a current AHA or ARC BLS Healthcare Provider certification.
