Duties
The caregiver (CNA) will be responsible for:
Care for census of 5-6 patients Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Provides assistance in toileting, positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen and patient's laundry.Feeds and assists with meals.Reports any changes in resident's/patient's condition to the NurseAbout You

Job Summary:
Ennoble Care is hiring for a full-time Hospice RN Case Manager to visit patients in the Berks / Lebanon County, PA region. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Responsibilities:
Manage your assigned patient caseload within your territory and be responsible for the following tasks, including but not limited to: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process.Document all notes in the EMR and submit them accurately and in a timely mannerCommunicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for all patientsEducate and coach all relevant parties on the best practices regarding call levels of care for the patient including: physical, mental, emotional, etcPartner with clinical leadership to manage/address any issues, share profesisonal support, and exchange feedback to enhance professional growth. Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation Supervise and document the NAC every two weeks or as assigned Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during off hours time or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nursesProvide appropriate support at time of death and period of bereavement for family members and loved onesParticipate in the Hospice orientation, in-service training, and community programs as related to hospice care Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice Accept other assignments as appropriateQualifications:

Ennoble Care is looking for a full-time, experienced Care Coordinator, Embedded that will work out of our client's senior living facility in the Salina/McPherson, KS area, who aligns with our motto, "To Care Is An Honor". This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Care Coordinator, you should be compassionate, experienced, and highly organized. Ennoble Care is seeking a full-time Care Coordinator to join our team in an office-based setting in Wichita, Kansas. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice–style model of care.

Purpose:
We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Philadelphia, PA region on a full-time or part-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.
What You'll Do:
Provide high quality care to primary and palliative patients in and around Philadelphia, PA.Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.Effectively communicate with office staff and clinical operations across various internal platformsProvide high quality, patient centered care to a panel of patientsDevelop impressions, diagnosis, and treatment plansFoster trust and consult with patients, families, and caregivers during visitsOrder diagnostic testing as medically necessaryPrescribe medications supplies and appropriate medically necessary interventionsWhat You'll Need:
Board Certified, with a valid NP or PA certification in the state of PAStrong clinical skills with the ability to accurately and confidently treat, assess, and diagnosePositive demeanor and strong people skillsAbility to confidently problem solve and operate independently1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)What you'll get:

Ennoble Care is seeking a Full-time, experienced Community Liaison for our Washington DC/Prince George's County, Maryland area's! The Community Liaison will be responsible for interaction in the community and promoting company services, such as our Housecall services.
Analyze the potential of the company's service area to determine target markets.Visit Doctor' offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities and other possible sources of referrals to present Agency credentials and obtain patient referrals.Analyze the company's organization to determine its strengths and weaknesses.Analyze past and current marketing data.Complete an analysis of the company's "product" line.Analyze patient/company relationships.Develop sales/marketing objectives and sales projections.Develop a marketing plan, identifying priorities and sets a reasonable timetable.Implement marketing plan staying within established timetable.Review and evaluate the analyses and plan on an established basis.Attend IDG meetings, as appropriate.Assist office staff as needed with going to updates, etc.Provide community outreach and education.Job Qualifications
At least 3 years experience in marketing, nursing or social work interacting with health agencies/professionals.Must be organized and detail oriented.Must be able to communicate effectively orally and in writing.Must be a licensed driver with an insured automobile in good working order.Base Salary: $50,000 - $95,000, Plus: Monthly, Quarterly, and Annual Bonuses
