    Heritage Wound Management Specialists (HWMS) is a mobile, home-based wound care practice dedicated to serving patients who face barriers to traditional clinic access. We partner with hospitals, home health agencies, skilled nursing facilities, and primary care providers to deliver advanced wound care directly in the home and post-acute setting.


    POSITION SUMMARY

    We are seeking an experienced Wound Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide advanced wound care services in patient homes and facilities throughout Eastern North Carolina. This role offers clinical autonomy, strong leadership support, and the opportunity to grow with a rapidly expanding organization.


    KEY RESPONSIBILITIES

    • Perform comprehensive wound assessments 

    • Develop individualized treatment plans 

    • Manage acute and chronic wounds 

    • Perform sharp and enzymatic debridement (within scope) 

    • Apply skin substitutes and biologics 

    • Manage NPWT (wound vacs) 

    • Implement compression therapy 

    • Order and interpret labs and cultures 

    • Prescribe medications and wound care supplies 

    • Maintain compliant medical documentation 

    • Collaborate with home health agencies, SNFs, and referring providers 

    • Educate patients and caregivers 


    QUALIFICATIONS

    Required:

    • Active North Carolina license 

    • Board Certification 

    • DEA (or ability to obtain) 

    • BLS certification 

    • Minimum 2 years experience 

    • Reliable transportation 


    Preferred:

    • Wound care certification (CWOCN, WCC, CWON) 

    • Experience with skin substitutes and NPWT 

    • Home health or post-acute experience 

    • Knowledge of Medicare documentation standards
