    Elwyn seeks a Behavioral Health Technician (BHT) to provide one-on-one interventions including but not limited to crisis intervention, intensive behavior management support, emotional support, time-structured activities, and psychological rehabilitative activities to children/adolescents who manifest a wide range of disabilities and functioning levels at homes, schools, day care, and/or community by implementing applied behavior analytic interventions and support. The provision of these services allows the child to remain in the least restrictive treatment or education setting.DUTIES AND RESPONSIBILITIESCarries out specific interventions that are identified in the behavioral treatment plan developed by the Behavioral Consultant (BC), the Interagency Team and the Elwyn treatment team

    Responsibilities

    Elwyn seeks a Mobile Crisis Specialist (MCS), acting as a member of the Mobile Assessment Team, to respond to mental health crises upon dispatch from approved referral sources. The MCS works with individuals and their community supports to assess and de-escalate a crisis, determine the level of care necessary to meet the individual's needs, develop alternatives to involuntary hospitalization through conducting assessments, treatment planning, counseling, and coordination of community care linkages and discharge process. The MCS coordinates the involuntary hospitalization process in situations where individuals present as dangerous to themselves or others. The MCS acts as an advocate and represents the needs and rights of persons served with outside social service agencies, families, etc.

    DUTIES AND RESPONSIBILITIES:

    Elwyn seeks a Lead Recovery Worker provides psychiatric rehabilitation with a recovery-oriented rehabilitation program model for individuals with serious and persistent mental illness and co-occurring disorders. The Lead Recovery Coach provides supervision to MPRS staff and PRS staff and supports the Program Director with day-to-day oversight of program operations.

    ESSENTIAL FUNCTIONS:

    The Mental Health Case Manager will be at our Young Adult Residential Treatment Facility assisting with obtaining, getting access to, and coordinating health care, continued behavioral health treatment, social services, advocacy, educational programs, recreational programs, housing and community integration resources. The Mental Health Case Manager also evaluates the need for customized services to the individual that will be delivered in the community as well as explores all available resources for the individual.     POSITION SUMMARY: As a member of the Interdisciplinary Care Team, the Nurse Case Manager is responsible for the coordination and implementation of health care services for each individual residing in Elwyn New Jersey's community living program sites and enhanced behavior homes. The nurse provides clinical and consultative services and educates the team about consumer-specific needs. The nurse also liaisons with external providers to facilitate care coordination and ensures that established standards of care are met.

    The nurse practices within the scope of New Jersey Board of Nursing laws and in accordance with Elwyn New Jersey's policies and procedures and those of Elwyn New Jersey's regulatory agencies.

    DUTIES AND RESPONSIBILITIES:

    Review ongoing medical, dental, and psychiatric treatment with the treatment team to ensure recommendations are addressed in a timely mannerAssist in identifying health care needs and provide site staff with individual training that focuses on general health conditions, treatments, and interventions specific to each individual's health needs and consistent with standards of careReview all Medical Appointment Forms, lab data, and diagnostic test results with the Treatment TeamProvide continuity of care through timely follow up with physicians and other health care providers to ensure that agency and licensure requirements are metMonitor individuals' weights and vital signs on a monthly basis and more frequently as needed; identify changes from baseline and report abnormal findings to the Primary Care Physician and Treatment Team.Summarize Medical Appointment Forms, telephone contacts, and ongoing health care follow up in the nursing progress notesMonitor consumer immunization status and ensure that each consumer remains up to date and current (as per NJ State regulatory guidelines)Monitor consumers for potential safety risks and the need for adaptive equipment, monitor all adaptive equipment to ensure that it remains in good working order and facilitate repair/replacement when equipment issues are identifiedComplete a monthly review of medication boxes and Medications Administration Records for each site.
