POSITION SUMMARY: As a member of the Interdisciplinary Care Team, the Nurse Case Manager is responsible for the coordination and implementation of health care services for each individual residing in Elwyn New Jersey's community living program sites and enhanced behavior homes. The nurse provides clinical and consultative services and educates the team about consumer-specific needs. The nurse also liaisons with external providers to facilitate care coordination and ensures that established standards of care are met.
The nurse practices within the scope of New Jersey Board of Nursing laws and in accordance with Elwyn New Jersey's policies and procedures and those of Elwyn New Jersey's regulatory agencies.
DUTIES AND RESPONSIBILITIES:
Review ongoing medical, dental, and psychiatric treatment with the treatment team to ensure recommendations are addressed in a timely mannerAssist in identifying health care needs and provide site staff with individual training that focuses on general health conditions, treatments, and interventions specific to each individual's health needs and consistent with standards of careReview all Medical Appointment Forms, lab data, and diagnostic test results with the Treatment TeamProvide continuity of care through timely follow up with physicians and other health care providers to ensure that agency and licensure requirements are metMonitor individuals' weights and vital signs on a monthly basis and more frequently as needed; identify changes from baseline and report abnormal findings to the Primary Care Physician and Treatment Team.Summarize Medical Appointment Forms, telephone contacts, and ongoing health care follow up in the nursing progress notesMonitor consumer immunization status and ensure that each consumer remains up to date and current (as per NJ State regulatory guidelines)Monitor consumers for potential safety risks and the need for adaptive equipment, monitor all adaptive equipment to ensure that it remains in good working order and facilitate repair/replacement when equipment issues are identifiedComplete a monthly review of medication boxes and Medications Administration Records for each site. Provide a written report summarizing any issues or discrepancies to the Nurse Practitioner, Program Director, Quality Improvement Specialist, and others as designated by the Supervising Nurse PractitionerComplete a general review of each residence on a monthly basis to validate that privacy, safety, infection control and general standards are met. Responsibilities
Elwyn's Nursing services are provided for those individuals who live either in Elwyn's residential programs, including Media Campus, or in Community Residential Services in Delaware, Chester and Philadelphia counties. The nursing team is led by a Health Care Coordinator who works in conjunction with the resident's Primary Care Physician and an interdisciplinary team to ensure that the residents are living a healthy life. If necessary, 24-hour nursing care can be provided, and decreasing levels of service are available to residents who function more independently. Nursing services at Elwyn include:
Medical case managementMedication administrationRoutine and emergency careNursing care planning and assessmentsTreatment as ordered by the Primary Care PhysicianPRIMARY FUNCTION - As a member of interdisciplinary team, the nurse provides normalized, empathetic, therapeutic and preventative nursing care to clients in an assigned interdisciplinary setting at Elwyn within the scope of the Pennsylvania Practical Nurse Law and in accordance with Pennsylvania DPW Department of Health's standards, IOC, and Elwyn's policies and procedures. Performs the following duties which are within your ability, training, license to perform.IMMEDIATE SUPERVISOR - The Licensed Practical Nurse reports clinically to: the Department of Nursing, Health Care Manager; Administratively to Assigned Residential Unit, Residential Unit Manager.

WORKING CONDITIONS - Ability to work in variety of environments and settings, flexible hours. Exposed to body wastes, discharges, odors and noises.
HAZARDS: Occasional exposure to hazards which may cause serious illness or injury (instruments, equipment, body waste, and infectious diseases).

POSITION SUMMARY: The Mental Health Program Lead is responsible for the oversight and training of all assigned shifts within the residential unit.
DUTIES AND RESPONSIBILITIES:

Responsibilities
As a member of the Mobile Assessment Team the Mobile Crisis Specialist (MCS) is responsible for responding to mental health crises upon dispatch from the Delaware County Office of Behavioral Health Dispatch Specialist. The MCS’s function is to work with individuals and their community supports to assess and de-escalate a crisis, determine the level of care necessary to meet the individual’s needs, develop alternatives to involuntary hospitalization through conducting assessments, treatment planning, counseling, and coordination of community care linkages and discharge process. The MCS will also coordinate the involuntary hospitalization process in situations where individuals present as dangerous to themselves or others. The MCS will act as an advocate and represent the needs and rights of persons served with outside social service agencies, families, etc.
• Complete an assessment for adults, adolescents, or children throughout Delaware County wherever the crisis is occurring
• Develop and secure appropriate services for individuals in crisis, including referrals to voluntary inpatient settings and applications for involuntary emergency examination and treatment under §302 of the Mental Health Procedures Act of 1976. Provide linkages to community supports, supportive counseling, and substance abuse services and always reflect the possibility of recovery
• Complete referrals for follow-up services by community mental health and other supports. Assist in completion of application for entitlements. Complete Emergency Care Provider Intake
• Provide all appropriate documentation set forth by Director including intakes, progress notes, medical charting, discharge plans, and individual assessments
• Advocate for individuals served with outside agencies including, but not limited to Social Security, Department of Public Welfare, Delaware County Office of Behavioral Health, Delaware County Base Service Units, etc.
• Interface with families of persons served and other interested parties
• Maintain communication with physician, psychiatrist, pharmacist, and other appropriate ancillary service providers such as speech therapy, physical therapy, and occupational therapy as relevant to providing emergent services
• Intervene in crisis situations
• Attend scheduled staff team meetings and appropriate in service training as directed by supervisor
• Participate as a member of the interdisciplinary team; maintain complete and accurate clinical record
• Conduct fire drills and building inspections and provide the appropriate documentation for same
• Remain on duty until relieved
• Maintain current certification/re-certification of CPR, First Aid and Medication Administration certification
• Provide counseling, guidance, support, resources and encouragement to persons served to take responsibility for and actively participate in their own recovery
• Collaborate with the team to promote a team culture in which each person’s point of view, experiences, and preferences are recognized, understood, and respected, and in which the person’ self-determination and decision making in treatment planning are maximized and supported
• Increase awareness of, and support participation in, self-help programs and advocacy organizations that promote recovery

DUTIES AND RESPONSIBILITIES:

POSITION SUMMARY: As a member of the Interdisciplinary Care Team, the Nurse Case Manager is responsible for the coordination and implementation of health care services for each consumer residing in Elwyn's community residential sites. The nurse provides clinical and consultative services and educates the team about consumer-specific needs. The nurse also liaisons with external providers to facilitate care coordination and ensures that established standards of care are met.
The Registered Nurse practices within the scope of Pennsylvania's Nurse Practice Act and in accordance with Elwyn's policies and procedures and those of Elwyn's regulatory agencies.
DUTIES AND RESPONSIBILITIES:
Review ongoing medical, dental, and psychiatric treatment with the treatment team to ensure recommendations are addressed in a timely mannerAssist in identifying health care needs and provide site staff with consumer-specific training that focuses on general health conditions, treatments, and interventions specific to each individual's health needs and consistent with standards of careReview all Medical Appointment Summaries, lab data, and diagnostic test results with the Treatment TeamProvide continuity of care through timely follow up with physicians and other health care providers to ensure that agency and licensure requirements are metMonitor consumers' weights and vital signs on a monthly basis and more frequently as needed; identify changes from baseline and report abnormal findings to the Primary Care PhysicianSummarize Medical Appointment Summaries, telephone contacts, and ongoing health care follow up in the nursing progress noteComplete Falls Assessment for identified 'at risk clients' as per Elwyn PolicyMonitor consumer immunization status and ensure that each consumer remains up to date and current (as per Pennsylvania (PA) State regulatory guidelines)Monitor consumers for potential safety risks and the need for adaptive equipment, monitor all adaptive equipment to ensure that it remains in good working order and facilitate repair/replacement when equipment issues are identifiedComplete a monthly review of medication boxes and Medications Administration Records for each site in collaboration with the Program Staff to ensure accuracy/no expirations of medications; provide a written report summarizing any issues or discrepancies to the Director of Operations of CRS, Program Director, Quality Improvement Specialist, and Senior (Sr.) Director of Community Nursing for PA and New Jersey (NJ)Complete a comprehensive on site physical assessment of each residence on a monthly basis to validate that privacy, safety, infection control and general standards are met; document results of the assessment in the electronic health record, and provide a written report summarizing any issues or discrepancies to the Director of Operations of CRS, Program Director, Quality Improvement Specialist, and Sr. Director of Community Nursing for PA and NJProvide coordination of clinical services between Elwyn's CRS Program and hospitals, physicians, pharmacies, and other community-based providers and services to ensure quality of care, confirm coordination of resources/services by email with providersWhen a consumer is admitted into a hospital, skilled nursing facility, or rehabilitation center:Collaborate with the facility's staff to ensure appropriate care and maintain active involvement in discharge planning throughout the consumer's stay with on-site visits as necessaryInitiate discharge planning on the day of admission to the facility, and schedule at minimum of biweekly meetings with the facility staff to assure a smooth transition at dischargeOversee the consumer's transition back to Elwyn by reviewing discharge recommendations, ensuring that medication/equipment is available, and providing staff training related to ongoing care and treatment needsMonitor ongoing medical/safety needs during the transition back to Elwyn, providing support as needed to ensure a smooth transition and placement at the facilityObtain and review all reports from all inpatient stays, Emergency Department visits, diagnostic procedures, and specialty appointments coordinating follow up health care needs as indicatedProvide nursing-on-call coverage as required, completing on call progress notes, documenting in the Electronic Health Record and forwarding to administration and the treatment team notification to ensure timely communicationComplete incident reports and Emergency Department/Hospitalization reports for Quality Improvement and Home and Community Services Information System/Enterprise Incident Management (HCSIS/EIM) system reporting purposesParticipate in meetings, committees, and seminars to ensure that nursing practice remains current with emerging trends in the care of the consumer with Intellectual Disabilities (ID)Complete all annual mandatory educational requirements as assigned by Elwyn within the designated timeframeOversee direct care staff in provision of healthcare services as assigned by the Health Care CoordinatorPerform cardiopulmonary resuscitation (CPR), first aid, and crisis intervention using agency-trained protocolsPerform other duties as assignedIntermediate Care Facilities (ICF)/ID-Specific Responsibilities
Complete quarterly Nursing Assessments and liaison with the Primary Care Physician/specialty providers regarding identified issuesReview Drug Regimen Reviews on a quarterly basis and liaison with the Primary Care Physician, Pharmacist, and treatment team to address the recommendationsPrepare semi-annual ICF Recertification and Utilization Review documents and review with consulting physician and QI representative; liaison with the Primary Care Physician to ensure medical review and sign offEnsure 90 day physician orders accurately reflect the current medication and treatment orders through review of orders, Medication Administration Records (MARs) and medication/treatment supplies on handCoordinate scheduling and completion of the Annual Nutrition Assessment with NutritionistProvide monthly consumer weights to the Nutritionist for ongoing monitoring purposes; report significant changes in the consumer's weight to the Primary Care Physician and NutritionistTranscribe new physician orders to MARIMMEDIATE SUPERVISOR: Senior Director of Community Nursing for PA and NJ

Elwyn Adult Behavioral Health Services seeks an Residential Services Assistant to be responsible for general daily care of program residents providing counseling and advocacy in a therapeutic setting and working with young adult through geriatric individuals with chronic mental illness and co-occurring disorders to achieve social, economic, emotional, and personal independence to live in the community.ESSENTIAL FUNCTIONS:Must work in the presence of an Associate Clinician at all times. Understand the signs and symptoms of mental illnesses and intervene therapeutically in a recovery-based environment.Knowledgeable of DSM classifications and conditions.Empower clients through advocacy and skill building.Exercise good judgment and decision making, particularly during crisis situations. Demonstrate knowledge of prescribed medications, an ability to monitor medications, and follow Elwyn Adult Behavioral Health Services state-specific medication guidelines.Demonstrate competency in the development, implementation, and modification of treatment plans, based on a comprehensive client assessment.Maintain client records in accordance with policy and procedures.Produce timely, accurate and thorough recordkeeping in accordance with agency, state or federal regulations.
