We are seeking a Registered Nurse RN to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.
Responsibilities:
Provide skilled nursing services in patients’ homes in accordance with physician orders and state regulations
Conduct comprehensive assessments and develop individualized care plans
Supervise and monitor patient care to ensure safety, quality, and compliance
Coordinate care with physicians, caregivers, and interdisciplinary team members
Perform OASIS assessments
Leadership and team support
Qualifications:
Previous experience in home health Ability to build rapport with patientsStrong problem solving and critical thinking skillsEnsure compliance with Indiana Department of Health RegulationsSupport survey readiness and participate in mock surveysMaintain accurate clinical documentatiom and chart auditsStrong assessment and care planning skills
