Teaches, supervises, and counsels the family and client in the physical therapy program. Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices. Supervises Physical Therapy Assistants and paraprofessionals at least once monthly. Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.Qualifications:
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association. Must be licensed or registered by the applicable State without restrictions. One year of home health experience required. Must have knowledge of Medicare and Medicaid guidelines Must have a valid driver's license and reliable transportation. Must have a current CPR certification.
