    Job Summary

    We are seeking a compassionate and reliable Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to join our Home Care team. You will provide one‑on‑one care in patient homes, following individualized care plans designed to support safety, comfort, and independence. The ideal candidate is patient‑focused, detail‑oriented, and committed to creating a healthy, supportive environment for every client. Strong communication skills and a genuine passion for helping others are essential. Prior Home Health, Hospital, Nursing Center or Hospice experience is a plus.

    Responsibilities

    Assist patients with daily activities, including transfers, bathing, dressing, grooming, toileting, and mobility.Support patients with household tasks such as changing bed linens, laundry, light cleaning, and meal preparation.Observe and report changes in patient condition, behavior, or needs to the supervising nurse.Maintain accurate records of care, progress, and concerns.Provide companionship and emotional support, including reading aloud, conversation, and engagement in mentally stimulating activities.Assist with range‑of‑motion exercises and other therapeutic activities as directed by the care plan.Note: CNAs/HHAs may assist with medication reminders but do not administer medications.

    Qualifications

    Completion of an approved CNA or Home Health Aide training program.Minimum one year of professional caregiving experience preferred.Valid Driver’s License.Current CPR certification.Strong communication, professionalism, and respectful interpersonal skills.Ability to work independently, follow care plans, and maintain patient confidentiality.Documents Required for Hiring

The role combines field-based business development with strategic marketing activities while maintaining ethical and regulatory compliance.

    Home Health RN/LPN – Job Description

    Job Summary
    We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Home Health Team.
    Full-time, Part-time, and PRN positions.  In this role, you will provide high‑quality, patient‑centered care to an adult or a pediatric client in the comfort of their home. The ideal candidate brings strong clinical skills, physiology knowledge, and experience in hospital, skilled nursing, home care, or community care settings. 

    This is an opportunity to provide individualize care and make a meaningful impact on the client and their family. While supporting the client in maintaining safety, independence, and dignity at home.

    Responsibilities

    •             Work12‑hour shifts, including overnights and weekends as needed.
    •             Provide direct skilled nursing care to adult and pediatric clients in their homes.
    •             Administer medications, monitor vital signs, and assist with daily living activities.
    •             Develop and implement individualized care plans to ensure safe, effective, and patient‑centered care.
    •             Collaborate with physicians, nurses, and interdisciplinary team members to support comprehensive care coordination.
    •             Document all care accurately using electronic medical records (EMRs).
    •             Utilize WellSky EMR (experience preferred; training provided).
    •             Maintain timely and accurate clinical documentation, including progress notes and discharge summaries. 
    •             Professionalism, strong communication skills, and respectful interpersonal skills.

    Requirements

    •             Current Florida RN or LPN license.
    •             Experience in a hospital, skilled nursing, or home/community care setting.
    •             Strong physiology knowledge and experience with airway management.
    •             Ability to travel to patients’ homes and provide care.
    •             Ability to work effectively in a fast‑paced environment while delivering high‑quality patient care.
